1.Exercise Therapy for Hypertensive Patients.

Hypertensive patients tend to experience cardiac diseases such as myocardial infarction more frequently than the normotensive subjects. Exercise training in hypertensive patients alliviates their symptoms, improves their cardiopulmonary endurance, and decreases their cholesterol levels. Regular exercise in hypertensive patients decreases the development of their symptoms, therefore makes patients comfortable due to the decreased sympathetic tones and increased parasympathetic tones. To prescribe exercise to hypertensive patients, it is essential to evaluate the patients' blood pressure while exercising. The components of exercise prescription include the mode, intensity, duration, and frequency of exercise, The best exercise for hypertensive patients is aerobic exercise which improves cardiopulmonary functions. It makes the patients take less medications. The aerobic exercise includes cycling, walking, and running. The intensity of the aerobic exercise should be 45~60% of their VO2max value. While the patient is taking medication, the prescription of exercise should be cautious, because most antihypertensive agents decrease exercise endurance, and thus make it difficult to continue exercise, compared with those without medication.

2.Evaluation of the Use of Rh(D)'Control Test in Rh(D) Typing.

Clinically, the Rh blood group system is important since Rh antibodies are readily induced by transfusion or pregnancy in individuals negative for the antigert and may cause hemolytic reactions or hemolytic disease of the newborn. Since the D antigert is strongly immunogenic, donors and patients are routinely typed for D status and patients are generally given D compatible blood. But under several circumstances such as spontaneous agglutination of red blood cells coated with immunoglobulin, antisera with additives may cause false positive results in test using high-protein reagents. And facton in the patient' s own serum may also affect the test, since unwashed red blood cells suspended in their own serum or plasma are frequently tested. Therefore, manufacturers and American Association of Blood Banks(AABB) recommend that the Rh(D) control test with Rh(D) control reagent which contains the same additive present in high-protein anti-D except for the anti-D. This study was undertaken to evaluate the usefss of the Rh(D) control test in Korea where Rh(D) negative population is small. Red blood cells from 1115 in-patients and 468 out-patients at Korea University Medical Center were employed in Rh(D) typing and Rh(D) control test in parellel. 1580 cases are Rh(D) positive and 3 cases were Rh(D) negative. No agglutination was observed with Rh(D) control test. Though AABB and manufacturers recommended that the Rh(D) control test should be done in parellel with Rh(D) typing test, the authers concluded that there were no need to run the Rh(D) control test in Korea.

3.Incontinentia Pigmenti Achromians ( Ito ): Report of two cases.

Incontinentia pigmenti acb.romians(Ito) is characterized by progressive bizarre or whorl-like hypopigmentation on trunk and extrementies during childhood. It is similar to a negative picture of incontinentia pigmenti(Blocb-Sulzberger) and not infrequently associated with mental, bony and ocular defects. The incidence of this disease is predominent in femaIe without hereditary background. Case 1. Two years old female was visited to our clinic because of mottled depigmented patches on right thigh for about 8 months. Case 2. 14 months oId female was visited to our clinic because of linear and. mottled depigmented natches on their limbs for about 7 months. Histopathological findings of both cases shows the focal depigmentation on basal layer. They are treated with steroid ointment with moderate to good effects.

4.Perception of Patient Safety Culture and Safety Care Activity of Entry-level Nurses.

PURPOSE: We investigated the relationship between patient safety culture and safety care activity, and identified factors for safety care activity of entry-level nurses. METHODS: The subjects of the study were 204 entry-level nurses working at five general and advanced hospitals with over 500 beds located in C and D City. The data were analyzed using descriptive analysis, t-test, ANOVA, Pearson's correlation and hierarchical multiple regression with SPSS/WIN 18.0 program. RESULTS: Perception of patient safety culture had significant correlations with safety care activity. Multiple regression analysis showed that demographic variables of quality of nursing on patient safety and health state of entry-level nurses affected safety care activity. In addition, patient safety culture in ward and communication affected safety care activity. These factors explained 38% of variance. CONCLUSION: Based on the results of the study, patient safety culture promotion strategies to facilitate supportive work environment and effective communication are needed.

6.A Study on An Emergency Transfer System in Pusan Area.

Five hundred and thirty five cases of patients who were transferred to the Department of Emergency Medicine in Pusan University Hospital from Aug 1, 1997 to Aug 31, 1997 and from Oct 1, 1997 to Oct 31, 1997 were studied prospectively. The results were as follows: 1. Total 535 patients were transferred to the emergency department of PNUH(Pusan National University Hospital) for 2 months(average 8.2 a day). Male to female ratio was 1.58:1 and peak age group was 6th decade(17.0% of total). 2. In distribution about arrival time of transferred patients, 40.7% was from 08:00 to 16:00, 39.3% was from 16:00 to 24:00 and 20.0% was from 0:00 to 8:00. 3. Cases that were communicated with the emergency department of PNUH were 7.7%, cases that were communicated with other departments of PNUH were 10.1%, and 82.2% of total cases were transferred without any communication. 4. A 40.7% of patients was transferred by a hospital ambulance, 33.1% of patients used non-emergency vehicles such as taxis or buses, 25.3% of patients was transferred by an 119 or 129 rescue ambulance, and 0.9% was transferred by on foot or others. 5. Transferring hospital was divided into 3 groups: primary hospital, secondary hospital, and university hospital. The majority was secondary hospital(83.6%). 6. The cases that patients wanted to be treated in PNUH were the greatest in number as 55.5%, the cases transferred due to emergency surgery were 9.3%, the cases transferred without specialized department concerned were 28.6%, the cases transferred because of bed deficit were 1.7% and the cases transferred due to other cause were 4.9%. 7. Traumatic patients were 32.5%, and among them 9.9% was due to motor vehicle accident, among these 78.5% was mild patients. Non-traumatic patients were 67.5% and among them 74.2% was mild patients. 8. Among all transferred patients, 75.7% was admitted for general care, emergency operation, and ICU care. The percentage of discharge was 19.1%, the percentage of death was 2.4%, and the percentage of transfer to other hospital was 2.8%. Generally, because of bed deficit and of availability of operation room, patients were transferred to other hospital. 9. Among specialized departments, the proportion of the department of internal medicine, neurosurgery and pediatrics was 29.3%, 17.2% and 11.4% respectively. In conclusion, it is considered that proper patient transfer and effective medical service can be achieved through the well-organized EMSS, and cooperative interhospital communication.

