1.The present activity and job satisfaction of 119 rescuer.

BACKGROUND: It goes without saying for the importance of pre-hospital care in EMS system. At present 119 rescuers take charge of the majority for the pre-hospital care in Korea. But the present activity and job satisfaction of 119 rescuers is not studied. METHOD: The authors analyzed the questionnaire from the 98 rescuers had taken the lessons on emergency medical technician training program at the Central Fire Fighting School, Cheonan city. We used a Likert's five stage barometer for the analysis of job satisfaction. RESULTS: On regional distribution of rescuers 29.6% was in metropolitan city and 70.4% was from each province. On sexual distribution male was 93%, average age was about 30 years old. The academic background was commonly below the high school graduation. Sixty-one of 98 rescuers (62.2%) was married. The monthly mean income is 1.07 million Won. There was a statistical difference of the monthly mean income between the age, sex, and the period of work (p<0.05). The average duration of job was 27 months. In most cases. the patients or relatives decided the evacuation hospital (89.8%). 67.0% of rescuers answered the necessity of emergency communication system during transport. The serious problems during transport were the lack of manpower, the knowledge of emergency care, the deficit of emergency equipment and telecommunication system. The priority of decision for evacuation hospital is the presence of special medical team, the severity of patients, the presence of modern hospital. The emergency equipment of ambulance was satisfactory in general. Almost all the rescuers (99.0%) want continuous education and 92.7% of them want at the emergency center or general hospital twice a year. The degree of job satisfaction was the job of special knowledge and skills, the professional work and self-assurance in order. CONCLUSION: 119 rescuers were gratified generally at the job satisfaction. But it was poor at work condition. The authors think that a government office and the korean society of Emergency Medicine must hard work for the improvement of work condition and the administration of continuous education.

2.A Clinical Observation on Tumors of the Genito-urinary Tract.

122 cases of the genitourinary tract tumors admitted to the Department of Urology, National Medical Center, during the period, Aug. 1968 to July 1975 and the followings are the results. 1. 122-cases(12.8%) were tumor patient among 954 (male 702, female 252) Total admitted patients during 7 years. 2. Age distribution was from 5 months to 83 years showing the highest incidence in the 7th decade (30.3%) and cases over 41 years of age were 99(81.1%) 3. Sex ratio of male and female was 5.4:1(103:19) 4. In 122 cases, 42 cases{34.4%) were benign, 77 cases(63. 1%) were malignant and remainders were uncertain pathologically. 5. Among 122 cases, 57 cases(46.7%) were vesical, 35 cases(28.7%) prostatic, 19 cases(15.6%) renal, 5 cases(4.1%) penile. 3 cases(2.5%) testicular and 3 cases(2.5%) were urethral tumors. 6. Operations were performed in 114 patients (93.5%).

3.Antiviral Effect of Antisense Phosphorothioate Oligonucleotides Targeted to Herpes Simplex Virus.

To search the effective antisense oligonucleotide that inhibit the growth of Herpes simplex virus type 1 (HSV-1), six kinds of phosphorothioate oligodeoxynucleotides (S-ODNs) were synthesized and the antiviral activity was assessed by measuring cytopathic effect in Vero cells infected with HSV-1. Of the three dodecamer S-ODNs, cornplernentary to the translation initiation site of IE2 (AS2) and scrambled S-ODN (AS1) showed more significant antiherpetic activity than AS4 complementary to the IE4. Accordingly, the antiviral effect of dodecamer S-ODN was not specific. In contrast to the no inhibitory effect of sense strand S-ODN of ICP8 (AS6), two S-ODNs complementary to the translation initiation site of ICP8 (AS3) and that of IE1 (AS5) showed potent antiherpetic activity assessed in vitro HSV-1 virus yield assay. Antiherpetic effect of AS3 was decreased in proportion to the addition of AS6. The synthesis of viral protein ICPS and IE1 were inhibited in AS3 and AS5 treated HSV-1 infected Vero cells, respectively. These findings suggest that antiherpetic effect of AS3 is specifically mediated by targeting ICPS. S-ODNs had no effect on Vero cell viability. The data suggest that the 19-mer S-ODNs may be effective in antiviral chemotherapy.

