1.Competitive exclusion against Salmonella gallinarum of Salmonella enteritidis infected chickens.

To evaluate the degree of competitive exclusion against Salmonella gallinarum(S. gallinarum) of Salmonella enteritidis(S. enteritidis) infected chickens, fifty-six, 4-week old Hyline layer suspected of S. enteritidis infection were challenged with S. gallinarum. All chickens were tested for S. enteritidis isolation using cloacal swabs and serum plate agglutination test using S. enteritidis Ag. before challenge and classified into four groups(SE isolated, SE nonisolated, SE seropositive and SE seronegative). None of the SE isolated and the SE seropositive groups died after challenge and the average weight gains were 245.5g and 254.6g, respectively. But in the SE nonisolated and the SE seronegative groups, mortality was 18.2% and 20.6% and the average weight gains were 150.1g and 111.2g. The incidence of reisolation of S. gallinarum of the SE isolated and the SE seropositive groups were 41.7% and 47.6% from liver, 33.3% and 47.6% from spleen and 8.3% and 14.3% from cecum, respectively, and the SE nonisolated and the SE seronegative group were 63.6% and 64.7% from liver, 84.1% and 88.2% from spleen and 47.7% and 52.9% from cecum. The serological response of the SE isolated and the SE seropositive groups hardly changed from 75.0 and 81.8% before challenge to 75.0 and 85.7% after. But, the other two groups were found to be significantly higher after challenge and increased from 0 and 18.2% to 100%. Consequently, S. enteritidis preinfected chickens were found to be significant different in terms of mortality, weight gain, reisolation of S. gallinarum and serological response compared to noninfected chickens. Moreover, our study shows that S. enteritidis infected chickens appear strong competitive exclusion against the colonization of S. gallinarum.

2.A Case of Congenital Arteriovenous Fistula at Left Subclavian Artery Accompanied by Notching on Superior Aspect of Rib.

High output heart failure due to congenital arteriovenous fistula is a rare disease and is difficult to treat. A 27 year old man, suffering from dyspnea, palpitation, lower extremity edema, and abdominal distension admitted May, 1987. At admission there were continuous thrill and bruit at left supraclavicular and infraclavicular area. The heart sound was irregular and rapid, and grade IV/VI pansystolic murmur was audible at the apex. Chest PA showed marked cardiomegaly with pleural effusion and notching at superior aspect of left 2nd rib. Selective left subclavian artery angiogram showed extensive congenital arteriovenous fistula at the area of left subclavian artery and its branches. After setrial feeding artery embolization and palliative operation, the fistula size decreased much with clinical improvement.

3.Comparison of Magnetic Resonance Imaging and Discography in the Diagnosis of Internal Disc Derangement.

PURPOSE: To compare the effectiveness of Magnetic Resonance Imaging and discography in the diagnosis of internal disc derangement (IDD) . MATERIALS AND METHODS: This study was confined to 90 discs of 30 patients diagnosed as IDD by MRI & disco-CT. We compared the pain nature of discogram, degree of annular tear in the disco-CT and degree of disc degeneration in MRI. The presence of HIZ (High Intensity Zone) in MRI was also compared with the pain of discogram. RESULTS: Those discs with more severe annular tears in the disco-CT showed more definite pain pattern in the discogram. More degeneration in the MRI was also correlated with more anatomical deterioration in disco-CT. Of the 27 discs with normal MRI, 6 (22%) showed severe pain provocation in discography. Of the 63 discs with degeneration in MRI, 14 (22%) showed no pain provocation in discography. Of all discs, HIZ was present in 16% (15/90) . When HIZ was present in a disc of a symptomatic patient, the possibility of it being a painful disc was 93%. CONCLUSION: In the diagnosis of IDD, MRI was helpful is seeing the degree of disc degeneration to rule out disc herniation or spinal stenosis. But the discogram is considered the only way for definite diagnosis of painful discs.

4.Last 10 years's Statistics of Newborn Babies Sex Ratio in Seoul Seong Ae Hospital and Review of Future Problems.

