1.Leiomyosarcoma of the Breast.

Leiomyosarcoma of the breast is extremely rare. The natural history of leiomyosarcoma of the breast remains largely unknown because of the low incidence and lack of long-term follow up. But leiomyosarcoma of the breast appears to be less aggressive biologically than other sarcomas of the breast such as rhabdomyosarcoma. In an attempt to define diagnostic criteria of smooth muscle tumors of the breast, the reported cases of leiomyoma of the breast and nipple were also reviewed and compared with leiomyosarcomas by Chen et al(1981). Based on the result of this review, tumors with three or more mitoses per 10 HPF are leiomyosarcoma and those with no mitosis, necrosis, and significant atypia are leiomyomas. We have experienced a case of leiomyosarcoma of the breast. The patient was a 39 year-old female and was admitted due to a rapid growing right breast mass. The breast mass was noted 4 years ago and then this year the mass growed rapidly. Simple mastectomy was performed. Grossly the mass measured 11.0x8.0x8.0 cm in dimensions and was well circumscribed, pinkish, multinodular, and it was protruded from the cut surface. Microscopically the mass is composed of interlacing bundles of spindle shaped cells and multifocal large areas of hyalinized collagenous tissue. The individual spindle cells are slightly pleomorphic and occasionally show mitotic figures, 6~7 per 10 HPF in the most cellular area.

2.Factors Influencing Regeneration of Calvarial Defects in Rats.

An experimental study was done to evaluate factors influencing guided regeneration of bone in standardized calvarial bony defect. An 8 mm circular transosseous calvarial bony defect was made. Various material such as demineralized freeze-dried bone (DFDB), BioMesh , Millipore filter and its combination was placed in the bony defect. A sequential histopathologic, histochemical, immunohistochemical, and histomorphometric studies were done on the guided bone regeneration in the calvarial bony defect. Bone formation was sigificantly enhanced when the DFDB was retained within the bony defect with a protective bioabsorbable membrane. Inframembranous DFDB-filling was required to prevent collapse of the membrane and preserve spaces for bone regeneration. The bioabsorbable membrane should presumably remain intact for longer than at least 5 weeks to facilitate bone regeneration. The new bone formation was dependent on the barrier-effect (preserving secluded spaces) and inflammation-inducing property of membrane, and guiding bone regeneration of the grafts. Macrophages recruited by grafts were partly involved in decrease of bone regeneration via the sequential events of release of fibronectin, chemotactic effect of the fibronectin to fibroblasts, and collagen lay-down.

3.Cyclic Exotropia: One Case of Clinical Report.

Cyclic strabismus is now well recognized, but remains a rare entity. This heterotropia represents an interesting ocular motility problem in which the ocular deviation is present on a rhythmic basis, usually a 48 hour cycle. On the strabismic day, constant heterotropia is large and associated with suppression and no diplopia. On the nonstrabismic day, no deviation or only a small heterophoria is present with good binocular function. Most of these strabismus were presented with esotropia, but in our case, the pattern of deviation is cyclic exotropia. This is a very rare and unique feature.

4.On the Test of the Reliability and Validity of the Disabled's Motivation Scale for Rehabilitation.

PURPOSE: to test of the reliability and validity of the Disabled's Motivation for Rehabilitation Scale. which was developed in 2002. METHOD: An experimental version of the scale was distributed to a sample of 441 disabled with ages above 18 and below 80 years. The subjects of the test-retest were 60 disabled. RESULTS: revealed a satisfactory level of test-retest and internal consistency. The overall fit of the factor model to the data was good. Correlation among the subscales revealed a simple pattern that, in general. provides support for the self- determination continuum and the construct validity. In testing concurrent, criterion-validity, there was a positive correlation between the motivation scores for rehabilitation and the Health-Related Hardiness scores and a negative correlation between the motivation scores for rehabilitation and the Learned Helplessness scores. CONCLUSION: The Disabled's Motivation for Rehabilitation Scale revealed a useful instrument with a high degree of reliability and validity. In this sense, this instrument can be effectively utilized in rehabilitative nursing for the disabled.

5.Life Style Changes of Adults who become Disabled in Adulthood.

PURPOSE: This study aimed at uncovering the disabled adult's way of thinking and behavior due to their changed life style. METHOD: In depth interviews with 11 participants were held and analysed based on the Grounded Theory Methodology by Strauss and Corbin (1990). RESULT: This study showed us that the disabled are getting independent by selfconfidence through developing coping strategies for their physical, emotional, social, sexual problems. And in the process of creating these strategies, support structures, satisfaction for performance effect, financial burdens, and mobility had an effect on the intervening factors. Adults who become disabled go through many changes. First, they have vague hopes of improving. Second, they become discouraged. Third, they accept their disability. Next, they learn coping strategies and finally, they integrate the changes into their new life style. CONCLUSION: This study can be utilized in rehabilitation nursing so that the disabled can adapt to their changed life style quickly and lead independent and self-confident lives.

