1.A Case of GEneralized Pustular Psoriasis.

We have presented a case of generalized pustular psoriasis appearing in a 18 year-old femaIe patient wbo suddenly developed annular lesions on the lower abdomen and crural areas with erythematous, scaly and pustular margin, following mild febrile attack. Histopathological findings showed typical spongiform pustule of Kogoj and Munro macroabscess. All the skin lesions were almost cleared with systemic corticosteroid and methotrexate in 3 weeks.

2.Clinical Significance of N-Acetyl-beta-D-glucosaminidase in the Patients of Non-insulin-dependent Diabetes.

BACKGROUND: Routine renal function tests are not sensitive enough to detect early renal complication of diabetes. To detect the complication as soon as possible, we measured urine N-Acetyl-beta-D-glucosaminidase(NAG) and evaluated in comparison with microalbumin and beta2-microglobulin(beta2-MG). METHODS: 87 patients with type II diabetes visited Catholic University Hospital of Taegu Hyosung during the period October 1995 to March 1996. We collected 24 hour urine samples and measured NAG, albumin excretion rate (AER), beta2-MG. urinalysis, BUN, creatinine(Cr) Cr clearance(CrCl), fasting and 2 hour postprandial blood sugar and hemoglobin A1c. RESULTS: The average age of the patients was 53+/-15 years old and their average disease duration was 5.8+/-5.0 years. Abnormal rates of each renal function tests were as follows : NAG/gCr 52.1%, AER 51.7%, CrCl 42.5%, BUN 18.4%, beta2-MG 13.8% and creatinine 6.9% in order. From 36 patients whose AER was within normal limit, 13 of them(36.1%) showed increased level of NAG/gCr. Of 38 patients with increased NAG/gCr results, the 31 patients (81.6%) recorded abnormal results of renal function tests. Among 87 patients studied 60 patients(68.5%) showed increased level of NAG/gCr or AER results. Compared with AER test alone. the combined tests with NAG/gCr increased 16.8% of detection rates of renal complication in type II diabetes. CONCLUSION: Urine NAG/gCr and AER tests were very useful for detecting the early renal complication of type n diabetes. As increase of NAG/gCr suggest the proximal tubule damage, it is necessary to have further evaluation about the proximal tubule damage of renal complication in type II diabetes.

3.A Clinical Study on Dermatitis Herpetiformis.

A clinicopathological study was performed with 20 cases of dermatitis herpetifom is encountered at Department of Dermatology, College of Medicine, Busan National University Hospital during the period from January 1969 to August 1977. We used as criteria for diagnosis the history of chronicity and recurrence with good general health, pruritus, symmetric grouping of vesicles and papules, excoriat ions, pigmentations and ready response to DDS. The results were as folIows; 1.The age of onset was varied from 5 to 74 with most; prominent in the age group between 20 and 29 years. 2. Clinical forms included 16 cases of papulovesicular form, 1 case of papular form and, 3 cases of vesicobullous form. 3. The common predilection sites were scapular area, back, sacral area and extensor surfaces of extrernities while oral mucosa was affected in only one case. 4. Histopathological findings of 16 biopsy specimen obtained from the early vesicular lesions and surrounding erythematous areas showed subepidermal bullae in 12 cases and microabscess in 3 cases. 5. Most of the patients (80%) complained either moderate or severe pruritus.

6.The Effects of Self-efficacy and Social Support on Depression in Elementary School Children.

PURPOSE: The purpose of this study was to investigate the effects of self-efficacy, social support, and depression in elementary school children. METHODS: The participants of this study were 619 students from seven elementary schools located in Jollabuk-do Province, Korea. The methods included data analysis, descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficients and stepwise multiple regression using SPSS/WIN version 15.0. RESULTS: The mean scores of self-efficacy, social support, and depression were 2.9+/-0.58, 3.2+/-0.44 and 14.0+/-7.32, respectively. There was a significant positive correlation between self-efficacy and social support (r=.517, p<.001). Significant negative correlations were observed between self-efficacy and depression (r=-.557, p<.001) and between social support and depression (r=-.571, p<.001). The variables of depression in elementary school children were self-efficacy and social support, which explained 42.7% of the cases. CONCLUSION: The findings from this study suggest that current intervention programs are not sufficient to curb depression in elementary school children and therefore it is needed to develop nursing intervention programs to promote self-efficacy and social support.

7.A Case of Acne Conglobata.

