3.Erythrodysesthesia Syndrome due to Fluorouracil.

Erythr odysesthesia syndrome is a. peculiar toxic reaction due to anti-cancer drug. lt is characterized by a painful, purple-colored patch on the palms and soles, and may form bullae. We report a case of erythrodysesthesia syndrome in a 46-year-old woman. 5-FU and leucovorin were administered for 5 days because of her colon cancer. Two weeks after anti-cancer therapy, purple-colored erosive patch appeared on the face, and 10 days later, purple-colored vesicular patches were seen on both palms and soles, accompanied with pain. Histopathologic examination reveals a severe epidermal necrosis. Individual keratinocyte shows cytologic atypia and apoptotic body.

5.Short Term Effects of Hypercholesterolemia on Corpus Cavernosal Tissue in Rabbits .

PURPOSE: To study the short term effects of hypercholesterolemia on corpus cavernosal tissue, we performed isometric tension studies and RT-PCR was used to identify mRNA expression of VEGF on isolated strips of corpus cavernosum, on rabbits which underwent partial ligation of the abdominal aorta and received a diet containing 2% cholesterol. MATERIALS AND METHODS: New Zealand White rabbits were divided into a control group(n=6) fed with a regular diet and a hypercholesterolemic group(n=10) which underwent partial ligation of the abdominal aorta and received a diet containing 2% cholesterol. After 3 weeks, the relationship between hypercholesterolemia and cavernosal smooth muscle pathophysiology was studied. To characterize the cavernosal smooth muscle reactivity in hypercholesterolemia, we performed isometric tension studies with phenylephrine(PE), acetylcholine(Ach), sodium nitroprusside(SNP) and electrical field stimulation(ES). RT-PCR was used to identify the mRNA expression of VEGF. RESULTS: PE-induced cavernous smooth muscle contraction in the hypercholesterolemic group was increased compared with the control group(p<0.05), while relaxation by Ach and ES in PE pretreated cavernosal smooth muscle in the hypercholesterolemic group was decreased(p<0.05). Relaxation by SNP in PE pretreated cavernosal smooth muscle was not different between any experimental groups. Expression of VEGF mRNA in cavernosal smooth muscle was significantly increased in the hypercholesterolemic group compared with the control group. CONCLUSIONS: Hypercholesterolemia caused a significant increase in PE-induced contraction of cavernosal tissue. Hypercholesterolemic cavernosal tissue retained the ability of relaxation induced by endothelium-independent, but relaxation induced by endothelium-dependant was impaired. Furthermore, expression of VEGF mRNA in cavernosal tissues was up regulated in the hypercholesterolemic group. This study showed that a short period of hypercholesterolemia could induce pathophysiologic change of cavernosal tissue.

7.Occurrence of Respiratory Atopy in Atopic Dermatitis.

BACKGROUND: Respiratory atopy (RA) is frequently associated with atopic dermatitis (AD) and the age of onset is usually later than that of the dermatitis. OBJECTIVE: The aim of this study was to investigate the prevalence, the onset, and the duration of RA associated with AD in Korea. We also tried to correlate the existence of RA with the severity and prognosis of AD. METHODS: One hundred and eighty one patients with AD were studied. Information about the age at onset and the duration of RA were obtained from the medical history of patients or from the medical records. The severity of AD was graded. RESULTS: RA was found in 66 cases of AD (36.5%). The average age at onset of asthma was 6 years and the average duration of asthma was 6 years. The average age at onset of allergic rhinitis (AR) was 12.4 years and the average duration of AR was 6.8 years. The prevalence of asthma with AD was the most common in the age group between 8 and 11 years while the prevalence of AR was the most common in the age group between 20 and 23 years. The higher occurrence of RA in the older age group was considered to represent persistence of AD in-to adult life. We could not find any difference in the severity of AD between pure AD patients and AD patients with RA. CONCLUSION: The prevalence of RA in AD in Korea was considered to be similar to that of other reports. The average onset of asthma in Korea was a little later, whereas the average on-set of AR was a little earlier than that in the other reports. The duration of RA, which has hardly been mentioned in the literature, was considered to be about 6 years in Korea. It is likely that RA is a poor prognostic factor for AD, but does not affect severity of dermatitis.

8.Emergency nurses’ experience of coping with moral distress

Purpose:
We explored emergency nurses’ experiences of coping with moral distress.
Methods:
A qualitative research design was used. We conducted in-depth interviews with 11 nurses working at a regional emergency medical center in South Korea. Data were analyzed using conventional content analysis.
Results:
The main theme of nurses’ coping with moral distress was “a passive emotion-based response.” We also extracted three categories of coping strategies: “uncritical adaptation to anguished situation,” “self-justification for not acting,” and “flight from ethical decision making.”
Conclusion
Nurses coped with moral distress in a passive and emotional way, possibly because of perceived incompetence and compliance to an organizational hierarchy. The findings imply that effective approaches to addressing moral distress in emergency nurses are needed at the individual and organizational levels.
9.The Usefulness of Indices for Central Obesity Estimated by Bioelectrical Impedance Analysis in the Diagnosis of Metabolic Syndrome.

BACKGROUND: Recently-developed equipment based on bioelectrical impedance analysis (BIA) not only measures total body fat but also displays several estimated indicators that reflect intra-abdominal fat, such as waist circumference (WC) and waist-to-hip ratio (WHR). This study examined the usefulness of these indicators in the diagnosis of metabolic syndrome (MS). METHODS: A total of 632 people over 20 years of age (355 men and 277 women, mean age 48.61+/-11.08 years, mean BMI 23.62+/-3.00 kg/m2, 117 MS patients) were enrolled in the study. Measurements of WC and hip circumference were measured by one individual, and WHR was calculated. BIA was performed to estimate waist circumference (BIAWC) and waist-to-hip ratio (BIAWHR). Receiver operating characteristic (ROC) curve analysis was used to examine the usefulness of BIAWC and BIAWHR in diagnosing MS. RESULTS: The areas under the curve (AUCs) were 0.836 (95% CI 0.805-0.864) for WC, 0.814 (95% CI 0.782-0.844) for BIAWC, 0.815 (95% CI 0.782-0.844) for WHR, and 0.805 (95% CI 0.772-0.835) for BIAWHR. The difference between the AUCs of WC and BIAWC (0.022, 95% CI -0.004 to 0.048) and the difference between the AUCs of WHR and BIAWHR (0.010, 95% CI -0.015 to 0.034) were not significant. CONCLUSIONS: The indices for central obesity estimated by BIA had high agreement with the direct method, and they were not inferior to direct measured indices for predicting metabolic syndrome.
