4.Localized Cystic Disease of the Kidney: A case report.

Localized cystic disease of the kidney is a rare entity with the gross and microscopic features of autosomal dominant polycystic kidney disease localized to only a portion of a kidney, and negative family history. We report a case of localized cystic disease of the kidney in a 38-year-old woman who complained of intermittent right flank pain for 1 year. The resected kidney showed multiple cysts measuring up to 4.0 3.5 3.0 cm, which were scattered throughout the mid- and lower poles of the kidney. Microscopically, the cystic lesion was composed of numerous cysts of variable size, lined by flattened epithelium. The intervening septa of the cysts contained normal or compressed renal tubules and glomeruli. Neither dysgenetic tissue such as immature cartilage or primitive mesenchymal tissue nor malignant cells was identified. Localized cystic disease should be included in the differential diagnosis of cystic lesions in the kidney.

5.Effect of Calcium and Activated Vitamin D ( 1- hydroxyvitamin-D ) on Bone Mineral Density in Osteoporosis.

Forty-five elderly female osteoporotics were given daily dose of 750 mg of calcium and 1.0 microgram of activated vitamin D (1-a hydroxyvitamin D) orally for 24 months to evaluated the effect of drugs on bone mineral density. Bone mineral density was evaluated and repeated every 6 months for 2 years. Average bone mineral density (L (2)-L (4)) and T score were 0.773 (SD 0.120) and -2.90 (SD 0.97) before treatment, and increased to 0.808 (SD 0.121) and -2.59 (SD 0.97) after 24 months of therapy, respectively (p<0.01). During treatment, complications including hypercalcemia, hypercalciuria, or renal stone were not observed, and indicators of renal function were normal. Therefore the activated vitamin D treatment in osteoporosis is effective in diminishing normal decline of bone mineral density, especially in areas with low calcium and vitamin D intake, such as South Korea.

7.A Case of Marfan Syndrome with Left Atrial Myxoma Confirmed by Echocardiography.

The Marfan syndrome, a rare disease causing a marked decrease in life expectancy by involving the skeletal, ocular, and cardiovascular systems, is known as a connective tissue disorder that is inherited autosomal dominant. The cardiovascular complications directly related to the cause of death are associated with more than 90% of the Marfan syndrome. Aortic aneurysm with rupture could occur because of dilatation of aorta due to defect of media. Aortic and mitral insufficiency, mitral valve prolapse, bacterial endocarditis, arrhythmia, and aneurysm of interatrial septum are also frequently observed. A few literatures were reported domestically pertaining to the Marfan syndrome thus far, and there are increased discoveries of cardiovascular complications of the syndrome with the application of echocardiogram. However, the Marfan syndrome with LA myxoma has never been reported both domestically and internationally(INDEX MEDICUS, 1966-1989). Therefore we hereby report a case of the Marfan syndrome with LA myxoma from the observation of a patient who was admitted to Eulji General Hospital at Taejeon i August of 1989.

10.Comparative Study of Clinical Effect by Topical Application of Fabry's Solution and Fabry's Solution with Antibiotics (Chloramphenicol or Clindamycin) in Acne.

Antibiotic therapy for acne is now considered one of the most effective regimen. Thirty years ago, oral antibiotics were introduced for acne vulgaris and topical preparations have been available by prescription for more than 15 years. Recently, clindamycin, erythromycin and tetracycline were regarded as the most effective and widely used antibiotics. Many authors reported that clindamycin was a mast effective topical antibiotic for acne vulgaris but side effects, i.e., contact dermstitis or pseudomembranous colitis were reported. The author studied the clinical effectiveness and side effects of Fabrys salution, which has been used as an antiseptic and keratolytic agents, and Fabrys solution containing chloramphenicol or clindamycin. Fifty three patients who were followed for 6 weeks were studied. They were divided into three groups: group I was treated with Fabrys solution(F solution), group II was treated with Fabrys solution containing chloramphenicol(F-c solution) and group Ill was treated with Fabrys solution containing clindamycin(F-cd solution).
