2.Transcatheter Arterial Embolization of Splenic Artery Aneurysms: A Single-Center Experience.

PURPOSE: The purpose of this study was to report on splenic artery aneurysms (SAAs) treated by transcatheter embolization in our single-center institution and to evaluate the clinical outcomes of patients with SAA by aneurysm location. MATERIALS AND METHODS: The original medical records and imaging results of 52 patients with SAA treated in our center between January 1, 1995 and December 31, 2013 were reviewed. Of these cases, 7 patients (13.5%) underwent surgery, 4 patients (7.5%) underwent serial observation, and 1 patient had stent insertion only, leaving 40 patients (78.9%) who underwent endovascular treatment using a coil, with or without N-butyl-2-cyanoacrylate. RESULTS: Aneurysms were located in the distal third of the splenic artery in 27 patients (67.5%), in the middle third in 9 cases (22.5%), and in the proximal third in 4 cases (10%). Of the 40 included patients, 25 were female (62.5%). Twenty-eight patients (70%) were asymptomatic. The mean aneurysm diameter was 2.48 cm (range, 0.8-6.0 cm). Complications involved pancreatitis (n=1) and early spleen infarction (n=29: <1/3 in 14, 1/3-2/3 in 10, and >2/3 in 5). Postembolization syndrome was noted in 26 patients (65%). There were no significant differences by aneurysm location in the postoperative increase in the values of white blood cells, amylase, lipase, and C-reactive protein (P=0.067, P=0.881, P=0.891, and P=0.188, respectively). CONCLUSION: At our institution, endovascular management is safe, has high technical success, and represents the first-line treatment for SAA, regardless of aneurysm location.

3.Albendazole Therapy for Multiple Neurocysticercosis.

Albendazole is a new drug for the treatment of cerebral cysticercosis. We have recently experience a case that had shown poor therapeutic response to praziquantel. The patient have multiple parenchymal cysts. She was treated with 15mg/kg/day of albendazole for 1 month to confirm healing of the intracranial lesion by the follow up brain CT scan and CSF-ELISA associated with marked clinical improvement.

4.Injury of the Genitourinary Tract, Sung Won Kwon. M.D. and Dong Han Kim. M.D..

Ninety-one cases of genitourinary tract injuries which were admitted to Severance Hospital during the period from Jan. 1960 to Aug. 1968 were analyzed clinically and statistically. And it was investigated compared with previous papers 1) The rate of genitourinary injuries accounted for 7.3 per cent to the total admission patients during eight years sixty-nine per cent of all injuries were in men and thirty-one per cent in women. The age distribution was much the same as that found in previous literature. Most favorable age was in from 20 to50 years for 72.5 percent 2) The meet common involvement site was in urethra for32.9 per cent while external genitalia showing most common site in war wound Also iatrogenic ureteral injuries was relatively high in spite of most less commonest incidence in previous paper 3) Thirty-six per cent of all injuries weredue to traffic or vehicular accident and twenty-four per cent 'due to iatrogenic. Majority of these iatrogenic injuries occurred in pelvic operation. Falls kick and blow were listed not less frequently. 4) On observation of kidney injuries. predisposed hydronephrotic kidney was injured more frequently due to mild external forces and three cases of sixteen kidney injuries had hydronephrosis dueto stone and congenital ureteral stricture in this series 5) An incidence of ureteral injury during radical hysterectomy was variable in literature. but it was noted around 10 percent generally. In reviewing 197 cases of radical hysterectomy for carcinoma of cervix, incidence of ureteral injuries was 12 cases(7.3 per cent) and it was listed 9.3 percent including bladder injuries. 6) In investigating bladder injuries, 53 percent of injuries were associated with pelvic bone fracture And 87 per cent was extraperitoneal type and 13 per cent wasin intraperitoneal rupture. 7) Also majority of urethral injuries were associatedwith pelvic bone fracture especially in car accident cases and then involved posterior urethra for 85 per cent. And bulbous urethra was injured by straddle injury for 70 per cent.

