3.Activities of Daily Living of Elderly in a Rural Area and Study for Related Factors.

BACKGROUND: The population is aging and life expectancy is increasing. It is important to evaluate the functional ability of elderly for optimal care for elderly. This study was conducted to assess functional disability of elderly and to find out the factors having an effect on the disability. METHODS: It surveyed 1,541 individuals aged 65 years and older in Cluing-Yang province. Six ADL categories (bathing, dressing, feeding, transferring, toileting, and walking) were assessed and related factors were analyzed. RESULTS: ADL was independent in 85.2% of elderly and dependent in 14.8% of elderly. 55.1% of dependent elders were living without family. Bathing was the most common disable AI)L category. The factors having an effect on the disability of ADL were brain disease, self-rated health status, dyspnea, constipation, heart disease, diabetic mellitus, and living status. CONCLUSION: In a rural area, special prevention programs for brain disease, heart disease, and diabetic mellitus and a rehabilitation health service may improve ADL status of elders.

5.In search of Jong Ha Kim, the first Korean medical doctor in modern Daejeon city

Recently, it was revealed that Jong-Ha Kim was the first self-employed Korean physician in Daejeon city of South Korea during the Japanese colonial period. However, little is known about him. In this paper, the author aimed to search for historical records of his activities as the first Korean physician in Daejeon city.Current Concepts: Kim’s clinic in Daejeon city was called Chung-Ang Clinic. This is all that has ever been confirmed about Jong-Ha Kim. The author has been searching for his medical record for the last year. Kim’s medical records were collected from journal papers on the modern history of Korea, National Institute of Korean History database, Naver news library archive, official gazette of the Japanese government-general of Korea, database of Seoul National University alumni, and a resume of a professor at the Hamhung Medical School. The results are as follows. He was born in Shinchang-ri Hamheung city Hamgyeongnam-do on August 9, 1900. He graduated from the missionary school established by Canadian missionaries and was admitted to Kyungseong (Keijo) Medical School in 1918. It was recently discovered that he participated in the Independence movement on March 1, 1919. His picture, an old leaflet for his clinic which he had used, and his resume as a professor at the Hamhung Medical School were discovered for the first time.Discussion and Conclusion: The historical records of Jong-Ha Kim’s life provide insight into the lives of contemporary doctors in modern Korea. During the period of 1900 to 1950, which encompasses his historical records, significant events took place in Korean history, and he actively participated in these events. Despite facing numerous difficulties as a doctor in modern Korea, Jong-Ha Kim made decisions that had a great impact. He was a great doctor who served as a good role model.
6.The Development of Molecular Detection Method and Differentiation of Genotypes of Enterovirus.

In this study, the feasibility of identification and genotypic differentiation of enteroviruses was investigated by using nested reverse transcription-polymerase chain reaction (nested RT-PCR), single-stranded conformation polymorphism (SSCP), and restriction fragment length polymorphism (RFLP) techniques. Two hundred seventy-four clinical samples were assayed by both nested RT-PCR and tube culture method using MRC-5 and MK cells; 58 (86.6%) out of 67 enterovirus culture-positive samples contained enteroviral RNA. In addition, 114 (55.1%) of 207 samples from patients with suspected enteroviral CNS disease with negative viral cultures were positive by the nested RT-PCR. The nested RT-PCR products were genotyped by the SSCP method and the results were compared with serotypes. We could differentiate 6 subtypes, 3 of which are similar to coxsackievirus B3, B5, echovirus 11, plus 3 other subtypes. RFLP cleaved with Sty I, Bgl I, and Xmn I yielded characteristic patterns for each laboratory strains. This study demonstrates the usefulness of the RT-PCR for the rapid diagnosis of enterovirus infection and the potentials of the SSCP method for differentiation of enterovirus strains.

8.Identification of the Source of Hematuria by the Ratio of the Urinary Erythrocyte MCV to That in Blood.

The frequent occurrence of hematuria in children necessitates diagnostic evaluation. Hematuria may occur with either renal or lower urinary tract disease. Clarification of the etiology of hematuria, whether microscopic or gross, is facilited by localizing the site of bleeding to the kidney (glomerular) or the lower urinary tract (non-glomerular). The mean cellular volume (MCV) of urinary red blood cells (RBCs) of pediatric patients with glomerular (group I; n=77) and non-glomerular (group II; n=34) hematuria was determined using Coulter Counter Model S plus IV. We found that re blood cells of glomerular origin had a smaller volume than non-glomerular cells(73.79 9.75 m3 vs 83.55 3.77 m3, p<0.001). If an urinary MCV equal to 80.56 m3 was taken as the cut-off value between glomerular and non-glomerular hematuria, a correct assessment of the site of bleeding was made in 89 (80%) of the 111 patients studied (sensitivity 76%, specificity 88%). The ratio of the urinary erythrocyte MCV to that in blood(Umcv/Bmcv) was compared with the diagnosis. If an Umcv/Bmcv ratio equal to 0.95 was taken as the cut-off value between glomerular and non-glomerular hematuria, a correct assessment of the site of bleeding was made in 93 (83%) of the 111 patients studied (sensitivity 79.6%, specificity 94%). Coulter counter analysis of urine provides a simple noninvasive and objective aid to the diagnosis of hematurai. This test, when used early in the management of pediatric patients with hematuria, may help to avoid invasive investigations.

10.4 Cases of Pseudomonas Scleritis after Pterygium Excision.

It is known that the treatment of pseudomonas aeruginosa infection associated with scleral necrosis after pterygium excision is difficult. Pseudomonas scleritis were treated with systemic and topical antibiotics, debridement, scleral homograft,and conjunctival autograft in 3 cases of 4 cases. Another 1 case was transfered to other hospital with patient`s request after short medical treatment. The interval from pterygium operation to onset of pseudomonas scleritis ranged from 2 to 17 years with an average of 10 years. In all cases with operation, the graft was stabilized after mean time of 42 days (ranged 35 to 48 days) from operation. The complications were visual disturbance, endophthalmitis, complicated cataract, exudative retinal detachment, exotropia, and posterior synechia of iris. In 1 case, a corticosteroid which was used in the early of treatment induced exacerbation of disease.Because the complications of disease were so severe,early prophylactic operation will be necessary in noninfected scleral necrosis, and when infection is suspected, it should be necessary to do culture and sensitivity test with an intensive anti-pseudomonal therapy.
