Acute colonic pseudoobstruction was first reported in 1948 by Sir H. Ogilvie. It is characterized by an acute, abrupt right-sided colonic distention in the absence of physical reasons for obstruction. If untreated, it may progress to cecal perforation, peritonitis, and death. Although primary cases have been reported, 87 percent of cases were secondary results of medical or surgical conditions. The most common medical conditions were infections, cardiac disease, and neurologic problems. In surgical conditions, cesarean section has been reported as the most common cause of Ogilvie's syndrome. In 1995, the first case of Ogilvie's syndrome complicated after a cosmetic surgical procedure(abdominoplasty) was reported by Bradley et al. We report a case of Ogilvie's syndrome complicated after breast reconstruction with TRAM flap that developed in the postoperative third day in 35-year old woman. The complications are primarily related to decreased flap perfusion. No reference of intestinal pseudoobstruction was found in the literature conducted in regard to complications of TRAM flap breast reconstruction.

Aortic root diameter by two dimensional and M-mode echocardiography in predicting prosthetic aortic valve size preoperatively was measured in 10 adult patients undergoing aortic valve replacement. Correlation of aortic root diameter measured by two dimensional echocardiography and actual prosthetic valve size implanted by aortic valve replacement was excellent. Correlation coefficient was statistically significant (r=0.91, P<0.001). Correlation of aortic root diameter measured by M-mode echocardiography and actual prosthetic valve size was also excellent. Correlation coefficient was statistically significant (r=0.86, P<0.001). This study demonstrates that aortic root diameter by two dimensional and M-mode echocardiography can accurately predict prosthetic aortic valve size in patient undergoing aortic valve replacement.

We investigated the poor prognostic factors for trans pars plana vitrectomy in the complicated cases of proliferative diabetic retinopathy. A total of 150 eyes in 125 patients with complications of diabetic retinopathy were treated surgically. The postoperative visual acuity was decreased according to Snellen E chart compared to the preoperative in 53 eyes out of 150 eyes. The cases with poor visual outcomes are as follows; 4 cases with rubeosis iridis, 12 of 14 cases with retinal detachment involving three qua tors of the retina including the fovea, 6 cases with increased IOP(>21 mmHg) and 23 of 30 cases with chronic renal failure.

We evaluated the effects of eye movements on facial feature recognition and memory retrieval. Thirty-eight participants learned the faces of five men, including features of the faces (eyes, nose, and mouth), and then performed a recognition memory task for partially manipulated versions of the faces. Bilateral eye movements, recognition accuracy, and mean fixation duration were evaluated. We observed differences in fixation durations for the manipulated features of the faces (eyes, F(3,78)=11.95, P<0.001; and mouth, F(3,78)=21.38, P<0.001). These findings demonstrate that eye movements have a functional role in learning and recognizing human faces. Furthermore, fixation durations increased for the manipulated facial features, suggesting that eye movements during recognition are not simply patterns produced during learning.

OBJECTIVE: Many evidences raise the possibility that the panic disorder (PD) patients with respiratory subtype (RS) may have characteristic structural abnormalities. We aimed to explore the structural differences between PD patients with and without the respiratory symptoms. METHODS: Patients with PD were recruited from the Department of Psychiatry at Korea University Anam Hospital. Respiratory subtype (RS) was diagnosed when at least 4 out of 5 of the following respiratory symptoms were present during the panic attack: fear of dying, chest pain/discomfort, shortness of breath, paresthesias, and a choking sensation. We acquired high-resolution MRI scans and used FreeSurfer to obtain a measure of cortical thickness for each patient. RESULTS: Cluster based analysis revealed significantly decreased cortical thickness in the left hemisphere in the caudal-middle-frontal, superior frontal, and posterior parietal areas in the RS group. No significant difference was observed in any of the limbic areas. CONCLUSION: Respiratory symptoms of panic disorder were associated with a reduction in cortical thickness in the left frontal and parietal areas. This finding leads to the assumption that the frontoparietal network is the crucial component in a larger cortical network underlying the perception of dyspnea in RS.

Thrombotic thrombocytopenic purpura (TTP) is an uncommon disorder, frequently fatal, characterized by a pentad of thrombocytopenia, microangiopathic hemolytic anemia, fluctuating neurologic symptoms, fever, and renal disease. TTP is characterized by widespread noninflammatory thrombotic occlusions of arterioles and capillaries with material containing fibrin and platelets. Generally, gross infarct is rare in the patient with TTP. We present a case of TTP with multiple gross infarcts on brain CT scan. A 53-year-old-man was presented with left hemiparesis and confused mentality. On laboratory study, he had microangiopathic hemolytic anemia & thrombocytopenia. Brain CT showed multiple gross infarct areas in the right basal ganglia, right parietal lobe and right cerebellar hemisphere. When managing the patient with acute stroke syndrome, we should consider TTP as a diagnostic possibility for early proper treatment, even if CT scan of brain show multiple gross infarctions.

Purpose: The purpose of this article is to evaluate effects of self-determination theory-based on-campus and off-campus prevention programs on smartphone overdependence among university students. Methods: This was a pre-posttest quasi-experimental study with a nonequivalent control group (CG). Seventy-eight students were recruited as participants. They were allowed to choose either an experiment group (EG) or a CG. On-campus smartphone overdependence prevention program was provided to participants in experimental group 1 (EG1), while on-campus program combined with off-campus prevention camp was provided to those in experimental group 2 (EG2). Instruments used in this study included a smartphone overdependence self-diagnosis scale, a basic psychological needs scale, and a self-regulation ability scale. Data collection was performed at baseline, immediately after intervention, at 1 month and 3 months after intervention. Data were analyzed using mixed analysis of covariance. Focus group interview was performed for qualitative evaluation. Results: After the intervention, smartphone overdependence and basic psychological needs exhibited significant interactions between group and time. Smartphone overdependence scores decreased in EG1 and EG2 but increased in CG (F = 4.56, p = .001). Basic psychological needs improved in EG1 and EG2 but deteriorated in CG (F = 5.04, p = .009). Focus group interviews revealed that participants strived to control their smartphone usage through individual efforts and by interacting with new friends in college even after completing the program. Conclusion In this study, on-campus only program and combined intervention of on- and off-campus programs were both effective in maintaining and managing smartphone use. However, participants perceived that the off-campus program provided an opportunity to apply the theory learned in on-campus to the real world.
