Carcinoma, Bronchogenic*

Sex cord tumor with annular tubules (SCTAT) is a rare ovarian neoplasm which usually occurs in two forms. In patients associated with Peutz-Jeghers syndrome, the tumors are usually small, bilateral or multifocal, and show benign clinical course. However, tumors from patients without the syndrome are often large, usually unilateral, and rarely show malignant behavior. Serous surface papillary carcinoma (SSPC) is an aggressive neoplasm which involves peritoneal linings, including ovarian surface. Recently, we encountered a case of an unusual combination of SCTAT and SSPC in the ovary of a 55-year-old Korean woman presented with abdominal distention for one year. Systemic review and physical examination were within normal limit, except for abdominal discomfort and distention. There was no stigmata of Peutz-Jeghers syndrome in all diagnostic examinations, including gastroscopy and colonoscopy. Pelvic computed tomography showed adnexal mass with multiple peritoneal nodules. Exploration revealed uterine and ovarian surfaces covered with multiple, yellow-white papillary nodules. However, the sizes of both ovaries were within normal limit. Typical serous papillary carcinomas were identified in nodules from peritoneum and ovarian surfaces. Well-circumscribed columnar epithelial cell nests composed of ring-shaped tubules encircling hyalinized basement membrane-like materials were found in the ovary away from serous surface papillary carcinoma.

Focal nodular hyperplasia (FNH) is a rare, benign hepatic tumor which was usually discovered incidentally by imaging procedure performed for some other reasons. FNH is typically asymptomatic and, it seldom bleeds. There is no evidence to support any relation with primary liver cancer. Accordingly, the preferred management is conservative, and excision is reserved for large symptomatic and complicated lesion, or when the diagnosis remains uncertain. Although many cases of FNH has been described to date in the other countries, only four cases of FNH has been reported in Korean literature. In the present report we describe a 7 cm sized asymptomatic lesion of FNH in a 23-year-old woman, that was disclosed by various kinds of imaging procedure. The left lateral segmentectomy was performed. The mass was firm and showed areas of localized growth of mature hepatocytes and septal fibrosis accompanied with marginal ductal proliferation, consistent with FNH. It also displayed an incomplete stellate architectual configration consisted of a central fibrous scar.

Unilateral renal agenesis is uncommon congenital condition with important implication when operation is performed. We report on two cases of unilateral renal agenesis but blind loop of ureter in a 2 year male child with vesical diverticulum and a 8 year old girl with contralateral hydronephrosis. An excretory urogram of the former revealed nonvisualization of left kidney and normal right kidney, it of the latter showed marked hydronephrosis of right kidney and nonvisualization of left kidney. These cases were finally confirmed by exploratory surgery with transvesical retrograde pyelography and histopathological findings of resected surgical specimen.

A retrospective analysis of 108 patients who had undergone transurethral prostatectomy for benign prostatic hyperplasia between January 1988 and December 1992 was performed to evaluate risk factors influencing intraoperative and postoperative morbidity and mortality. Of 108 patients complications occurred in 32 cases, with the morbidity rate of 29.6% but no death occurred. The most common postoperative complication was bleeding in 9 cases(8.3%) followed by incontinence in 7 cases(6.5%), capsular perforation in 4 cases(3.7%), failure to void in 4 cases(3.7%) and urethral stricture in 4 cases(3.7%). Risk factors which increased the morbidity of transurethral prostatectomy were age greater than 75 years and the presence of associated medical disease(p<0.05) but a resection time of more than 90 minutes, weight of resected tissue more than 30 gram and amounts of irrigating solution of more than 20 L did not increase the postoperative morbidity significantly. In conclusion, meticulous preoperative and postoperative cares are necessary because poor general condition increase the postoperative complications and long-term, prospective randomized studies are required to evaluate risk factors influencing postoperative morbidity after transurethral prostatectomy.

PURPOSE: Mycoplasma pneumoniae pneumonia is one of the most common community- acquired pneumonias in childhood. Recently, it has tended to occur at a young age. We reviewed clinical manifestations of M. pneumoniae pneumonia in the young. METHODS: A total of 378 children who were admitted to the Bundang Jesaeng Hospital due to M. pneumoniae pneumonia from 2003 to 2006 were retrospectively analyzed. M. pneumoniae infection was confirmed by mycoplasma antibody titer >1:320 or four-fold increase of titer. The children were divided into an infant group (Group A, 0-4 years) and an older children group (Group B, 4-14 years). We analyzed the differences between the two groups according to incidence, clinical manifestations, laboratory and radiologic findings. RESULTS: The number of patients in Group A was 147 (38.9%) and in Group B, 231 (61.1%), respectively. Seasonal distribution showed the highest frequency in autumn and the lowest frequency in spring in both groups. The most common symptom was cough (98.6% and 96.5%, respectively) and fever (85.7% and 91.3%, respectively). The most common physical findings were crackle (72.8% and 73.3%, respectively). The common radiologic findings were lobar pneumonia in both groups (55% and 76.1%, respectively), followed by bronchopneumonia (29.2% and 11.6%, respectively). Group A had a lower incidence of high antimycoplasma antibody titer (>1:5,120) and severe pulmonary complication than Group B. CONCLUSION: Group A had the more incidence of sputum production, leukocytosis, bronchopneumonia and duration of fever less than 7days. Group B had the more incidence of weak breathing sound, lobar pneumonia and duration of fever more than 7days.

Multicentric pigmented Bowen's diseas. (MPBD) is a bowenoid atypia in the genitalia of young adults which was first described by Lloyd in 1970. Clinically the lesions appear to be benign warts, but histologically they show bowenoid atypia., Although the pathogenesis and cours of MPBD is as yet unknown, many authors have suggested viral etiology. We recentiy encountered two cases of MPBD, one a 25 year-old male and the other a 38 year-old female, confirmed clinicallr and histopathologically. Case 1 showed spontaneous regression without any treatment, but case 2 showed recurrence of the lesions inspite of three times of surgical excision. The recurrent MPBD which has proven to be resistant to surgical therapy is unusual according to the previous reported cases.
