1.Gel Precipitation Reaction in Rats experimentally infected with Clonorchis sinensis.

In the diagnosis of parasitic helminthic diseases, the value of examining and identifying the parasitic eggs and/or adult parasites from patient's urine or stool are well appreciated. However, these methods have a limited value in the diagnosis of tissue or intracellular parasitism, and we have to rely on supplementary methods such as immuno-serological test. The author tested the value of gel precipitation reactions as a diagnostic method of clonorchiasis by observing the appearance of bads on rats experimentally infected with Clonorchis sinensis. And the therapeutic effect of CIBA 35'058-Ba was evaluated by this serological method. The antigen was prepared from the adult worms infected in rabbits by Tsuji method. Rats infected with 40 metacercariae each were bled on 7, 14, 21, 26, 28, 39, 42(43), 49(53) days after infection to observe the appearance of precipitin bands by both Pucterlony method and immunoelectrophoresis. Fifteen rats were separately infected and treated with CIBA 35'058-Ba in dose of 15mg/kg of body weight. The following results were obtained : 1. It was observed that there exist individual variations in the appearance of the first precipitin band with the range of 2-4 weeks after infection. 2. The number of precipitin bands was increased until 6-7 weeks after infection. In all cases, 3 precipitin bands were appeared by Oucterlony method and 6-7 bands were appeared by immunoelectrophoresis after 6-7 weeks of infection. 3. It was hardly possible to notice any change in the number of bands after the administration of CIBA 35'058-Ba. This result suggested that the drug has no effect on clonorchiasis which was confirmed by the autopsy of the experimental rats.

2.Ligamentoplasty in Lumbosacral Vertebrae using BOP.

BOP is made of N-Vinyl pyrrolidone-Methylmethacrylate monomer, polyamide fibres, and calcium glusconate. BOP is biocompatible, biodegradable, and osteoconductive matrix, easy to use and quite safe. Authors have operated 14 patients who has bilateral hernaiated nucleus pulposus, multiple herniated nucleus pulposus, or herniated nucleus pulposus accompanied with spinal stenosis, spondylolysis or spondylolisthesis in lumbosacral portion from September, 1989 till August, 1990. The results were satisfactory. We analysed this patients and report the method of ligamentoplasty with the discussions about spinal fusion and spinal instability.

5.Glycerol Interference on the Measurement of Triglyceride Concentration.

BACKGROUND: As more than 80% of Korean hospital laboratories don't use glycerol blank for the triglyceride measurement, we investigated free glycerol interference on the measurement of triglyceride concentration. METHODS: We collected 237 specimens which had more than 300mg/dL of triglyceride measured by Abbott (without glycerol blank, USA) reagent from the 217 patients visiting Catholic University Hospital of Taegu Hyosung from September, 1995, to May, 1996. We retested them with Youngdong (without glycerol blank, Korea) reagent, and IRC reagent (with glycerol blank, Japan), and also measured free glycerol concentration with Youngdong reagent. Then we examined the clinical records of the patient showing increased glycerol concentration. RESULTS: Average triglyceride concentration measured by Abbott reagent was 448.0+/- 165.9 mg/dL and average glycerol interference to triglyceride concentration was 3.4+/- 6.7%, There were 8 patients (3.4%) who revealed more than 10 % of glycerol interference. Except these patients the average glycerol interference was 2.4+/- 1.4%. Among these 8 patients, 3 patients were treated with Frucenil and other 3 were with Intralipose(R), and the other 2 were diagnosed as diabetes and preeclampsia respectively. In vitro test, Frucenil mixed with serum in 1% (v/v) increased triglyceride concentration as high as 640.6% with Abbott reagent, but it did only 48.4% with IRC reagent. CONCLUSIONS: Triglyceride measurement by non glycerol blank method exceeded the recommended limit of National Cholesterol Education Program's precision, and this method extremely overestimated the triglyceride concentration for the patient with the glycerol containing fluid therapy. So all laboratories would be encouraged to use the glycerol blank method for triglyceride measurements.

7.A Case of Pseudoinvasion in Peutz-Jeghers Polyposis.

Peutz-Jeghers polyp with pseudoinvasion is characterized by the presence of mucosal islands within muscle layer often associated with mucinous cysts. It occurs when mucosal components completely penetrate the muscularis propria, and shows no cytologic signs of malignancy. In most instances, the lesion may be the result of forces exerted during intussusception in the involved small intestine. We describe a case of Peutz-Jeghers polyposis with pseudoinvasion in a 24-year-old female patient. This patient presented with intussusception and a segmental resection of the ileum was performed. Microscopically, it disclosed a hamartomatous polyp with pseudoinvasion. There were glandular islands and mucinous cysts embedded within the bundle of muscle, layer, extending, to the subserosa. There was o evidence of cellular dysplasia or desmoplasia.

10.Change in Granulocyte - Macrophage Colony - Stimulating Factor in Neonatal Infection.

PURPOSE: The neuophils contribute as an important defence mechanism against bacterial infection. In neonates with infection, not only the number of neutrophils are decreased due to inhibited production but also phagocytic function is impaired resulting in high mortality rate. The purpose of this study is to establish the effectiveness of exogenous granulocyte-macrophage colony-stimulating factor (GM-CSF) in infected neonates by comparing the serum levels of GM-CSF and various hematologic paratmeters in non-infected and infected neonates. METHODS: The study included 50 neonates without infection and 23 neonates with infection who were admitted to neonatal intensive care unit of Chungnam National University Hospital in between May 1998 and February 1999. The total number of white blood cell (WBC) counts, absolute granulocyte counts (AGC) and the serum GM-CSF concentration at birth were measured in non-infected neonates and they were stratified according to birth weight and gestational age. The total number of WBC counts, AGC and the serum GM-CSF concentration at postnatal 7th and 14th day were measured and compared with that of infected neonate's. Neonates with infection were divided either to receive exogenous GM-CSF & antibiotics or antibiotics only; The total number of WBC counts, AGC and the serum GM-CSF concentration at post-treatment 2nd and 7th day were measured and compared. RESULTS: Decreased total number of WBC counts, AGC and the serum GM-CSF concentration at birth were observed with lower birth weight and gestational age. In infected neonates, the total number of WBC counts, AGC and the serum GM-CSF concentration increased more when compared with that of non-infected neonates. Tendency towards increased total number of WBC counts, AGC and the serum GM-CSF concentration were observed after exogenous GM-CSF treatment. CONCLUSION: We recommend the use of GM-CSF in neonates with infection, especially those without increasing total WBC counts, and absolute granulocyte count as a norrnal physiological response.