6.The Effect of Uremic Plasma on the Proliferative Activity of CFU-GM in in-vitro Culture of Mouse Bone Marrow.

The increased susceptibility in patients of chronic renal failure to infection has been reported to be attributed to defects in granulocyte and lymphocyte function and proliferative activity of hematopoietic cells. The definite cause of the frequent infection in uremic patients, however, is still controversial. The effect of uremic plasma on the aspect of the hematopoietic cells has been scarcely been studied. In the present study, mouse bone marrow was cultured with uremic plasma, to evaluate the effect of uremic plasma on the proliferative activity and morphological features of CFU-GM. The results obtained were as follows. 1) The number of colonies in group co-cultured with uremic plasma was more reduced than that of normal plasma group. 2) There was no difference between the group cultured with predialytic uremic plasma and that of postdialytic plasma in number of colonies, macroclusters and microclusters. 3) The forms of colony were granulocytic and monocytic forms at 5 day of culture. Electron microscopically, granulocytes disclosed electron dense azurophilic granules and electrolucent specific granules in the cytoplasm, and monocyte showed numerous vesicles and vacuoles in the cytoplasm which had finger-like projections. 4) The molecular weight of inhibitory factor in the uremic plasma was supposed to be less than 50,000 daltons.

7.Intrascrotal Dermoid Cyst.

Dermoid cyst is a benign cystic teratoma consisting of 3 germ cell origin tissues with predominant ectodermal origin and most commonly found in the ovary but rare in the scrotum. We reported two cases of intrascrotal dermoid cysts developed in 7 months and 3 years old boys which were confirmed hiatologically. In one case, the cyst developed from the testis and in another from the paratesticular tissue.

9.Wolff-Parkinson-White Syndrome and Pre-excitation Dysrrhythmia.

Since its initial description in 1930, the preexcitation or Wolff-Parkinson-White(W-P-W) syndrome is characterized by a special electrocardiographic pattern and various paroxysmal tachyarrhythmia, which was found to have reciprocating tachycardia frequently. The W-P-W syndrome develops when some part of a ventricle is activated earlier than normal conduction pathway, and described as one type of ventricular preexcitation syndromes by Durrer (1974). The diagnostic criteria of the W-P-W syndrome are 1) initial slurring (delta wave) of the QRS complex, 2) short P-R interval, 3) widened QRS complex and 4) secondary T wave change. The initial slurring of the QRS complex (delta wave) which is the most important finding of preexcitation syndrome results from a premature activation of a portion of the ventricle through an accessary pathway which bypasses the A-V node and bundle. These accessary conduction fiber includes Kent's bundle, Jame's fibers, Mahaim's fibers and its combination. Recent developments in the field of electrophysiology and surgical therapy became to support the concept of anomalous pathways and the possible determination of the re-entry circuit of paroxysmal tachycardia. Total 12 cases including 9 cases of classical W-P-W syndromes and 3 cases of L-G-L syndromes were followed with special interest of pre-excitation phenomenon and paroxysmal tachyarrhythmia at the National medical Center during the period of Jan. 1975 to Feb. 1979 and found to have paroxysmal tachyarrhythmia in 8 cases out of 12 cases. His bundle electrogram (HBE) and right atrial pacing were recoded in 2 cases of W-P-W type B to support the existance of an anomalous pathway. Treatment was instituted in accordance with recent advanced knowledge for the paroxysmal tachyarrhymia and pre-excitation and references were reviewed.

10.A Case of Generalized Syringoma.

A case of syringoma with unusual distribution in a young healthy woman is reported. Physical examination revealed numerous angular or lichenoid papules, varing from skin color to light brown in hue, on the anterior aspect of the neck, upper chest, breast, sternal area, inframammary area and abdomen with grouping tendency. Some papules had fine scales on their surface. The histologic findings were those of charateristic features of syringoma.
