A case of Peutz-Jeghers syndrome was experienced with melanin pigmentation in the oral cavity and lips associated small intestine polyposis. Diagnosis of Peutz-Jeghers syndrome was established by clinical characteristics of skin and X-ray studies of gastrointestinal tracts.

We compared the DNA sequence difference of isolates of Clonorchis sinensis from one Korean (Kimhae) and two Chinese areas (Guangxi and Shenyang). The sequences of nuclear rDNA (18S, internal transcribed spacer 1 and 2: ITS1 and ITS2) and mitochondrial DNA (cytochrome c oxidase subunit 1: cox1) were compared. A very few intraspecific nucleotide substitution of the 18S, ITS1, ITS2 and cox1 was found among three isolates of C. sinensis and a few nucleotide insertion and deletion of ITS1 were detected. The 18S, ITS1, ITS2 and cox1 sequences were highly conserved among three isolates. These findings indicated that the Korean and two Chinese isolates are similar at the DNA sequence level.

The chronic paranasal sinusitis are caused by various orign such as dental, general and anatomic problem. Among them, the odontogenic maxillary sinusitis which concerned by cral & maxillofacial surgeon have been treated by medicine, conservative treatment such as endodontic treatment, and Caldwell-Luc operation. But, New technique, so called Functional Endoscopic Sinus Surgery, have recently been performed in the ENT dept. This technique have the mechanism that widening maxillary ostium through removal of involved nasal mucosa and adjacent tissue accelerates adequate drainage and regeneration of normal mucosa and have merits such as minimum morbidity and complication. So, we would report one case, 22 years old female, of 3 patents who were diagnosed as odontogenic maxillary sinusitis and treated by Functional Endoscopic Sinus Surgery and Apicoectomy, and had good results.

Ipsilateral fracture of the femur and tibia is also called “floating knee”. And it is well known that its treatment is difficult and functional result is poor in most cases because it has often comminuted, open fracture with severe soft tissue injuries. Authors reviewed 20 cases in 18 patients which were treated from September 1989 to March 1993. All were followed for more than one year. The results were as follows: 1. Among 20 cases of femoral fracture, 15 cases were treated with internal fixation and others with external fixation and among 18 cases of tibial fracture(2 amputations excluded), internal and external fixation were equal in number. 2. The bony union time was shorter in the intramedullary nailing than the plate and screws, and external fixations. 3. The end result was better in Group I(internal fixation for both fracture) than Group II (one or both fractures were treated with external fixator) because the femur had less severe comminution, and soft tissue injuries. 4. There were many complications in Group II than Group I, That is, wound infections, delayed union or non-unions. There were 11 operations for the treatment of complications,.

A fistulous comunication between kidney and colon after trauma is rare and usually causes severe damage to kidney necessitating nephrectomy. Intrarenal aneruysm after trauma is also rare and its accurate diagnosis is important. Authors have experienced a reno-colic fistula and intrarenal aneruysm occured in a same patient afterpenetrating renal injury.

Congenital duodenal obstruction in the newborn infant may be due to a variety of causes. Duodenal obstruction often presents with bilious vomiting and upper abdominal distention. Diagnosis is usually established on plain x-ray of the abdomen by the classic finding of the double-bubble. In the period July 1986 to June 1990, 16 patients with congenital duodenal obstruction were operated and the following results were obtained. 1. Sixteen patients were comprised of 11 males and 5 females, the ratio of male and female was 2.2:1. 2. Thirteen patients (81%) had been admitted to our hospital during one month of life. 3. Congenital duodenal obstruction was in 16 cases; malrotation in eight (50%), annular pancreas in six (38%), type 1 atresia in one (6%), and wind-sock anomaly in one (6%). 4. There were two premature patients and six patients of small for gestational age. 5. Overall, bilious vomiting, occurring in three fourths, was the single most frequent presenting complaint. 6. Polyhydramnios occurred in two of the patients. 7. Diagnosis was possible with clinical symptom and simple abdomen. 8. The operative procedures performed were; duodenoduodenostomy in five, duodenojejunostomy in two, excision of wind-sock membrane in one, and Ladd's procedure in eight. 9. A total of ten associated congenital anomalies were found in six patients. 10. Postoperative complications occurred in five cases (31%).

Infantile hypertrophic pyloric stenosis(IHPS), which occurs three of 1,000 live births, is a major cause of 1 nonbilious vomiting of early infancy but its etiology and pathogenesis are still obscure. The operation of pyloromyotomy as described by Ramstedt in 1912 remains the standard of care for the treatment of IHPS. From January 1993 to October 1996, 35 infants with IHPS were surgically treated and the following results were obtained. 1. Thirty-five patients comprised 32 males and 3 females, and the ratio of male to female was 10.7:1. 2. The most prevalent age group was between 2 weeks and 8 weeks. 3. Of 35 infants, first born babies were 23 cases(65.7%). 4. Breast feeding was in 23 cases(65.7%). 5. The body weight percentile at admission was lower than 50 percentile in all 35 cases. 11. A total of seven associated anomalies were noted in six patients. 12. All 35 cases were treated with Fredet-Ramstedt pyloromyotomy. 13. There were postoperative complications of wound infection in 2 cases. Intermittent nonprojectile vomiting was presented in 8 cases(22.9%) after operation, but one of them was relieved in 13 days and the rest were relieved within one week by adjustment of oral intake.

Ovarian cancer has the highest mortality rate among gynecologic malignancies in developed countries. Most women present with advanced disease and require a combination of surgery and chemotherapy. One patient presented with recurrent ovarian cancer was initially treated with taxol and platinum-based compounds. Although response to these agents occurred, tumor progression was evident by elevated CA 125 levels and CT findings after a period of 4 months. This patient was then treated with topotecan and exhibited a response and stopped therapy. Topotecan exhibited activity in this patient with ovarian cancer resistent to both platinum and paclitaxel. We report a case of advanced ovarian cancer which was treated with topotecan after taxol-cisplatin treatment failed.

The Pumping technique generally has been known as a diagnostic method that predicts intracapsular status and selects required evaluation and treatment. But, in addition to this indication, when it is followed by Lavage and Manipulation technique, this may be used as therapeutic method that is effective on Closed Lock with limited mouth opening and pain. Therefore, we examined results of this technique abut Closed Lock and would like to report therapeutic effects of Lavage and Manipulation technique. We selected 40 patients of 200 patients diagnosed as Closed Lock over 53 months from SEP. 1991 to PEB. 1996, who were possible follow-up and performed by Lavage and Manipulation technique. Appraisal between pre-procedure and post-procedure was performed just after procedure, in 1 month and in 3 months, and therapeutic effects were divided by marked, useful, a little useful and ineffective. As a result, Lavage and Manipulation technique had remarkable increase of maximum mouth opening(>5mm) in 39 persons(98%) just after proceure, 36 persons(90%) in 1 month, and 39 persons(98%) in 3 months, and subsided pain completely just after procedure and had remarkable decrease of pain in 30 persons(75%) in 1 month, 32 persons(80%) in 3 months. So we concluded that the Lavage and Manipulation technique is effective therapy of Closed Lock which can be used with ease.
