2.Treatment of Chronic Osteomyelitis by Using Antibiotic-Acrylic Bone Cement Composites: Preliminary Report of 7 Cases

Antibiotic-acrylic bone cement composites demonstrated antibacterial activity as a result of diffusion of antibiotic from bone cement into surrounding enviroment, accordingly, the use of antibiotic incorporation to bone cement for the prevention and treatment of infection after joint surgery replacement has been adovocated. Early report of clinical success by using antibiotic-acrylic bone cement composites are encouraging. Our authors applied this concept to the treatment of chronic osteomyelitis. Antibiotic contained with bone cement was chosen by sensitivity test on pus culture finding. Seven cases of chronic osteomyelitia treated by using antibiotic-acrylic bone eement composites were .studied in the Department of Orthopaedic Surgery, Kyung Hee University Hospital from July 1979 to March 1981. The follow-up period was from 3 months tc 22 months. The results were as follow: 1. Six cases showed no evidence of recurrence except one by using antibiotic acrylic bone cement composites. 2. The impregantion of antibiotic in acrylic bone cement shall be regarded to be effective in increasing the local antibacterial activity by leaching the antibiotic into surrounding tissue over a period continiously in treatment of chronic osteomyelitis. 3. The use of antibiotic-acrylic bone cement compositea into dead space by saucerization are consid- ered an one such approach to the decrease of dead space. 4. The impregnation of antibiotic in acrylic bone cement is regarded a new concept in treatment of chronic osteomyelitis.

6.The Effect of Intramedullary Vascular Bundle Implantation in the Femoral Canal

Tureta and his colleagues have stressed the important role that vessels play in osteogenesis. Numerous arthors attempted to provide new bone formation by placing vascular pedicles into the bone. Some reports are convining, but experimental model and histological investigation leave something yet to learn. We conducted an experiment to demonstrate the feasibility of using a vascular bundle to create new bone formation in medullary canal. Vasculsr bundle of the femoral artery and vein was transplanted in same side of the femoral canal of rabbits. The transplanted vascular bundle was studied histologically for eight weeks. At 3 weeks transplanted artery started proliferation of intima and media, and its lumen was obliterated completely at 6 weeks. Transplanted vein collapsed from begining, and didn't involve in revascularization and new bone formation. At 2 weeks the proliferation of capillaries promoting revascularization originated from the adventitia of the transplanted artery, and peaked at 6 weeks. New osteoid appeared around the transplanted vessel at 2 weeks, and surrounded the entire vessel in 5 weeks, and had no more progression in amount at 6 weeks. Cortical hypertrophies showed in the vascular bundle transplanted group from 3 weeks. Examination of control group revealed no sign of new osteoid formation around the hole during the whole study period.

7.PROGNOSIS FOLLOWING THE LAG-SCREW FIXATION FOR THE FRACTURED MANDIBULAR CONDYLE.

PURPOSE: This is to evaluate the clinical results and radiologic changes of the mandibular condyle fractures following the open reduction and fixation using the Lag-screws introduced by Eckelt and Martin Co. MATERIALS & METHODS: Ten patients who had been treated by the Lag-screw fixation for the unilateral fracture of the mandibular condyle at the high level and followed up for over 6 months(ranged from 24 weeks to 33 weeks). The incisal opening by time elapsed, displacement of the fragments, bone resorption around the Lag-screws, operating time consumed, and untoward complications were evaluated. The data were tested by repeated measure ANOVA and paired t-test. RESULTS: The maximum mouth opening was increased by time as follows ; 20.2+/-2.8mm soon after reduction. 26.3+/-3.9 at the 2nd week, 37.7+/-4.2mm at the 4th week, 44.4+/-4.3mm in PO 2 months(P<0.05). The bone resorption at anterior to lag-screw nut was measured to 1.9+/-1.0mm, while the posterior resorption was 2.6+/-1.9mm on average(P<0.05). Reduction and fixation of the fragments by Lag-screw were done within 80 minutes including the skin closure. The clicking sound of the TMJ(40%), weakness of the marginal branch(60%) were complicated but transient for 4-8weeks. There were no signs of bony displacement, but loosening of screws were observed at the time of removal. CONCLUSION: Open reduction and fixation with condylar Lag-screw(Martin co., Germany) thru the ramus can be a good option to reduce the high level(Kruger's Level III & IV) fracture of the mandibular condyle with anterior or medial displacement. However, this procedure requires 2nd surgery to remove the devices and it may complicate improper reduction for delayed fractures and in case of 's' curved mandibular ramus.

8.Trichobezoar dectected by ultrasonography: case report.

The authors experienced a case of gastroduodenojejunal trichobezoar detected by ultrasongraphic examination. We thought that tricholbezoar had relatively specific ultrasonographic findings such as a broad hyperechogenic superficial bandlike rim with a complete posteior sonic shadowing and multiple linear echogenic strands on its surface representing hairs, which were better seen after water ingestion. So we were able to get the correct diagnosis of trichobezoar prior to conventional barium studies in a patient who could not be suspected suspected clinically.

9.Clinical Analysis of Chronic Ischemic Foot Ulcer using Ischemic Index with Flowmeter and Wagner Classification.

It is currently necessary to manage chronic ischemic foot gangrene because the rate of hospitalization and operations is increasing as a result of diet change and the growth of an aging population. Chronic ischemic foot gangrene is caused by Buerger`s disease, atherosclerosis and particularly, diabetes. In this study, we used the Wagner ischemic index with Doppler flowmeter and Wagner classification on 62 patients over 8 years from 1991 to 1998 as an index for treatment and prognosis. We measured the systolic pressure to determine the ischemic index, and according to Wagner the blood flow was inadequate in cases of diabetic foot gangrene and nondiabetic foot gangrene when it was below 0.45 and 0.35, respectively. According to the observation of clinical symptoms, we divided the cases by Wagner classification. Based on the above data, we performed conservative treatment, skin graft, local flap, superficial sural arterial island flap and amputation. We then followed up the patients and observed the treatment results. We concluded that the Wagner ischemic index with Doppler flowmeter and Wagner classification was useful in determining, selecting, and predicting the rate of survival or death as a result of amputation and graft when a surgical operation is performed.