7.Analysis of scientific literatures on the oral &maxillofacial surgery from the view point of informational science: Korean, Japanese, American, and International joms.

In order to examine a way of utilizing the papers published in the field of oral &maxillofacial surgery, the subjects and the references from total 3,392 papers. ; 263 papers in the KJOMS(Journal of Korean Association of Oral &Maxillofacial Surgeons. vol. 17-21), 237 papers in the KJMPRS(Journal of Korean Academy of Maxillofacial Plastic and Reconstructive Surgery. Vol 13-17), 1,233 papers in the JJOMS (Journal of Japanese Association of Oral &Maxillofacial Surgeons. vol 37-41), 1,212 papers in the AJOMS(Journal of American Association of Oral &Maxillofacial Surgeons. vol 49-53), and 447 papers in the International JOMS(Official publication of the International association of Oral &Maxillofacial Surgeons vol 20-25) have been analyzed. The results were as follows: 1. The subjects of KJOMS and KJMPRS were composed of various 28 items. Among 15 main subjects excluding the minor subjects below 1%, there were Aquired deformity &-Reconstruction (17.4%), Jaw deformity (14.2%), Trauma (14.2%) and Malignant tumor (10.8%). 2. The main subjects of JJOMS were Malignant tumor (22.8%) and Benign tumor (20.6%), In AAOMS, Trauma (13.0%), Jaw deformity (13.0%), and malignant tumor 12.4%) were main subjects. In IJOMS Malignant tumor (16.1%), Acquired deformity &Reconstruction (13.0%), and Trauma (10.3%) were main subjects. 3. IN KJOMS and KJMPRS, Scientific and Clinical articles was 63.5% and Case reports was 36.5%. But scientific and Clinical articles was 43.4% and Case reports was 56.6% in JJOMS, 47.1% and 52.9% in AAOMS, 50.1% and 49.9% in IJOMS. 4. The number of institutes in KJOMS and KJMPRS was 46. The main 6 institute published the 57.4% of total articles. In IJOMS, Nationality of the author was composed of 42 nations. West Europe Area published 52.8% of total articles, and The dominant nation of the author were West Germeny (16.3%), U.K. (12.1%), and Japan (11.6%). 5. Authors cited 30.4 references per articles in average, but only 2.7 domestic articles was cited in KJOMS and KJMPRS. It was 17.4 references, but 9.3 domestic articles was cited. In AJOMS and IJOMS, were 19.5 and 18.0 references.

10.A Comparative study of the Implants used in the Management of Blowout Fracture.

We developed an animal model to recreate the condition of an open fracture in communication with the maxillary sinus. We then studied wound healing of the sinus wall structures following fracture in the presence of autogenous bone and alloplastic implant. This model is designed to simulate the repair of an orbital floor fracture in humans. The New Zealand White rabbit was used as the animal model. Standardized 8mm defects were made bilaterally in the maxillary sinuses to include bone and mucosa in 36 rabbits. Two different implants and autogenous calvarial bone graft were placed in the soft-tissue pockets to obturate the defects, exposing one surface of the implant to the open sinus. Medpor porous polyethylene, silicone and calvarial bone implant were compared. Animals were killed at 1, 2 and 8 weeks after implantation. Gross examination of the specimens for the amount of mucosal closure and implant tissue fixation was performed. Histological sections were evaluated for bone and soft-tissue morphology juxtaposed to the implant. Complete closure of the mucosal defect was demonstrated with each type of implant. Medpor implants showed both vascular and soft-tissue ingrowth into pores by week 1. Bone ingrowth was seen by week 2. Closure of the Medpor obturated defects occurred more rapidly than in the silicone group. The Medpor implants and calvarial bone demonstrated bone and soft-tissue fixation, callus formation and maturation, while mature overlying mucosa was reconstituted over the defects. Silicone implants demonstrated a fibrous tissue reaction within 1 week of implantation and they never became fixed to bone or soft tissue. Maxillary sinus wall regeneration occurred in all defects. This study supports clinical observations of maxillary sinus wall regeneration in humans.