A case of acne conglobata associated with hidradenitis suppurativa, dissecting cellulitis and disseminated superficial actinic porokeratosis in 44 year-old man is presented with brief review of literature. The patient had acne vulgaris for a few years during his highschool days, about 5 years prior to the development of such a typical lesions of acne conglobata as multiple comedones, papules, pustules, abscesses, cysts and draining sinnses and subsquent hypertrophic scars and keloids. We are ps,rticularly interested in association of hidradenitis suppurativa and dissecting cellulitis with ance conglobata that comprise follicular occlusion triad in tbis teint. The pstient was treztei with systemic antibiotics and corticosteroids injections and surgical interventions with considerable improvement.

8.Use of Mammary Lymphoscintigraphy and Intraoperative Radioguided Gamma Probe in Sentinel Lymph Node Biopsy of Breast Cancer.

PURPOSE: The sentinel lymph node is defined as the first draining node from a primary tumor and reflects the histologic feature of the remainder of the lymphatic basin status. The aim of this study was to evaluate the usefulness of lymphoscintigraphy and intraoperative radioguided gamma probe for identification and removal of sentinel lymph node in breast cancer. MATERIALS AND METHODS: Lymphoscintigraphy was performed preoperatively in 15 patients with biopsy proven primary breast cancer. Tc-99m antimony sulfide colloid was injected intradermally at four points around the tumor. Imaging acquisition included dynamic imaging, followed by early and late static images at 2 hours. The sentinel lymph node criteria on lymphoscintigraphy is the first node of the highest uptake in early and late static images. We tagged the node emitting the highest activity both in vivo and ex vivo. Histologic study for sentinel and axillary lymph node investigation was done by Hematoxylin-Eosin staining. RESULTS: On lymphoscintigraphy, three of 15 patients had clear lymphatic vessels in dynamic images, and 11 of 15 patients showed sentinel lymph node in early static image and three in late static 2 hours image. Mean detection time of sentinel lymph node on lymphoscintigraphy was 33.5+/-48.4 minutes. The sentinel lymph node localization and removal by lymphoscintigraphy and intraoperative gamma probe were successful in 14 of 15 patients (detection rate: 93.3%). On lymphoscintigraphy, 14 of 15 patients showed 2.47+/-2.00 sentinel lymph nodes. On intraoperative gamma probe, 2.36+/-1.96 sentinel lymph nodes were detected. In 7 patients with positive results of sentinel lymph node metastasis, 5 patients showed positive results of axillary lymph node (sensitivity: 72%) but two did not. In 7 patients with negative results of sentinel lymph node metastasis, all axillary nodes were free of disease (specificity: 100%). CONCLUSION: Sentinel lymph node biopsy with lymphoscintigraphy and intraoperative gamma probe is a reliable method to predict axillary lymph node metastasis in breast cancer, and unnecessary axillary lymph node dissection can be avoided.

9.Histological Changes of Innervated Muscles after neourolysis: An Experimental Study on Rabbit Sciatic Nerve

The normal vascularization of peripheral nervesand the vascular factor in peripheral nerve les ons have regained increasing interest among surgeons. So, several attempts have been made to assess the relative importance of the vasa nervorum and intrinsic longitudinal vascular plexuses of nerve in maintaining the blood supply.of a segment of nerve trunk. The purpose of our experiment was to determine in laboratory animals the maximum extent to which a nerve can be mobilized without impairing its vascular supply so much that nerve function is jeopardized. All our studies were carried out on both sides of the sciatic-tibial nerve of thirty-two rabbit which were anesthetized intraperitoneally with urethane. The experimental procedure differed in three groups, Group I in which the sciatic nerve was mobilized 3 cm in length, Group II was mob lized 7 cm length and Group III was mobilized 10cm long. The tibialis post. muscles of each group were analyzed at intervals ot 1,2,4,6 and 8 weeks after neurolysis. Each muscles were examined grossly and histologically after hematoxylin and eosin staining. Experimental studies showed that a peripheral nerve is a well vascularized structure with a considerable reserve capacity in its microirculation. The intrinsic collateral system is well developeed and experimental deta supported the view that peripheral nerves may be mobilized over a cons derable length with or the only minium interference with their microvascular flow. The results were as follows: 1. The first evidence of histologic change in the muscle fibers was in the sarcolemmal neclei. 2. Localized atrophy of muscle fibers were observed at the six weeks after neurolysis. 3. With increasing length of neurolysis, abnormal finding were developed in early stage. 4. Massive atrophy of muscle fibers were noted in the muscle fibers which neurolysed more 7cm.
