4.A case of palatally impacted incisor treated by onlay technique with surgical exposure.

The patient, a girl of 19 years in good health, had a class I malocclusion. The maxillary left centra1 incisors and both lateral incisors had already erupted. But the space for the right central incisor was partially closed by the mesial drifting of the neighboring teeth. The caused a shift in the midline and a cross-bite relation on the incisors. X-ray examination revealed the presence of the right central incisor in the alveolar bone and odontoma just above the crown of the right central incisor. After enough space for the impacted incisor was created in the dental arch with a open-coil spring the rectangular incision was made. Removing the odontoma uncovered the flat surface of the labial aspect of the incisor. During the tooth had erupted of its own accord, any unnecessary force had been imposed on the tooth. When it was decided that the tooth should be brought out by the mechanical device, the gold cast onlay with hook was used and run a light elastic between this hook and the main arch wire. Finally the tooth was brought down to the arch level. The result was excellent. Fortunately the esthetic problem and any detrimental effects on the psychological make-up could be avoided.

5.Metronidazole in amoebiasis 1. The use of metronidazole on the treatment of hepatic and pulmonary amoebiasis.

Total 9 cases of extraintestinal amoebiasis, amebic liver abscess and amebic lung abscess, were treated with metronidazole. The diagnosis and progress check were done by (1) stool examination, (2) size and pain of liver, (3) hematological examination, (4) urinanalysis, (5) X-ray or liver scanning (6) examination of aspirated pus from liver and (7) immobilization test of E. histolytica. Results are summarized as follows: Amebic hepatitis: Metronidazole was given to 3 cases with daily dose of 0.9-1.2 gm for 3-7 days. Clinical symptoms were completely restored by the treatment. Amebic lung abscess: One case was treated with metronidazole for 3 days with the daily dose 1.2 gm. X-ray findings showed remarkable improvement by the treatment. An additional administration of the drug for 6 days resulted complete resolution of the pneumonic changes. Amebic liver abscess: Four cases were treated with metronidazole. The daily doses were 0.4-2.4 gm and continued for 2-10 days. Aspiration of the abscess for 1-10 times or continuous drainage of pus were performed during the course supplementarily. All cases were recovered to normal by the combining treatment. No remarkable side effect was noticed by the drug administration, and no relapse was experienced up to date.

7.Tracking the Fate of Muscle-derived Stem Cells: an Insight into the Distribution and Mode of Action.

PURPOSE: To examine the fate of muscle-derived stem cells (MDSC) after injection into different host conditions and provide an insight for their mechanism of action. MATERIALS AND METHODS: MDSCs differentiated in vitro towards the endothelial lineage and transfected with lentivirus tagged with green fluorescent protein (GFP) were injected into two animal models mimicking vascular diseases: hindlimb ischemia and carotid injury models. Injected cells were tracked at the site of injection and in remote organs by harvesting the respective tissues at different time intervals and performing immunofluorescent histological analyses. Stem cell survival was quantified at the site of injection for up to 4 weeks. RESULTS: MDSCs were successfully tagged with fluorescent material GFP and showed successful implantation into the respective injection sites. These cells showed a higher affinity to implant in blood vessel walls as shown by double fluorescent co-stain with CD31. Quantification of stem cell survival showed a time-dependent decrease from day 3 to 4 weeks (survival rate normalized against day 3 was 72.0% at 1 week, 26.8% at 2 weeks and 2.4% at 4 weeks). Stem cells were also fo und in distant organs, especially the kidneys and liver, which survived up to 4 weeks. CONCLUSION: MDSCs were successfully tracked in different vascular disease models, and their fate was assessed in terms of cell survival and distribution. Better understanding of the donor cell properties, including their interaction with the host conditions and their mechanism of action, are needed to enhance cell survival and achieve improved outcomes.

8.Preimplantation Development and Apoptosis of Mouse Embryos in the Medium Containing Extracellualr Matrix.

OBJECTIVE: To verify the effect of Matrigel, a ECM complex from Engelbreth-Holm-Swarm(EHS) mouse sarcoma on the preimplantation development and apoptosis of mouse fertilized eggs. METHOD: Late Pronucleus stage eggs were cultured through the blastocyst stage in the presence of Matrigel (0.5%, v/v). Characteristics of apoptosis and cell number assessed by Hoecst staining and TUNEL labeling at the blastocyst stage, respectively. RESULTS: Morphological development, number of cells per embryo was significantly increased but rate and number of TUNEL positive nuclei of the embryo were decreased in the presence of Matrigel. CONCLUSION: This result suggested that at low concentration of Matrigel improves both viability and morphological development in the preimplantation mouse embryos.

10.Abdominal Injury by Falls from a Height in Children.

OBJECTIVE: Falls from a height are the leading cause of injury and death among urban children. In this study, objective was to describe the incidence, clinical characteristics, and treatment results of the children who fell from a certain height and admitted over abdominal injury. METHODS: The medical records of patients under 15 years old admitted over abdominal injury by a fall from one meter or more height between January 1997 and December 2003 at Ewha Womans University Mokdong Hospital were analyzed retrospectively. RESULTS: Of 585 consecutive patients who admitted to our hospital after a fall from a height of 1 to 31.2 meters, 28 patients (4.8%) suffered from blunt abdominal trauma. The male to female ratio was 2.1: 1. The children between 12 months and 13 years old were the patients with 5.5 years old of the median age, and the median height fallen was 3 meters. Fifteen patients (53.6%) were during the summer and seventy-nine percent of the falls occurred between noon and 9 pm. Eighteen (64.3%) of falls occurred in residential place and 19 (67.8%) of patients arrived at the emergency department in 30 minutes after the accidents. Only 16 patients (57.1%) complained of abdominal pain. Liver injuries were found in 12 (42.9%), spleen injuries in 5 (17.9%), kidney injuries 3 (10.7%), pancreatic injuries in 1 (3.6%) and nonspecific abdominal injuries in 9 (32.1%) of cases. The increased SGOT and SGPT were found in each 23 (82.2%) and 18 (64.3%) of cases. Eleven patients (39.3%) had been associated with head injuries and limb injuries in each 17.9% and thoracic injuries in 7.1%. Twenty-five patients (89.3%) were recovered under the conservative treatments and the median length of hospital stay was 6 days (range, 2 -20 days). CONCLUSIONS: Despite the absence of abdominal symptoms or shock, falls from a height in children may carry a significant intra-abdominal organ injuries. Moreover the height of free fall cannot reliably predict the degree of the abdominal injury. Accordingly after a fall from height, the abdomen should be evaluated by CT scan for potential injury.
