1.Evaluation of Cardioprotective Effects of DelNido Cardioplegia.

BACKGROUND: The aim of this study is to define the cardioprotective effects (functional and metabolic) of newly developed DelNido cardioplegic solution (containing plasma solution, mannitol, magnesium and lidocaine). MATERIAL AND METHOD: This study assessed the function of rat hearts after itermittent infusion of DelNido cardioplegia with different preserving methods(Air or Icebox) for 2hours and perfusing the hearts on a Langendorff apparatus. Heart rate, left ventricular developed pressure (LVDP) and coronary flow, were measured at pre-ischemic, post-reperfusion 15min, 30min and 45min. Coronary flow was standardized to dry heart weight. Each weight was weighted to calculate water content. Creatine kinase-MB isoenzyme release was measured and ultrastructural assessment was done with electron microscopes. DelNido group was better than St, Thomas group and Icebox group was better than Room-air group. CONCLUSION: DelNido cardioplegia have better myocardial protective effects than St. Thomas cardioplegia when they were preserved in the Room-air. But we can not tell the difference between Delnido cardiplegia with Air preserving method and St. Thomas cardioplegia with Icebox.

6.The CT examination of changes in intracranial hematoma density

The study was undertaken to asses the changes in the size and densities of intracranial hematomas by analyzingthe sequential CT examination of 23 patients ar Soon Chun Hyang College Hospital from July '80 to Aug. '81. Theresuls were as follows; 1. The high densities of hematoma were the cause of hematoma, hematoma locaiton, age andsex of patients, and initial hematoma size. 3. The CT findings that indicate complete absorption of hematomas werenormalization of mass effect, not diappearence of hematoma densities. 4. In intracerebral hematoma, the highdensity of hematoma reduced it's density and mass effect about 4-6 weeks duration. 5. In subdural hematoma, about7 weeks after head trauma, the CT findings were normalized.

7.Existance of cholinergic and purinergic receptor on the detrusor muscle of rat urinary bladder.

This study was aimed at investigation of the stimulatory innervations on the rat urinary bladder. Detrusor muscle strips of 15 mm long were suspended in isolated muscle chambers containing 1 ml of PSS maintained at 37℃ and aerated with 95% O²/5% Co². Isometric myography was performed, and the results were as followings: Muscle strips showed “on-contraction” by electric field stimulation (EFS) frequency-dependently. The EFS-induced contraction was not affected by hexamethonium, a ganglion blocker, but abolished by tetrodotoxin, a nerve conduction blocker. Physostigmine, a cholinesterase inhibitor enhanced the EFS-induced contraction which was inhibited by hemicholinium, an inhibitor of choline uptake at the cholinergic nerve ending. Such an EFS-induced contraction was antagonized by atropine only partially, and the atropine-resistant portion was completely abolished by the desensitization of purinergic receptors by prolonged incubating of the strips in the presence of high concentration of ATP. Bethanechol, a cholinergic agonist, elicited concentration-dependent contraction. Adenosine triphosphate (ATP), a purinergic agonist, induced a weak but concentration-dependent contraction of short duration. Bethanechol-induced contraction was not affected by ATP-desensitization, and ATP-induced contraction was not affected by tetrodotoxin. These results suggest that there are at least two main stimulatory components of innervations in the detrusor muscle, cholinergic muscarinic and purinergic; and those receptors are independent each other.

9.Clinical Analysis on the Outcome from the Treatment of Locally Invasive Cervical Cancer-Analysis of 195(Ia-IIb) Cases.

OBJECTIVES: The purposes of this clinical study were 1) to assess 5-year survival rates in 195 patients with locally invasive cervical cancer(stage Ia-IIb) treated by surgery, neoadjuvant chemotherapy+surgery and postoperative radiation in selected cases, 2) to evaluate the prognostic factors affecting the survival rate, and 3) to get helpful information for the better treatment. METHODS: A retrospective analysis was conducted of 195 patients. They were diagnosed and operated on during the period of Jan. 1988 - Dec. 1993 in Dept. of OB/Gyn, Pusan Paik Hospital, Inje Medical School. The 5-year survival rates were estimated using the Kaplan-Meier method, and prognostic factors were assessed using Wilcoxon test. RESULTS: The 5-year survival rates for FIGO stages Ia, Ib, IIa, and IIb were 100, 86.1, 76.9, and 81.1%, respectively. Factors that affect the 5-year survival rates were clinical stage(P=0.0001), cell type(small cell vs other, P=0.0001), depth of invasion(< or = 5 mm vs >5 mm, P=0.0013), tumor size(< or = 3 cm vs >3cm, P=0.0035), and lymph node metastasis(0 vs 1 vs more than 2, P=0.0001). There was no difference in 5-year survival rates between without neoadjuvant chemotherapy group and with neoadjuvant chemotherapy group which had poor prognostic factors. The predicted recurrence rate of the postoperative radiation group showed a lower 5-year survival rate than the no postoperative radiation group(P=0.0001). CONCLUSIONS: We conclude that the factors affecting the prognosis were FIGO stage, cell type, depth of invasion, tumor size, and lymph node metastasis, and speculated that the survival rates could be improved by using neoadjuvant chemotherapy for more advanced locally invasive cases properly.

10.Two Cases of Conservative Treatment of Cervical Pregnancy.

Implantation of a blastocyst within the endocervical canal is termed a"cervical pregn-ancy", which is one of the most dangerous forms of ectopic pregnancy. It is even less common, but the incidence appear to be increasing, in part due to newer forms of assisted reproduction. Proper management is indicated as soon as the diagnosis is suspected. Hemorrhage may be massive and sometimes fatal. In the past, hysterectomy was often the only choice available because of profuse hemorrhage that accompanied attempts at removal of the cerv- ical pregnancy. We report two cases of cervical pregnancy treated conservatively without hysterect- omy. Hemorrhage from the implantation site was controlled by intracervical placement of a Foley catheter balloon without hemostatic cervical sutures at 3 and 9 o clock. Immediate insertion of a Foley catheter into the cervical canal and inflation of the catheter bulb after endocervical suction curettage appears to be a simple and effective technique of managing cervical pregnancy in a patient who wishes to maintain childbearing capacity.