6.Hemangiopercytoma of the Meninges: The immunohistochemical study for the relationship between hemangiopericytic meningioma and peripheral hemangiopericytoma.

Hemangiopericycic maningioma is clinically and pathologically similar to peripheral Hemangiopericytoma and now tends to be terned as hemangiopericytoma of central nervous system. The authors studied 3 cases of hemangiopericytic meningioma obtained from 3 patients, 1 case of meningotheliomatous meningioma, angioblastic meningioma and transitional meningioma, and 2 cases of peripheral hemangiopericytoma, which had operated from November 1988 to May 1989 at the department of neurosurgery, Pusan Inje University Hospital. The authors analysed and compared the immunohistochemical finding and light microscopic apearance. The results obtained were summarized as follows; 1) Classic maningioma (meningotheliomatous meningioma, transitional meningioma and angioblastic meningioma) shows reactivity to both EMA and vimentin. 2) Hemangiopercicytic meningioma and peripheral hemangiopericytoma are reactive only to vimentin, so the two tumors are suggested as same type of tumor. 3) One of 3 cases of hemangiopericytic meningioma shows whorling and interlacing bundles of spindle cells, the peculiar light microscopic features of transitional meningioma, suggesting transitional or mixed form of hemangiopericytic meningioma and transitional meningioma.

8.Studies on Benzo(a)pyrene of the Suspended Particulate in Atmosphere of Seoul City.

This study was carried out to investigate air pollution by total suspended particulate(T.S.P.), benzene soluble matter and benzo(a)pyrene in Seoul city. The sampling areas were divided into commercial(Kwang Hwa Moon), industrial(Ku Ro Dong) and residential area(Shin Chon). Sampling was undertaken by High Volume Air Sampler for four seasons from January 1977 to November 1977. The T.S.P. was extracted with Soxhlet apparatus by benzene and benzo(a)pyrene was separated by column chromatography and thin layer chromatography. The concentrations of benzo(a)pyrene were measured by means of fluorophotometer, and following results were obtained. 1. Arithmetic average concentration for 1-day averaging time of total suspended particulate were 275.6 microgram/m3 in Kwang Hwa moon, 325.9 microgram/m3 in Ku Ro Dong and 193.0 microgram/m3 in Shin Chon. 2. The seasonal variance of total suspended particulate at Ku Ro Dong and Shin Chon were 102.7 microgram/m3 99.6 microgram/m3 respectively and at Kwang Hwa Moon 39.9 microgram/m3. And the concentration of autumn is higher than of that spring at Ku Ro Dong and at Shin Chon, but at Kwang Hwa Moon, the seasonal variance is very little. 3. The concentrations of 50% frequency from geometric mean for 1-day averaging time were 264 microgram/m3 and 178 microgram/m3 at Kwang Hwa Moon, Ku Ro Dong and Shin Chon. And geometric standard deviation were 1.27, 1.38 and 1.41 respectively. 4. The concentrations of benzene soluble mater were 26.9 microgram/m3 Kwang Hwa Moon, 22.7 microgram/m3 at Ku Ro Dong and 15.5 microgram/m3 at Shin Chon, and the ratios to the T.S.P. were 9.8%(range 5.6-14.8%), 7.0%(range 2.4-14.4%) and 8.0%(range 5.5-22.1%) respectively. 5. The concentrations of benzo(a)pyrene were 8.5 microgram/m3 (range 0.8-29.9 microgram/m3 ) at Kwang Hwa Moon 10.9 microgram/m3 (range 1.1-52.0 microgram/m3 ) at Ku Ro Dong and 5.8 microgram/m3 (range 1.5-11.4 microgram/m3) at Shin Chon. 6. The results of this investigation were relatively high in compared with the recommended standards of suspended particulate in air of U.S. Environmental Protection Agency and observed levels of Benzo(a)-pyrene in U.S. city.

9.Glomerular Hypertrophy in Focal Segmental Glomerulosclerosis.

It is not clear whether glomerular hypertrophy is related to the pathogenesis of focal segmental glomerulosclerosis (FSGS). We analyzed renal biopsies from 20 adults with FSGS by morphometry, and the data were compared with those from age- and sex-matched patients with minimal lesion. Mean glomerular volume in the FSGS group was significantly larger than that in the minimal lesion group[(3.4 + 1.1 vs 2.5 0.5)x10(6) micrometer3, P<0.01]. The percentage of glomeruli with global and segmental sclerosis in FSGS group was significantly correlated with the mean glomerular volume (r=+0.66, P<0.001). Relative interstitial volume of renal cortex in the FSGS group was correlated with the serum creatinine concentration(r=+0.5, P<0.05). These results suggest that glomerular hypertrophy observed in our patients with FSGS was related to nephron loss caused by glomerulosclerosis. The interstitial fibrosis may lead to obliteration of postglomerular interstitial capillary network with secondary elevation of glomerular capillary pressure resulting in progressive loss of renal function.
