5.Long term noise exposure of steel mill workers, hearing loss and blood pressure.

A cross-sectional study was conducted to investigate the effect of long term noise exposure on blood pressure among steel mill workers. The workers participated in periodic medical examinations performed from August 27 to September 6 in 1990. Examined were 1,034 workers with high-level noise exposure(average 91.8+/-5.2 dB{AA)), and 390 workers with low-level noise exposure(average 75.2+/-4.6 dB(A)). No significant difference was found in systolic or diastolic blood pressure between the two exposure groups. Prevalence of hypertension (> or = 160 mmHg systolic or > or = 100 mmHg diastolic) was higher in a younger age group (< or = 40 years old) of high-level noise exposure than of low-level noise exposure. However, the difference was not statistically significant. Furthermore, in younger ages, prevalence of. hypertension appeared to be higher in the hearing loss group (> or = 25dB at 1000 Hz or > or = 40 dB at 4000 Hz in at least one ear) than in the normal hearing group. From multiple regression analysis, hearing loss, body mass index, age, alcohol and family history of hypertension were proven to be predictors of diastolic blood pressure (p<0.02). When regression was performed for each age group, hearing loss was a strong predictor of diastolic pressure in the younger age group (p<0.01).

8.Clinical Efficacy of Dermis-Fat Graft vs. Posterior Tenon's Capsule Suturing in Anophthalmic Orbit.

The anatomic and physiologic changes of the anopthalmic orbit affect cosmetic appearance of the patient and function of the socket and the prosthesis. During the past three years, the authors studied 13 cases of dermis-fat graft and 27 cases of posterior Tenon's capsule suturing with a plastic implant, and compared the post'operative effects of the two procedures. Dermis-fat graft produced good fornix formation with no remarkable fat atrophy after both primary and secondary proceddure. Posterior Tenon's capsule suture technique showes good effect to keep implant from migration or extrusion. Size of the plastic implant and fixation of the extraocular muscles were important factors also for good cosmesis and implant motility. There revealed no significant difference between the two procedures in EOM motility, enophthalmos, deep superior sulcus deformity, implant migration or prolapse etc.

Wilson disease is a rare genetic disorder of copper metabolism. Although CT findings suggest the disease when CNS pathologic changes are advanced, characteristic brain MR findings are reported to manifest early in the disease. 10.Photographic Analysis of Lip Repair in the Bilateral complete cleft lip and palate.

Characteristic residual deformities are thought to be unavoidable in most bilateral complete cleft lip and palate patients. Newly-adopted designs and modified surgical strategies were applied to 40 consecutive patients of bilateral complete cleft lip and palate and some characteristic deformities could be overcome. The future midline on the lateral lip segment was marked on the white roll of thickest vermilion and the alar base was transposed medially without circumalar incision. The prolabial width was reduced to 4-5 mm in consideration of further widening. To evaluate the outcome of bilateral cleft lip repair, 15 patients with adequate photographic records were assessed separately and overall by a panel of judges. Gross facial attractiveness of the visual analog recording scale correlated well with the cleft impairment scale. Full-bodied vermilion with tubercle was achieved and upper lip tightening and tower lip pouting could be avoided. Adequate philtral width was maintained over a 3-year follow-up. Nasal tip projection and proper alar position could be accomplished without circumalar incision. Cleft impairment scale showed impaired characteristics os short columella, no nostril sill, transverse nostril axis, flat and undimpled philtrum, convex-profiled upper lip and a downward drawn nose. Columellar length and nostril axis were expected to improve with growth and development. Absence of the philtral dimple and convex-profilled upper lip still remain to be solved. A reliable, easy to perform, and inexpensive evaluation protocol was provided for bilateral cleft lip repair and selected surgical procedures and postoperative outcomes could be assessed with photographic records.