OBJECTIVE: This study was aimed to investigate of newborn babies sex ratio in Seoul Seong Ae hospital and review of future problem. METHODS: We reviewed retrospectively the 21,332 cases of deliveries in women visiting at Seoul Seong Ae hospital from Jannuary 1994 to December 2003 (10 year's period). We investigated 1st baby and 2nd baby sex ratio and in case of 3rd baby, subdividing then into tree groups (first and second are female, first and second are male, first and second are male and female). We arranged the data at the base of childbirth registers and patients' obligation records. RESULTS: The results were as follows, 1. The total sex ratio in last 10 years was 111.2:100 (male:female). 2. 1st baby sex ratio was 109.2:100. 3. 2nd baby sex ratio was 110.6:100. 4. 3rd baby sex ratio was 133:100. CONCLUSION: These findings suggest that unbalance of sex ratio in 3rd baby is extremely serious trend in this country and is expected a tremendous social problems in early 21th century.

6.The Study of Factors Related to Care Burden of Caregivers in Patients with Dementia.

OBJECTIVES: The purpose of this study was to examine the determinants of care burden of caregivers in patients with dementia. METHODS: 128 dementia patients and patients' caregivers participated in this study. Care burden using the BI ; cognition using the K-MMSE ; dementia severity using the CDR ; activities of daily living using the S-ADL and the S-IADL ; behaviour problems using NPI were measured. RESULTS: The results showed that care burden was related significantly with the K-MMSE, the CDR, the S-ADL, the SIADL and the NPI total score. Of the behavior problems, agitation/aggression, anxiety, disinhibition, irritability, and aberrant behavior correlated positively with care burden of caregivers. Regression analysis indicated that agitation/aggression, anxiety, the CDR contributed to care burden. CONCLUSION: Agitation/aggression, anxiety and dementia severity contributed to care burden of caregivers. The identified determinants of care burden and the stress of caregivers suggest areas of therapeutic intervention to reduce caregiver's burden so that the institutionalization can be delayed.

8.A Study on the Reliability and Validity of Seoul-Activities of Daily Living(S-ADL).

OBJECTIVES: Seoul-Activities of Daily Living(S-ADL) was developed to assess elderly person's basic activities of daily living. This study aims to develop standardized ADL assessment scale and confirm the reliability and validity of the S-ADL. METHODS: It was participated in 336 controls and 145 patients diagnosed Alzheimer's disease. RESULTS: In controls, it was statistically significant to age, but not sex, education, region and presence of spouse. Also, reliability and validity were statistically significant. Principal axis factoring analysis revealed three factors that accounted for 66.67% of the total variance(1.self-care/hygiene, 2.ambulation, 3.toileting). According to each CDR stage, there were significant difference, except for CDR 0.5 and CDR 1 suggesting early dementia. Particularly, it was remarkable for functional impairment in CDR 2 and CDR 3 suggesting moderate to severe dementia. The order of the loss of function was (1) self-care/hygiene, (2) toileting, and (3) ambulation. CONCLUSIONS: Our study showed that the S-ADL could be a very reliable and valid tool for the assessment of functional disabilities of Korean dementia patients. Particularly, S-ADL would be useful in assessing daily function of moderate to severe AD.

10.Correlation of Treadmill Exercise Test and Coronary Angiogram in Coronary Artery Disease.

The exercise ECG gives a valuable indirect evidence of insufficient myocardial perfusion during stress, but it has been impossible with ECG data alone to localize the site or sites of partial or complete arterial occlusion. The coronary angiography has proved to be valvuable as a technique for identifying anatomical lesions. However, arteriography alone dose not provide evaluation of the underlying myocardium. The two tests are complementary, one supplying indirect physiologic data and the other providing anatomical licalization of obstruction. The author reviewd 58 patients out of 86 patients who performed treadmill test and coronary angiography at the cardiac laboratory of Yonsei University Severance Hospital from January, 1981 through December, 1983. The following results were obtained. 1) Among the 58 patients in this study, 33 had negative exercise test and 25 had positive test. 2) Among the 14 patients who ended exercise test due to chest pain,2patients had negative exercise test. Coronary angiography on all the 14 patients showed significalt coronary artery stenosis. 3) With this multigraded treadmill test, the sensitivity was 68.8% and spedccificity was 88.7%. 4) The appreance of ischemic change during the early stage of exercise, the deeper ST segment depression and the downsloping ST segment have the higest specificity as indicators of coronary artery disease. 5) Four patients showed ST segment elevation in the exercise ECG, and their coronary angiogram showed 1-vessel disease in 1 patientts, 2-vessel dosease in 2 and 3-vessel disease in 3. All of them had multiple stenosis or complete obstruction of the left anterior descending artery on coronary angiogram and ventricualar dyskinesia on the left ventriculogram.
