1.Clinical Evaluation of Liver Function in Biliary System Surgery.

Halothane is usually a safe and effective inhalation anesthetic agent but on occasion it has damaged and the blame put on other anesthetic agents. The author selected at random 33 patients who had had biliary system surgery at Wonju Christian Hospital between December 1983 to August 1984. Their physical status was ASA class l and ll, liver function tests were abnormal state and they had no previous history of operation or liver disease. They were divided into 4 groups according to the anesthetic agent used (halothane: 10, ebtrane: 10, Thalamonal neuroleptanesthesia:10, lidocaine epidural anesthesia:3) the liver function tests were performed before surgery, and on the 1st, 3rd, 5th and 7th postoperative days. The result were as follows: 1) Total protein, albumin, and total cholesterol were not significantly changed. 2) The values of SGOT gradually decreased in the Ethrane group and they were slightly increased on the first postop day in all 3 groups. The changes of the values were in the abnormal range. 3) The values of SGOT gradually decreased in the ethrane group and they were slightly increased on the first postop day in the halothane group. 4) The values of alkaline phosphatane, total bilirubin and direct bilirubin gradually decreased in the Ethrane group and the NLA group. They were slightly increased the first posopday in the halothane group and there was no significant change in the Epidural group. it is suggested that the influences of variable anesthetic agents on liver function is not significant in biliary system surgery with abnormal liver function.

2.A Case of Intrauterine Intracranial Hemorrhage due to Coagulopathy Caused by Chronic Intrauterine Liver Failure.

The term "neonatal hepatitis" is used to designate hepatic inflammation of unknown cause. Liver failure is present with coagulopathy, metabolic instability, cholestasis, or other manifestations. It can be present with a chronic pattern with near normal serum aminotransferases, consistent with a prenatal liver injury. There have been case reports which show intrauterine intracranial hemorrhages associated with trauma, malformations or maternal coagulopathy. However, to our knowledge, there is no report of intrauterine intracranial hemorrhage resulting from fetal coagulopathy caused by chronic prenatal liver injury. We report a rare case of intrauterine intracranial hemorrhage due to intrauterine coagulopathy caused by chronic intrauterine liver failure.

3.Influence Factors on Health and Medical of Retirees Expense to Public Pension Recipients.

BACKGROUND: This study aims to identify the monthly average medical expenses of public pension recipients, and analyze the determinants of total health and medical expenses and Western and Oriental medicine expenses, medical service expenses, and medical supplies expenses. METHODS: This study used the fifth year data of 2013 out of the raw data of the Korean Retirement and Income Study collected by the National Pension Research Institute. This study conducted t-test, analysis of variance, and linear regression to verify publicly the relevance between pension recipients' general characteristics and health and medical expenses status using IBM SPSS ver. 21.0 for data analysis. RESULTS: It was analyzed that there is a difference in the spending of expenditure and health care costs according to public pension recipients. Medical expenses of the national pensioners was higher compared to the special corporate pensioners. The national pensioner is related expenditure size, education level, family members living together, residential areas, status of spouse, number of chronic illness, and status of limitation in daily life with psychological health status. CONCLUSION: Therefore, fairness does not occur fire to the medical use between the special corporate pensioners and national pensioners, aggressive of government such as health policy and financial support for the retiree pension policy that reflects the reality intervention would be required.

4.Hand Foot Mouth Disease with Meningomyeloencephalitis Caused by Enterovirus 71.

Hand-foot-and-mouth disease (HFMD) is a viral illness with a distinct clinical presentation of oral and characteristic distal extremity lesions. It is caused by members of the enterovirus genus, namely, coxsackievirus A16 or enterovirus 71. HFMD is more severe in infants and children than adults, but generally, the disease has a mild course and is self-limiting. However, in recent epidemiology of HFMD in Asia, severe cases with complications have been reported. Enterovirus 71 may cause myocarditis, pneumonia, and severe neurological complications such as brainstem encephalitis, aseptic meningitis, acute flaccid paralysis, meningoencephalitis, meningomyeloencephalitis, and even death. A large outbreak of HFMD in Taiwan and Singapore caused by enterovirus 71 had a high mortality rate; the deaths resulted from pulmonary hemorrhage, interstitial pneumonitis, or brainstem encephalitis. HFMD was also epidemic in Korea in the spring of 2010. Severe cases with complications including neurological complications and death have been reported in HFMD patients, thus it has become a public health issue. We report five children who had severe HFMD caused by enterovirus 71 with meningomyeloencephalitis which caused a disturbance in consciousness, motor weakness, strabismus, and voiding difficulty. There was no mortality, and no patients exhibited sequelae.

