2.MR Findings of Intravertebral Vacuum Cleft: Report of Two Cases.

Intravertebral vacuum cleft in collapsed vertebra was considered as a typical finding of avascular necrosis. However, several authors reported some cases of intravertebral vacuum cleft in primary or secondary neoplasm, or in spondylitis emphasiging the differenhal diagnosis. MRI is known to be a useful diagnostic modality for diferentiation between benign and malignanct conditions causing vertebral collapse. We report MRI findings of two cases with intravertebral vacuum cleft diagnosed as posttraumatic collapse with avascular necrosis on radiological and clinical bases.

3.Computed tomographic findings of gallbladder carcinoma

It is well known the CT is very useful in the evaluation of gallbladder carcinoma. We have studied 19 cases of gallbladder carcinoma with whole body scanner from May 1980 to Aug. 1983. 1. The sex distribution were 11 males and 8 females with over 40 years of age. 2. The computed tomographic classificications of the cases were 7 of intraluminal fungating mass(single 5 and multiple 2), 5 of mass filling the gallbladder, 5 of irregular thickened GB and 2 of mixed type, irregular thickned wall and intraluminal fungating mass. 3. CT demonstrated metastasis of other organ, 8 cases of the liver, 5 cases of the extrahepatic bile duct, 3 cases of peritoneum, 2 cases of duodenum, 1 case of the stomach, 1 case of the pancreas, and 15 cases of lymph nodes. 4. Associated diseases were6 cases of gall stone, 2 cases of clonorchiasis, 1 case of pancreatic pseudocyst, and 1 case of renal cyst.

4.New Plexiglass Hanging Drop Culture Apparatus.

The purpose of this paper is to describe a new apparatus which utilizes the principle of Van Tieghems hanging drop culture method. After using various methods of culturing fungi authors have found van Tieghems method to yield the most satifsymg result. Until now, this method has been slightly more tedious due to durability, storage, and quantity of culture necessary for each species. The new apparatus consists of 20mm plexiglass tubing cut in 16mm lengths and attache to a 16cm x12cm x2mm plexiglass plate with paraffin or manicure. These tubes a.re ranged in rows approximately 6cm apart with at least 8mm between tubes. Plates of varying sizes can be made to accornodate specific experimente. The authors developed thse inexpensive unbreakable plexiglass plates to allow for aimultaneous growth of various fungi and therefore more accurate comparison. Since these plates can be stored and handled easily, most af the disadvantages of using van Tieghems hanging drop culture rnethod have been eliminated.

5.A Case of Poikilodermatomyositis.

Poikilodermatomyositis is a variant of dermatomyositis which is characterized by polymyositis associated with mottled dark brownish pigmentation, seattered atrophic patch and telangiectasia of the skin. Poikiloderma usually occurs in patient with long standing dermatomyositis. The authors observed a case of typical poikilodermatomyositis in a 40-year-old male. Skin examination revealed dark brownish pigmentation, telangiectasia and whitish atrophic patches on entire body. Muscles of shoulder girdle were markedly wasted. Muscle weakness was so severe that he could not elevate his arm above shoulder level. He also had difficulty in going up stairs. Histopathologic findings of the skin were similar to those of poikiloderma vasculare atrophicans. Light microscopic and eIectronmicroscopic examination of muscle reveaIed myositis.

8.Mycological and Clinical Observation on Dermatophytosis.

