2.Effect of Tolamolol on the Cardiac Arrhythmias induced Halothane Anesthesia.

There are many reports of ventricular arrhythmias following the injection of epinephrine under general anesthesia with halothane or cyclopropane. Raventos reported that catecholamines given animals during chloroform, cyclopropane or halothane anesthesia have caused ventricular arrhythmias which sometimes developed into ventricular fibrillation. But there are a few reports of atrial arrhythmias in patients under general anesthesia with alone halothane. While it is known that hypercapnia can elicit arrhythmia during halothane anesthesia, Black and coworkers that hypercapnia of 60 to 140mmHg (average 92mmHg) is necessary for the occurrence of arrhythmias during halothane anesthesia. Hellewell and Potts reported 12 cases of arrhythmias under halothane anesthesia which were treated by propranolol, producing a return of sinus rhythm in an average time of 46 seconds. The authors reported one case with atrial arrhythmias induced by halothane anesthesia, which were arrested by the IV injection of 5mg of tolamolo, beta-receptor blocking agent. Thereafter arrhythmias did not reappear.

3.Epidemiologic Study of Enterobacter cloacae Infection in Neonatal Intensive Care Unit.

Enterobacter cloacae is isolated from 10 newborns in the neonatal Intensive care unit(NICU) of Seoul Red Cross Hospital from April, 1997 to June, 1997. Their gestational age is from 195-299 days(mean 244.3 days), and their birth weight was from 980-4,100g(mean 2,287g). We obtained 16 E, cloacae strains form their blood, urine, endotracheal tube, umbilical catheter and 4 of them were isolated from the same patient at different culture sites. We also obtained another 3 strains of E. cloacae from 14 persons who worked in NICU and 26 places of NICU. We analysed the molecular types of 19 isolates by using rep-PCR method. We get 6 different types of PCR-products from the patients and NICU workers, and NICU environments. We observed that the incidence of E. cloacae was decreased by handwashing and wearing personal plastic gloves during the care of NICU patients.

6.Correlation Between the Expression of Epidermal Growth Factor Receptor and MR Features in Glioma.

PURPOSE: The aim of this study was to find correlation between the expression of epidermal growth factor receptor (ECFR) and MR findings in the brain glioma. MATERIALS AND METHODS: MR features including edema, margin, necrosis, heterogeneity, hemorrhage and contrast enhancement were retrospectively analyzed with preoperative MR images in 41 patients with proven brain gliomas (8 low grade astrocytomas, 12 anaplastic astrocytomas, 21 glioblastoma multiformes). Immunohistochemical study of EGFR was done and their expressions were graded by both stained distribution and intensity. Correlation analysis between the MR features and EGFR expressions was done. RESULTS: Peritumoral edema was correlated with both distribution (r=0.71, p=0.00) and stain intensity (r=0.69, p=0.00) of EGFR expression. Other MR features showed no statistical correlation with EGFR expression. CONCLUSION: MRI is useful in evaluation of brain glioma, and peritumoral edema is useful findings that suggests EGFR expression as well as malignant histopathologic grade of the tumor.

7.Inflammatory Gastric Adenocarcinoma with Neutrophil Leukcytosis.

A 36-year-old man received a subtotal gastrectomy for Borrmann type 2 advanced carcinoma in the antrum of the posterior well. Preoperatively, peripheral white blood cell count was 16,400/mn3 with 80% of neutrophils. Microscopically, the tumor was of a poorly differentiated adeocarcinoma with intense eutrophilic infiltration which was totally confined within the neoplastic portion and in the metastatic focus in the regional lympho nodes. Repeated postoperative whits blood cell counts dropped down to the normal level. Neither infectious nor knowledge, this is the first case of gastric carcinoma with massive neutrophilic stromal reaction(inflammatory carcinoma) and leukocytosis in the literature in the korea.

9.Clinical Experience of Thumb-in-palm Deformity

A case of thumb-in-palm deformity due to burn scar contracture. Recently it is felt that the incidence of thumb-in-palm deformity due to old burn scar contracture is far less than that of other causes; for example carebral palsy, trauma, Co. poisning etc; This case is a 23 years old male soldier who had sustained burn on the left palm at his age of 4, and the left thumb has been gradually contracted, resulting to established thumb-in-palm deformity evantually. There is no active or passive motion in the metacarpho-phalangeal phalangeal joint and carpho-metacarpal joint of left thumb. The distal phalanx is moderately atrophied and its nail is deformed. On act 6th, 1975 he was operated on soft-tissue releasing with Z-plasty fasciotomy of palmar aponeurosis and myotomy of adductor pollicis and flexor pollicis brevis with split thickness skin graft. Postoperatively he could have full extension of the left thumb as well as active flexion of metacrpho-phalangeal and interphalangeal joint with slight adducted state. He is able to take opposition of left thumb at the present time postoperative four weeks.

10.Analysis of Operative Treatment of 154 Herniated Intervertebral Disc in Lumbar Spine

One hundred and fifty-four operative cases of herniated intervertebral disc were evaluated in department of Orthopedic Surgery, National Police Hospital from June 1973 to May 1979. The results were obtained as follows: 1. Out of the 154 patients, there were 137 male and 17 female cases. The ratio between male and female was 8:1. 2. The majority (72%) of the herniated intervertebral disc was found in the age group of 20 to 40 years. 3. The most common site of the lesion was between L4 and L5 vertebrae (70%). 4. About 90% of the patient showed significant sign in myelographic and clinical studies. 5. To prevent adhesion of sciatic nerve & dura after the operation, spraying the methyl-prednisolone over the operative site and covering dura with fat in spinal canal could be resulted in good method. 6. The dissecting type is considered to be what operators should observe attentively during the operation. 7. The result of treatment was excellent in 94 cases (61%) and good in 47 cases (30%).
