4.A Case of Left Ventricular Diverticulum.

Left ventricular diverticulum is rare cardiac malformation;fewer than 30 cases have been reported in the world. We present a case of left ventricular diverticulum with chest pain suggested unstable angina. Myocardial rupture of this case was occured during cardiac catherization. patient was recovered and discharged. Relevant literature was also reviewed.

5.Treatment of the Totally Displaced Supracondylar Fracture of the Humerus in Children by Three Crossed K-Wires Fixation.

Forty children underwent three crossed K-wires fixation after reduction of the totally displaced supracondylar fracture(type IE ) of the humerus from October 1994 to March 1997, were reviewed. The patients age ranged from 3 years and 2 months to 12 years and 7 months, averaging 6 years and 6 months. Preoperatively, five patients(12.5%) had concomitant nerve injury. Three patients(7.5%) revealed ischemic sign on distal part of the fracture. All the fractures were treated within 24 hours after arrival at the emergency room. After general anesthesia, all but one fractures reduced by closed means. Each reduced fracture was fixed by two parallel K-wires in lateral side followed by one crossed medial K-wire under fluoroscopic control. The preoperative distal ischemia of the three patient restored blood supply within 24 hours after reduction. The K-wires were removed after averaging 3.3 weeks after operation in out patient clinic. Five patients revealed neurologic deficit preoperatively recovered completely within 4 months after reduction of the fractures. The followup period ranged from 12 months to 32 months, averaging 17 months. By Flynns functional and cosmetic criteria, 39 patients(97.5%) among forty patients resulted in satisfactory criteria. The one unsatisfactory patient revealed cubitus varus treated by corrective osteotomy. In conclusion, three crossed K-wires fixation is considered as effective and safe method for the totally displaced supracondylar fracture of the humerus in children.

Bacterial endocarditis is a disease with protean manifestations whose presentation has recently changed greatly. Right-sided endocarditis associated with congenital heart lesions is now relatively less common. Patients with ventricular septal defect are at risk for right-sided endocarditis, but its incidence is low in children and adolescents. The organism is frequently a staphylococcus and the clinical course is dominated by septic pulmonary infarction and septicemia. We reported two cases of pulmonic vegetation and pulmonary infarction associated VSD with brief review of literature.

8.Mechanism of UV light-induced photorelaxation in isolated rat aorta.

Isolated rat thoracic aorta which is pharmacologically precontracted by phenylephrine induces photorelaxation when exposed to long wave length UV-light. The aim of the present study was to characterize the mechanism of UV-light induced by photorelaxation in the rat aorta. 1. UV light relaxed both endothelium-intact and -denuded rat aortic rings contracted by phenylephrine. The magnitude of relaxation on UV light was dependent on the exposure time and slightly greatly in endothelium-denuded rings than in endothelium-intact preparations. 2. L-NAME (10 nM-100 uM) but not D-NAME completely inhibited the photorelaxation in a concentration dependent manner. 3. The UV-induced relaxation was inhibited by methylene blue (1 -100 uM), and verapamil (100 nM), and removal of extracellular Ca2+. In contrast, UV-light induced photorelaxation was potentiated by N(w)-nitro-Larginine (L-NOARG) treatment. 4. In immunocytochemical analysis of UV-light induced iNOS and eNOS expression in rat aortas, at which expression levels were increased in a time-dependent manner on UV-irradiation in aortic endothelium and smooth muscle, respectively. These results suggest that UV light-induced photorelaxation may be due to nitric oxide from exogenously administered L-arginine as well as endogenous nitric oxide donors such as amino acid and arginine derivatives. Additional suggestion is that UV light stimulates the expression of nitric oxide synthases, and its activity for nitric oxide generation is dependent on cytosolic Ca2+ originated from extracellular space.

9.Modified classification of anemia by RDW.

The author obtained index of red cell volume distribution width(RDW) and other red cell indices in 210 patients of various hematoncologic conditions and 200 healthy control group using, an automated blood analyzer, Coulter Counter Model S-plus II. This study performed to classify various etiologic anemia based on the MCV and RDW, to evaluate availability to the differential diagnosis in korean anemic distoders somewhat different from etiologies of anemias in foreginers. In the most of cases, the increase or decrease of MCV were always combined the pararell changes of MCH and MCHC: But the values of MCV and RDW were not correlated in control group and patient group. So the terms of heterogenous of homogenous anemia were meaningful morphologic classification than hypochromic or normochromic anemia. The heterogenous microcytic anemia contained iron deficiency anemia. In heterogenous normocytic anemia, myelophthisic anemia, acute leukemia were contained. In heterogenous macrocytic anemia, megaloblastic anemia, hemolytic anemia were contained. The homogenous microcytic anemia was observed in anemia of chronic disorders. In homogenous normocytic anemia, acute blood loss, chronic leukemia, multiple myeloma were contained. The aplastic anemia was belonged to homogenous macrocytic anemia. The diagnostic significance of RDW in hemoglobinopathies is most importhant. But this study was not contained hemoglobinopathies. Instead RDW was very helpful to differential diagnosis of most common anemias, iron deficiency anemia and anemia due to chronic disorders in Korea.
