3.Subacute Bacterial Endocarditis and Cerebral mycotic Aneurysm.
Journal of the Korean Medical Association 2001;44(11):1225-1231
A 30-year-old man was transferred to Yonsei Cardiovascular Hospital after 20 days of hospitalization under the diagnosis of bacterial endocarditis with mitral regurgitation and septic splenic infarct. He had suffered from intermittent high fever and general weakness for 3 months. Echocardiogram showed moderate mitral regurgitation and multiple large vegetations in both leaflets. Streptococcus viridans was confirmed by blood cultures, and administration of cefazoline and gentamycin was initiated. During the antibiotic therapy, abdominal and left calf pain developed, and abdominal computed tomography showed splenic and right renal infarct. He also complained intermittent headache and visual distrubance. Brain computed tomography and four vessels angiography showed a small aneurysm in right hemisphere and other small aneurysm in fronto-occipital area of left middle cerebral artery. No more systemic embolization developed until the completion of six weeks of antibiotic therapy. The patient underwent mitral valve replacement with #29 Carpenitier-Edwards Bioprosthesis. At operation, the anterior and posterior mitral leaflets were found perforated and covered with leaflets were found perforated and covered with multiple vegetations. Follow-up cerebral angiography taken at seventh postoperative day, showed enlargement of cerebral aneurysm. Clipping of saccular aneurysms on left M3 and M4 were performed and complete resolution of aneurysm was confirmed by brain computed tomography. On operative finding, an aneurysm was found on with saccular type. The patient was dischanged without complication on eighth postoperative day.

4.Trends of Sexually Transmitted Diseases During Recent 4 Years: among Beneficiaries of a Health Center in Seoul.
Journal of the Korean Academy of Family Medicine 1997;18(2):159-168
BACKGROUNDS: Sexually transmitted disease is class 3 major communicable disease group in Korea, and nationwide surveillance is required to control it. It is necessary to investigate the trends and current characteristics of STDs for effective control. METHODS: We analysed the laboratory data of those who visited a health center in Seoul for venereal disease examination and had abnormal results from Jan 1, 1991 to Dec 31, 1994. Venereal disease examination was composed of serum VDRL(quantitative), Gram staining and culture of urine or urethral discharge. RESULTS: There were 1196 male cases(97.6%) among the total 1226 cases. Mean age was 31.1+/-8.4, and the most common age group was 25-34 yr-old(60.8%). Diagnosis was as follows : syphilis 48 cases(3.9%), gonorrhea 388 cases(31.6%), Nongonoccocal urethritis 773 cases(63.1%), others 10 cases(0.8%), mixed infection of syphilis and gonorrhea 2 cases(0.2 %), mixed infection of syphilis and NGU 5 cases(0.4%). The proportion of PPNG among gonorrhea was as follows : 1991 48.1%, 1992 38.0%, 1993 32.9%, 1994 58.8%. The fraction of NGU among whole urethritis patients increased year by year, 1991 68.0%, 1992 69.4%, 1993 73.3%, 1994 55.3%. There were more WBCs on Gram staining of gonorrhea cases than NGU cases(p<0.01). CONCLUSIONS: The STD patients among beneficiaries of a health center were mainly young male. Common diagnosis was as follows, NGU, gonorrhea, syphilis. The proportion of PPNG among gonorrhea decreased initially, then increased in 1994, and the fraction of NGU among whole urethritis cases increased initially, then increased in 1994.

Journal of the Korean Academy of Family Medicine 1997;18(10):996-1005
BACKGROUND: The quality of specimen for Papanicolaou smear depends on the sampling methods of uterine cervix. This study was designed to test specimen adequacy and the detection of disease using t,wo cervical cytological sampling methods. pastic spatula plus Cytobrish and Cervex-Brush. METHODS: Research subjects were t.he exarninee for screening for cevical cancer in Farnily Practice Center and Health Promotion Center of University Hospital. Research sub jects were classified to two groups randomly. 5 physicians used each device at random. A group 80 test smples(plast,ic spatula plus Cytobrush) were compared with B group test samples(Cervex Brush) for the presence rate of endocervial cells and specimen adequacy. The laborat.ory was blind to the sampling met, hods. RESULTS: The plastic spatula plus Cytobrush and Cervex-Brush were comparable in cap- turing in endocervical cells. But the capturing rates of both methods were high(over 70%). Both the pastic spatula plus Cytobrush and Cervex-Brush produced adequate samples, and the adequacy scores between two methods were not different. significantly. CONCLUSIONS: Use of the plastic spatula puls Cytobrush and Cervex-Brush produced adequate smear for interpretation.

Journal of Korean Academy of Adult Nursing 2000;12(4):606-618
The purpose of the study was to investigate the characteristics of the clients registered in the department of home health care nursing in a hospital and to analyze nursing intervention activities recorded in charts by application of Nursing Intervention Classification (NIC) system. For the descriptive survey study, data were collected by reviewing charts of 572 home health care clients between May, 1997 and July, 2000 at K hospital in Seoul. The average age of the clients was 66 years and the number of clients in their 70s ranked first with 28.2 percent(158 people). The mean length of home care service was 47 days with the highest frequency of less than four weeks (56 %). With regard to medical diagnosis, cancer showed the highest frequency (48%, 271 people), followed by cerebrovascular disease (19%), and pulmonary disease (6.9%). According to analysis of nursing interventions by the NIC system, the most frequently used nursing interventions in level 1 were interventions in the Physiological: Complex domain which were used 3,663 times (33%) among 11,107 total interventions. The Safety domain was the second most frequently used intervention, followed by the Physiological: Basic, and the Behavioral domains. In level 2, the Risk Management class was the most frequently used interventions with 3,108 interventions (27.9%), followed by Drug Management, and Tissue Perfusion Management classes. In level 3 interventions, Vital Sign Monitoring was the most frequently used intervention, 569 times (5.1%), followed by Health Screening, and Neurological Monitoring interventions. In sum, half of the clients in the study had cancer and were in their 70s. The most frequent reason for ending home care was death (40%), followed by readmission (28%). These findings represent clients with severe conditions referred to the home care nursing department as it was a University teaching hospital. Further research on analyzing nursing interventions performed in each institution needs to be conducted to develop a standardized list of nursing interventions to use in home health care settings.
