1.Cord Blood Leptin of Newborns with Normal Intrauterine Growth in 30-42 Weeks of Gestation.

PURPOSE: To evaluate leptin values in placental cord blood of newborns with normal intruterine growth after 30 weeks of gestation and to study the relationship of leptin with gestational age, birth size, ponderal index, BMI, placental weight, gender difference, Insulin-like Growth Factor-I(IGF-I) and Insulin-like Growth Factor binding protein-3(IGFBP-3). METHODS: Ninety healthy newborns(49 males and 41 females) with normal intrauterine growth were studied. They were classified into 4 groups according to the gestational age(GA):group I(30-33 weeks GA, n=17), group II(34-36 weeks GA, n= 24), group III(37-39 weeks GA, n=32) and group IV(40-42 weeks GA, n=17). At the time of delivery, newborn infant' weight, length, head circumference and placental weight were measured and BMI, ponderal index were calculated. A venous cord blood sample was collected at the time of delivery and serum leptin(RIA), IGF-I (IRMA) and IGFBP-3(IRMA) were measured. RESULTS: Cord blood leptin correlated positively with gestational age(r=0.730, P>0.01), body weight(r=0.686, P<0.01), length(r=0.419, P<0.01), head circumference(r= 0.498, P<0.01), BMI(r=0.750, P<0.01), ponderal index(r=0.704, P<0.01), placental weight(r=0.536, P<0.01), IGF-I(r=307, P<0.01) and IGFBP-3(r=0.736, P<0.01). Multiple linear regression analysis showed BMI, IGFBP-3 and sex(female) were independent predictors of leptin values. Leptin values increased progressively throughout gestation. Mean leptin values of group II(3.90+/-.28ng/mL) were significantly higher (P<0.01) than those of group I(1.92+/-.09ng/mL) and also values of group III(5.37+/-.52ng/mL) were higher than those of group II(P<0.05). But there was no significant difference between group III and group IV(6.45+/-.36ng/mL). Mean leptin values in females(5.07+/-.54ng/mL, n=41) were significantly higher than in males(4.07+/-.76 ng/mL, n=49) but there was no inter-gender difference in body weight, BMI, ponderal index, IGF-I, IGFBP-3. In addition, mean leptin values in females(7.45+/-.07 ng/mL) were significantly higher than in males(5.02+/-.09ng/mL) after 40 weeks of gestation. CONCLUSION: BMI, IGFBP-3 and sex(female) were independent predictors of leptin values. Leptin values increased significantly after 34 weeks of gestation and females had higher leptin values than males, especially in fullterm newborns. Leptin values in newborns had higher correlations with IGFBP-3 than with IGF-I as gestational age increased.

3.A Case of Varicella in a Full Term Pregnant Woman.

Varicella during pregnancy is a threat to both mother and fetus because of disseminated infection, varicella pneumonia and meningitis etc. And about 10% of babies acquire intrauterine infection, as indicated by the congenital varicella syndrome, neonatal varicella or zoster during infancy. We report a case of varicella in a 30-year-old, 39 weeks and 3 days' gestational age, full-term pregnant woman presented with generalized erythematous papules, vesicles, pustules and umbilicated pustules on the whole body. She was ordered absolute bed rest for delaying the delivery and treated with acyclovir and varicella-zoster immune globulin. However, she delivered a healthy baby which weighed 3,350 gm the next early morning. In our investigation of cord blood after delivery, varicella-zoster IgG and IgM by ELIZA method were negative and varicella-zoster virus DNA by polymerase chain reaction was also negative. The newborn revealed no clinical evidence of skin lesions and anomalies of varicella. So we supposed there was no intrauterine varicella infection.

4.Pineal Anlage Tumor: A case report.

The term "pineal anlage tumor" has been recently proposed and few cases have been reported. We report the first Korean case of pineal anlage tumor in a 6-year-old girl who complained of headache and vomiting for 2 months. Brain MRI revealed a well defined, lobulated, calcifying mass in the pineal region. Tumor was totally removed. Pathological examination revealed a primitive pineal parenchymal tumor with melanotic epithelial component that was similar to histologic findings of melanotic neuroectodermal tumor of infancy, so-called retinal anlage tumor and of the developing pineal gland. The tumor was composed mostly of small, undifferentiated cells, Flexner-Wintersteiner rosettes, and ganglionic differentiation. The tumor also contained the cartilage and skeletal muscle cells.

5.Pulmonary Lymphangiomyomatosis: A case report.

Pulmonary lymphangiomyomatosis is a rare disease of haphazard overproliferation of smooth muscle from perilymphatics, peribronchial and perivascular regions of the lung that leads to chylous effusion, hemoptysis, spontaneous pneumothorax and pulmonary insufficiency. The disease only affects women of child-bearing age. There is presently no successful therapy and most of the patients die within 10 years of diagnosis. We experienced a case of pulmonary lymphangiomyomatosis in a 30-year-old woman who had suffered from three episodes of spontaneous pneumothorax for 4 years. Open lung biopsy was performed. Microscopically, abnormal excess proliferation of smooth muscle tissue was identified in the wall of perivascular lymphatic channels as well as around small bronchi, and more distal air spaces. Varying sized cytic spaces Iined by cuboidal cells were also present. Tamoxifen, given if February 1991, did not influence the course of her disease. She died of respiratory failure at age of 31 in April 1991.

6.The Effect of Hypobaric Priming Solutions on Extracorporeal Circulation during Open Heart Surgery.