8.Introduction and contemporary condition of helicopter emergency medical services in Korea

As most medical facilities in Korea have been concentrated in large cities, the need to improve emergency medical services in islands and mountainous areas has emerged. Consequently, the Ministry of Health and Welfare and local governments have introduced emergency medical helicopters (known as helicopter ambulances or air ambulances) with doctors in medically vulnerable areas. Having been introduced in two regions in 2011, air ambulances are operational in seven regions as of the end of 2019. The flight time is from sunrise to sunset, except in Gyeonggi province, which is open all day. Although the criteria for transport vary depending on whether an ambulance is available for operation, it is basically intended for emergency critical diseases, such as severe trauma, stroke, and acute myocardial infarction. From September 23, 2011 to December 31, 2018, a total of 10,367 transfer requests were received, which included 534 (5.2%) interruptions, 2,657 (25.6%) rejects, and 7,176 (69.2%) transfers. A total of 7,209 patients were transferred during this period, which included 1,693 (23.5%) patients of severe trauma, 1,149 (15.9%) patients of stroke, 802 (11.1%) patients of acute myocardial infarction, and 3,565 (49.5%) patients suffering from other emergency diseases. Some economic research on air ambulances in Korea has been reported to be cost-effective, but additional research should be performed. In the future, it is necessary to widen the area of operation of air ambulances and find alternative means of transporting patients during unfavorable conditions such as night or bad weather.
9.Study of the Recognition of the Department of Emergency Medicine and the Direction of Development Thereof: Centering on Kwangju City and Chollanam-do.

BACKGROUND: The present study was performed in order to identify the possible future location of the Department of Emergency Medicine and future demand for and supply of manpower through a survey of the heads of Kwangju City and Chollanam-do hospitals. The survey covered the degrees of recognition and understanding, the necessity, the future prospects, and the direction of development of the Department of Emergency Medicine. METHODS: A total of 33 large-scale general hospitals were selected in county districts having more than 200 sickbeds as of November 1999. Among them, the heads of 29 hospitals responded to the survey. The survey had a total of 31 questions, including 6 questions on the status of the emergency room. Six(6) questions concerned the degree of recognition of the Department of Emergency Medicine. Six(6) questions concerned the degree of understanding of the Department of Emergency Medicine. Six(6) questions concerned the necessity for the Department of Emergency Medicine, and 7 questions concerned the future prospects and direction of development. The results of the survey were analyzed. RESULTS: 1) As to the question of the degree of recognition, the Department of Emergency Medicine was known to all. That is, 50% of the respondents were well aware of the department, and 50% a little. Nineteen(19) respondents(65%) answered that it was convenient to have the Department of Emergency Medicine in a university hospital, among which 16 respondents(55%) answered that it was because of the rapid treatment of emergency patients. 2) As to the question of the degree of understanding, particularly, the areas of medical examination and treatment by the Department of Emergency Medicine, 15 respondents answered that it was for the treatment of patients who needed to receive emergency treatment, while 12 respondents answered that it was for the diagnosis and treatment of all patients who visited the emergency room. Only 7 respondents(24%) answered positively as to the possibility of the Department of Emergency Medicine using thrombolysis medicine. 3) All except for one respondent had a positive view on the necessity for the Department of Emergency Medicine, and all except for 3 respondents wanted to have specialized doctors in emergency medicine work at the corresponding hospital. As to the type of schedule, 10 hospitals(34%) wanted to have only night shifts, and 11 hospitals(38%) wanted to have both day and night shifts. Most hospitals, needed two or more workers in the field. 4) As to the question of the direction of development, many respondents were of the opinion that the hiring of specialized doctors in emergency medicine would not be of greatly help to the finances of the hospital due to increased labor costs. It seems that improvement of the medical system to improve hospital financing is a required prerequisite for the development of the Department of Emergency Medicine. Also, 21 respondents(83%) were of the opinion that it would take a considerable amount of time to activate the Department of Emergency Medicine with respect to the future prospects of the department. CONCLUSION: It was shown in the present survey that the heads of hospitals in Kwangju City and Chollanam-do recognized the Department of Emergency Medicine in a positive way, but it was necessary to establish both a delivery system for emergency medicine and collaboration with and support by other medical areas for its future development. It was estimated that in the future, more than 55 doctors specializing in emergency medicine would be needed as medical manpower for the general hospitals in the districts.
