2.A Study on the Histopathologic Features of Bowenoid Papulosis and the Numerical Change in Langerhans Cells.

Fourteen cases of bowenoid papulosis were studied both clinically and histologically and were compared with 22 cases of Bowen's disease to find histologic differences between the two diseases. Bowenoid papulosis was prevalent in young, sexually active adults, consisted of multiple erythematous or hyperpigmented papules, and located mostly on the penis and vulva. The main histologic characteristics of bowenoid papulosis were an orderly background of keratinocyte maturation of the epidermis with monotonous pattern, the frequent presence of 'skip area', and the lesser degree of cytologic atypia than seen in lesions of Bowen's disease. In addition, mitotic figures more than 5 per x400 filed, formation of keratin pearl, and spindling of keratinocytes, which have not been described in other articles yet, are considered very important findings for the diagnosis of bowenoid papulosis. It is our opinion that bowenoid papulosis can be reliably distinguished from Bowen's disease by light microscopic features. Paraffin-embedded samples of bowenoid papulosis, Bowen's disease, and normal skin were investigated with antibodies against S 100-protein to relate the number of intraepidermal Langerhans cells and the disease. Langerhans cells were found to be reduced when compared with normal skin. No significant differences between Bowen's disease and bowenoid papulosis were found in density of Langerhans cells.

4.Capillary Hemangioblastoma in the Spinal Cord: A Case Report

The capillary hemangioblastoma, as originally defined by Lindau and by Cushing and Bailey, is widely regarded as being of Vascular origin and relatively uncommon. The most frequent site of this neoplasm is the cerebellum and second frequency is the spinal cord. We experienced a case of capillary hemangioblastoma arising at the upper thoracic spinal cord with the paraplegia and the sensory disturbance below the nipple line. The total laminectomy with mass excision was taken and the patient have shown good improvement of the neurological defecits in postoperative period.

5.Experimental Study on Serial Changes of Tc-99m-MDP Scintigraphy after Tibia Fracture

The purpose of this study was to investigate that there was any differences of uptake ratio of Tc99-m-MDP during the healing process related to fracture level in tibia. Thirty mature rabbits were devided into three groups and each group consisted of ten rabbits. The osteotomy was performed at the proximal third of tibia in group I, at the middle third in group II and at the distal third in group III, respectively. After fracture was reduced, the fractured tibia was fixed with K-wire and immobilized with long leg cast. Serial Tc-99m-MDP scintigraphy with pixel counting method was performed in thirty cases of tibial shaft fracture from one day to eight weeks after fracture. The results obtained were as follows: l. In group I, the mean uptake ratio of Tc-99m-MDP at fracture site was 1.40±0.45 at two days after fracture, 3.34±1.21 at two weeks and increased rapidly to a maximal value of 7.51%1.01 at three weeks and decreased thereafter to 4.530.83 at eight weeks. 2. In group II, the mean uptake ratio was 1.10±0.10 at three days, 3.931.±10 at three weeks and increased rapidly to a maximal value of 7.37±2.21 at four weeks and decreased thereafter to 4.54±0.96 at eight weeks. 3. In group III, the mean uptake ratio was 1.13 ± 0.11 at three days, 3.95±0.93 at three weeks and increased rapidly to a maximal value of 7.43±1.72 at four weeks and decreased thereafter to 4.69±0.93 at eight weeks. These experiments suggest that the changes of Tc-99m-MDP scintigraphy in the proximal third fractures of tibia was earlier than middle or distal third fractures and there was no differences between middle and distal third fractures of the tibia.

6.A Case of Subcorneal Pustular Dermatosis.

Subcorneal pustular dermatosis is a chronic, benign, relapsing pustular eruptions characterized histologically by subcorneal pustules which contain abundant neutrophils. We report a case of subcorneal pustular dermatosis occured in a 71-year-old male with typical climcal lesions. Biopsy of a pustule showed a subcorneal cavity with numerous neutrophils, which were also scattered throughout the Malpighian layer below. The result of imrnunofluorescent studies was negative. His eruption is controlled by DDS (25-100mg daily) but the eruptions recur quickly on sessation of the drug.

7.A Case of Reticular Erythematous Mucinosis (REM) Syndrome.

A 43-year-old male patient visited our clinic complaining of persistent erythematous skin eruptions on the anterior chest, abdomen, and back for 10 years. Reticular erythematous mucinosis (REM) syndrome was diagnosed by the clinical and histopathologic features. Mucin deposition is much more conspicuous in papular lesions than in plaque lesions. Therefore, we suggest that the papular lesions may show the characteristic changes of REM and that a biopsy specimen should be obtained from a papular lesion for proper diagnosis of REM syndrome.

8.Atelectasis Oecurring as a Pulmonary Complication after Goneral Anesthesia.

After simple clcsure of duodenal ulcer perforation under general anesthesia on a 34 year old male patient, atelectasis developed, Oo the 2nd postoperative day, dyspnea suddenly developed and atelectasis was confirmed by chest X-ray. Tracheostom,y was performed, and I.P.P.B. and suction were continued for a few days. The patient was discharged without further event.