Before beginning the extracorporeal circulation, perfusionists should supply oxygen into the oxygenator and establish blood flow through the blood line of the heart-lung machine. But these manipulation can induce severe hypocarbic state of priming solutions due to wash out of CO2 gas in the solution. This study was carried out to examine the relationship of blood gas changes between hypocarbic priming solutions and body circulation in 15 patients undergoing open heart surgery with extracorporeal circulation. PaCO₂, pH, buffer base and PaO2 were measured from priming solutions before and 15 minutes after the extracorporeal circulation. The results were as follows; 1) Before the extracorporeal circulation, mean PaCO₂ level was 12.1±7.8 mmHg in the priming solution. However, 15 minutes after extracorporeal circulation, the PaCO₂ level was maintained at 35.7±5.7 mmHg. 2) pH in the priming solution was variable from 6.93 to 7.99 (mean 7.45±0.29), but after 15 minutes it was ranged from 7.28 to 7.42 (mean 7.35±0.05). 3) Mean buffer base level in the priming solution was 7.9±3.5 mmol/l. but after 15 minutes, it was 19.6±1.2 mmol/l. 4) Mean PaO₂ level in the priming solution was 667.1±45.6 mmHg, but after 15 minutes, it was 280.7±131.7 mmHg.

7.Effects of Starvation and Perioperative Fluid Therapy on the Blood Glucose Concentrations during Anesthesia in Children.

This study included 38 children patients of less than 4 years old and 18 kg body weight. After 8 hours of starvation, the children were divided into 2 groups: Group I received Hartmann's solution and Group II received Hartmann's dextrose solution. In both groups, the rates of infusion were 10 ml/kg/hr before and during operation and blood samples were collected just before and 1 hr after induction of anesthesia, respectively. The results were as follows; 1) In the Group I, blood glucose concentration just before induction was decreased than control values that was checked at ward, and 1 hr value after induction was significantly increased then control values. 2) In the Group II, blood glucose concentration was increased just before and 1hr after induction than control values significantly respectively. 3) In the blood glucose concentration 1 hr after induction, difference between Group I and Group II was not significant. 4) In children, duration of starvation about 8 hrs did not significant influence on blood glucose concentration although dextrose was not administered.

8.Epstein-Barr Viral RNA(EBERs) Expression in Conventional Malignant Lymphoma and Polymorphic Reticulosis of Upper Aerodigestive Tract.

The author examined the immunophenotype and expression of Epstem-Barr virus RNA (EBERs) used in the situ hybridization technique in 20 cases of conventional malignant lymphoma and 28 cases of polymorphic reticulosis and malignant lymphoma with features of polymorphic reticulosis occured in the upper aerodigestive tract including the upper digestive tract(palatine tonsil), and upper respiratory tract(nasal cavity, paranasal sinus, nasopharynx). The results obtained were summarized as followings; 1. The favorable site of malignant lymphoma that occured in the upper aerodigestive tract was in the palatine tonsil(11 out of 20 cases, 55%), those of polymorphic reticulosis and malignant lymphoma with features of polymorphic reticulosis were nasal cavity and nasopharynx(19 out of 28 cases, 78%). 2. The immunophentype of malignant lymphoma that occured in the upper aerodigestive tract was mostly B-cell phenotype (15 out of 20 cases, 75%), and that of polymorphic reticulosis and malignant lymphoma with features of polymorphic reticulosis was predominantly T-cell phenotype(22 out of 28 cases, 79%). 3. The EBERs positivity of malignant lymphoma that occured in the upper aerodigestive tract was 25%(5 out of 20 cases), but that of polymorphic reticulosis and malignant lymphoma with features of polymorphic reticulosis was 57%(16 out of 28 cases). 4. The positive cases for EBERs revealed angiocentricity with necrosis(16 out of 21 cases, 76%), predominantly T-cell phenotype(19 out of 21 cases, 90%), and favorably involved the nasal cavity and nasopharynx(16 out of 21 cases, 76%). Based on the above results, it was concluded that polymorphic reticulosis and malignant lymphoma with features of polymorphic reticulosis that occurred in the upper respiratory tract was an EBV-positive angiocentric T-cell lymphoma favorably involving the nasal cavity and nasopharynx.

10.Morphologic Comparisons of Focal Segmental Glomerulo-sclerosis between Human and 5/6 Nephrectomy Rat Model.

This study was conducted to set up a common mechanism for varying phases of focal segmental glomerulosclerosis(FSGS) by comparing the morphological differences between human FSGS and changes in 5/6 renal ablation animal model, which has been accepted as experimental prototype for hyperfiltration theory as pathogenesis of FSGS. Both the human and the experimental rats showed very similar changes such as segmental glomerulosclerosis, vacuole formations or inclusion of small granules of podocytes, appearance of foamy cells in the capillary lumina, eosinophilic deposits along the mesangial area, and focal atrophy of tubules with associated interstitial fibrosis. The halo, frequently seen in human FSGS, is due to detachment of visceral epithelium from basement membrane, however, did not appear in the experimental rat specimen. On the other hand, the foamy cells and hyalinization were more frequently noted in the rat series and even involved the arterioles. The mesangial proliferation never appeared in the rat series occasionally found in human FSGS. In conclusion, the pathogenesis of FSGS cannot depend solely on the hyperfiltration theory of hemodynamic derangement, but has complex impairment of visceral epithelium and cells forming the constituents of basement membrane.