this study was first initiated to compare the effectiveness of the KOH mount versus actual culturing from 1928 skin scrapings obtained frorn December, 1975 to August, 1976 in Daegu area. Second, changes in both clicical patterns and fungal flora isilated among the causative organisms were observed and following results were obtained . The KOH mount reparte;d a higher number of positive cases, when compared with the culture method. Covered areas of the body reported higher KOH positive rates than exposed areas, while cultures from the finger and toe-nails r."ported unusually low positive rates. In 51. 3% of the cases, both culture and KOH rnount were pasitive. The positive coincidence rate was found to be highest with the lesions frorn the scalp, buttock and groin, and lowest with lesions from the hands and feet. The positive KOH scrapings when cultured, showed a 10. 3% negative rate while the negative KOH scrapings reported a 7. 3%. positive culture rate. The rate of culture contamination was highest in KOH negative cases and also highest among finger and toe-nail lesions. Weather was also thought to be a factor: the higher the temperature, the lower the frequency of contamination. The 1138 strains isolated in 1976 were compared to a similar study done between 1957---1959, the following result., were observed: Trubrum has markedly increased while T. ferrugineum has significantly decreased with M. gypseum reported for the first time in Korea. Although most of these species were fairly equally distributed thrcughout the urban and rural areas, M. canis and M. gypseum were lirnited to the urban areas. Compared to 1959 changes in the causative organisms rate of the following dermatophytosis were observed In T. pedis, T. corporis, T. manus and T. Unguiurn, T. rubrum was reported as the predorninant causative agent and t.he second most prominent causative agent was T. mentaprophytes. In. T. cruris, T, rubum. was also repoted as the predoniinant causative agent with E. floccosurn the second whiile in T. capitis, T. ferrugineum remained the predominant causative agent, The frequeny of dernatophytosis was reported as follows: Tinea nf the nands and feet reported the highest incidence, T. pedis alone accounted for 38. 8% of all dermatophytosis while T, cruris was second 21.0% Compared to the previously study T. capitis decreased significantly. The majority of cases were found to be in the ag group(20~39 years) 68.4%. T. capitis and T. cruris were reported predominatly in males while similar results were reported with T. pedis. T. rnanus and T. unguium in females. The monthly distribution of T. capitis, T. corporis and T, cruris remained fairly consiant while T. pedis and I manus greatly increased during the liottcr months KIay to August. Coexistence of dermatophytosis preferred females to males and T.cruris infected from T. pedis was also common in males. On second experiment we extended the length of application to 15 days and the number of sujects to 15 persons. There developed erytherna in 1 case after 13 days of application and the others never showed erythema after 15 days of application, whereas with 2% soap solution erythema developed in 2 cases in 24 hours, 9 cases by 48 hours and 13 cases by 72 hours of application. The other 2 cases never showed erythema after 15 days of application. With above result there is no need to calculata IT as Kligman already mentioned. It is apparent that topically applied Ginseng saponin is essentially non-irritant material to the skin.

9.A Case of Incontinentia Pigmenti.

A case of infantile acropustulosis(I.A.). ariginally reported by Kahn, was seen at the Department of Dermatology, Catholic Medical College in September 1979. An 11 month old male patient had been suffering from 1~2mm sized severe pruritic pustules on both hands and feet from 3 months of age. The lesions had the course of exacerbations and remissions from the onset and aggravated since last summer. The eruptions did not respond to topical and oral steroids and antiboitics. There was no family history of atopy, psoriasis and scabies. Histopathological findings revealed large, well circumscribed, intraepidermal pustules which were filled with neutrophilic polymorphonuclear leukocytes. The scraping tests for S. scabiei and dermatophytic fungi were negative. Bacterial culture from the pustular lesions was negative. Treatment was done with 4,4-diaminodiphenylsulfone(Dapsone), 2mg/kg/day in devided doses. Dramatic effect was noted within 2 days after treatment.

10.Clinical Experience of Plasma Exchange.

BACKGROUND: Plasmapheresis or plasma exchange is the procedure to remove various pathogenic materials from the circulation of patients and retransfuse the formed elements into the doners with type specific fresh frozen plasma or albumin for the replacement of the withdrawn plasma. METHODS: In order to evaluate clinical experience of plasma exchange, we examined a total of 133 plasma exchange procedure on 25 patients of January 1993 - May 1994 in Kyungpook national university hospital. RESULTS: The results are as follows: 1. There were 8 kinds of diseases and the most frequent diseases was Guillian-Barre syndrome(l 2 cases) and the decreasing order was CIDP(6 cases), multiple myeloma(2 cases), acute fulminant hepatitis(1 case), mushroom poisoning(1 case), lupus nephritis(l case), TTP(I case) and myasthenia gravis(l case). 2. The patient's age ranged from 13 to 65 years. 3. There were 14 males and 11 females and the sex ratio was 1.3:1. 4. We had got clinical improvement in 16 of 25 cases, especially most dramatic effect in Guillian-Barre syndrome. CONCLUSION: To achieve more successful outcome of plasma exchange, we should select patients with restrict application, prevent rebound phenomenon with intense practice, choose what kind of replacement fluid to consider patient's clinical condition and check serological parameter with clinical evaluation to monitor effectiveness of treatment.
