5.Computed tomographic evaluation of brain tumors

The advant of computerized cranial tomography represents a great advance in the diagnosis of very wide varietyof intracranial lesions. The CT findings of brain tumors have been reported by many authors, especially by Hatamin early contrast enhancement and evaluation of the intracranial lesions. A statistical analysis was performedwith 84 brain tumors cases of with diagnoses were confiremd at the department of Radiology and Pathology of theMiddlesex Hospital London from Jan. 1979 to Dec. 1980. The authors classified the intracranial tumors according toRussel and Rubinsteins classification, and especially for the changes after CT introduction. The results were asfollows; 1. Brain tumors were found most frequently in middle age group (25.7%) and the ration of male to femalwas 2:1. 2. Among the brain tumors, gliomas were found most frequently (57.1%), followed by Meningioma(20.2%),Metastatic tumors(11.9%), and pituitary adenomas (4.8%). 3. The incidence of glioma occupied(57.1%) of all braintumors and as a entity these were the highest. 4. Brain tumors occured more fequently in frontal regions (27.4%),parietal regions(17.0%), sella and para sella-regions(11.9%), cerebellum regions(9.5%). 5. Gliomas occured morefrequently inn temporal regions (20.2%) and followed by frontal regions (16.7%). 6. Since the advant of CT scan,the detection rate of brain tumors was increased, but there was no specific change in proportion of tumors.

6.An Experimental Study on Renal Functional Disturbance in Rabbits After Acute Ureteral Obstruction With Dynamic Gd-DTPA Enhanced MR Imaging.

PURPOSE: To evaluate the anatomic and functional change in acutely obstructed kidneys, and the probability of functional recovery after resolving the obstruciton. MATERIALS AND METHODS: There were 5 animal groups(6 rabbits in each group)-normal control group, 4 acute ureteral obstruction group. Each of the latter group was imaged with Gd-DTPA enhanced dynamic MR 2, 6, 24, and 48 hours after ligation of the left proximal urter, respectively. We compared the findings of dynamic MR imaging and histopathological findings. RESULTS: We could observe centripetal movement of dark band pattern which could be defined as 3 phasesrenal cortex, outer medulla, and inner medulla in normal rabbits. The appearance of the dark band pattern was delayed or absent in ureteral obstruction group with a linear relationship to the duration of the obstruction. CONCLUSION: Gd-DTPA enhanced MR can be used to differentiate acute obstructive nephropathy from other causes of nephropathy and foretell the prognosis of the obstruction.

7.The Successful Dissolution of 2 Cases of Fat Bezoars by Pancreatic Lipase.

Gastric bezoars are large masses or concretions, composed with plant fiber, hair and miscellaneous foreign bodies, found in the stomach and occur usually as sequele of gastric surgery. We experienced 2 cases of fat bezoars which developed after drinking of melted beef fst. They had postprandial epigastric fullness and pain, those were relieved on supine and left recumbent position. Upper gastrointestinal series showed a large movable mass in the stomach in hoth cases. On gastrofibercopic examination, a large egg sized white bezoar was cordirmed in both cases and a gastrie ulcer at antrum associated in one case. In attempt to removal of the bezoars, we tried to break them, but failed because of its character. The patients were treated with pancreatic lipase containg digestives, Pancreon-F and Azintal. From one day after this treatment, symptoms were completely relieved. Five days later, we confirmed disappearance of bezoars by gastrofiberscopy. and upper gastrointestinal barium

8.Immunohistochemical Detection of Prekeratin and Keratin in Hair Follicles and Epidermis of Human Fetus.

Authors attempted to examine the development of keratinocyte in the human fetal epiderrnis in term of the appearance of principal product of differentiation, the keratins. Immunohistochemical study (ABC method) using anti-prekeratin antibodies and anti-keratin antibodies was carried out on sampled skin from 9 human fetuses ranging in estirnated gestational age from 12 weeks to 27 weeks. As early as 12 weeks of embryonic life, the epidermis contained cytokeratin. At this stage of development, the embryonic epidermis consists of three-cell layer .' basal layer, intermediate layer, and periderm, all of them staining for prekeratin and keratin. A thick cornified layer developed at 24 weeks but was not stained for prekeratin and keratin. Hair germ of 12 week-fetus was recognized as bulges of basal cells that were stained for prekeratin and keratin. At 18 weeks of estimated gestational age, development has advanced to the bulbous hair peg stage at which point the primordia of nearly all the final structural components of follicles were established. In the final structureal cornponents of the follicles, outer root sheath and sebaceous gland were stained for prekeratin and keratin, whereas cortical and cuticular elements and inner root sheath were negative. There was no difference between immunostaining results for prekeratin and keratin except for the staining intensity. The appearance of these proteins early in development suggests their potential value in utero in the recognition of keratinization disorders.