5.The Eosinophilic Changes in Rhinorrhea due to Nasal Allergy.

Since the early part of the twentieth century, many authors have claimed that eosinophilia is found in the nasal secretions and blood of patients with allergic diseases. This observation has now become an established fact, and recent evidence based on extensive investigation, suggests that the eosinophil may play an active role in allergic disease. Thus, we report changes in nasal eosinophils in a group of nasal allergy patients treated by specific hyposensitization. The following results were obtained; 1. Eosinophilia was noted in 52.8 percent of untreated nasal allergy patients. 2. The eosinophilic count was gradually decreased with increasing S.D.V.(specific desensitizing vaccine) hyposensitization.

6.Thoracic Epidural Anesthesia for Upper Abdominal Surgery.

Patients with advanced pulmonary tuberculosis suffer from chronic obstrutive pulmonary disease, making them poor risks for anesthesia. Also, general anesthesia for patients with active hepatitis or arteriosclerotic cardiovascular disease complicates the intraoperative and postoperative course. Inhalation anesthetics, along with the assissted ventilation used in general anesthesia, further complicate the intraoperative and postoperative course. Although regional anesthetic techiques have been used extensively for pelvic and limbic surgery to avoid the complications of general anesthesia, thoracic epidural anesthesia has not been common for upper abdominal surgery because of the fear of damage to the spinal cord. But compared with the lumbar.approach, the danger of accidental damage to the spinal cord has been exaggerated. Thoracic epidural anesthesia has been employed in a number of cases of upper abdominal surgery at Y.U.W.M.C wish remarkable success. We have in describe the use of thoracic epidural anesthesia for upper abdominal surgery.

9.The Effect of K-MBSR Program on Stress, Stress Coping Style, Depression, Anger and Sleep of Middle Aged Women.

PURPOSE: The purpose of this study was to examine a Korean Mindfulness Based Stress Reduction (K-MBSR) program for middle aged women and to verify the program's effectiveness on stress, stress coping style, depression, anger and sleep. METHODS: Fifty-two women aged from 40 to 59 (26 in the experimental group and 26 in the control group) from G city participated in the study. Data were collected from February 13 to April 3, 2013. The experimental group received 8 sessions, scheduled once a week, with each session lasting two and a half hours. Outcome variables included stress, stress coping style, depression, anger, sleep and a physiological measure (EEG). RESULTS: There were significant decreases for stress (t= - 2.14, p=.037), depression (t= - 2.64, p=.011), state trait anger (t= - 3.79, p<.001) in the experimental group compared to the control group. CONCLUSION: The findings in this study indicate that the K-MBSR program is an effective program to decrease stress, depression, and state trait anger in middle aged women.

10.The Electron Microscopic Study of Enzymes in Eosinophils.

The author has made the electron microscopic study of enzymes in eosinophils in order to clearify the influence of hyposensitization in allergic rhinitis to the activity of enzymes in eosinophilic granules and the following results were obtained. 1. In all 3 control, hyposensitization and allergic groups, eosinophilic granules with matrix and crystalloid core in circulating blood and tissue was observed. 2. In all 3 groups, activity of acid phosphatase was not found in neutrophil, basophil, macrophage and glands as a form of coagulated activating colony of acid phosphatase. 3. In control and hyposensitizing groups, number of eosinophils were smaller than that was counted in allergic group. Activity of peroxidase in granule was weak and granular out flowing and rupture of cell membrane were not observed. 4. In allergic group, eosinophil count was high, activity of peroxidase in granule was strong and granular out flowing and rupture of cell membrane were severe. At the same time, many vacuoles, which were suspected to be the result of phagocyte the protein as foreign substance, wag observed. Judging from the fact that eosinophil has a specific relation to allergic diseases and the activity of peroxidase that exist as an enzyme in eosinophilic granule is strong, it is believed that the major function of eosinophil is phagocytosis of antigen, or antigen-antibody complex. On the other hand, the fact that activity of peroxidase was weak in hyposensitizing group lead us to believe that the activity of peroxidase may be used as an indicator for detecting hyposensitizing status in the treatment of allergic disease.
