2.Species Identification of the Genus Aeromonas Isolated from Freshwater Fish Culture-ponds and Seawater in Korea and Their Antibiotic Resistance Patterns.

Some species of mesophilic bacteria in the genus Aeromonas were isolated from a snakehead (Channa argus) culture-pond in the Myung-Ji area, a freshwater culture-pond in Pukyung National University, and seawater near the sewage treatment plant in Kwang-An Beach in Pusan. They were identified by biological characteristics, and their susceptibility to ten antibiotics including ampicillin, cefoxitin, ceftazidime cephalothin, cefotiam, chloramphenicol, nalidixic acid, rifampin, cefoperazone-sulbactam, and imipenem, was also studied. Four species in the genus Aeromonas, A. veronii biovar sobria (27 strains), A. caviae (14 strains), A. trota (1 strain), and A. schubertii (1 strain) were identified. A. trota, an unrecorded species in Korea, is reported for the first time in this study. Three strains of A. veronii biovar sobria showed multiple resistance to three kinds of antibiotics and one strain to four drugs. All strains of A. caviae were resistant to more than three drugs and seven strains to four drugs. A. trota was resistant to cephalothin and rifampin, but A. schubertii showed multiple resistance to ampicillin, nalidixic acid, and rifampin.

3.A Case of Spindle Cell Lipoma.

A 50-year-old man developed 3cm sized, well-marginated, subcutaneous mass on the right upper back. Histopathologic finding showed the lipomatous tissue was replaced by a mixture of uniform spindle cells and mature fat cells that were closely associated with a mucoid matrix and a varying number of collagen fibers. The patient had been treated with excision and showed no evidence of recurrence.

5.A Case of Risperidone-induced Neuroleptic Malignant Syndrome.

Neuroleptic malignant syndrome(NMS) is an uncommon but potentially fatal idiosyncratic reaction to neuroleptics, characterized by muscular rigidity, fever, autonomic dysfunction, and altered consciousness. The major theories to explain NMS is central dopaminergic blockade, but it is unclear. Risperidone is a new antipsychotic drug, a benzisoxazole derivative that blocks dopamine D2 receptor and serotonin type 2 receptor. The comparatively greater serotonin-blocking activity is believed to give risperidone the specific property of not causing any more extrapyramidal side effects than conventional antipsychotics at the optimal dose of 4-8mg/day. It is postulated that risperidone is unlikely to cause NMS. Here, we report a case of risperidone induced neuroleptic malignant syndrome.

6.Giant Intra-articular Osteochondroma of the Knee: A Case Report

The giant intra-articular osteochondroma of the knee is very rare and is probably result of synovial metaplasia of the fibrous capsule or of the adjacent connencive tissue of a joint, and of the fragment of articular cartilage. The gross findings and histology are consistent with the osteochondroma. The authors experienced a case of giant intra-articular osteochondroma of the right knee in a 19 years old man who had history of knee injury when he was 16 years old. The mass was excised and the symptome was relieved.

7.Development of a LC-MS/MS for Quantification of Venlafaxine in Human Plasma and Application to Bioequivalence Study in healthy Korean Subjects.

A simple, rapid and selective liquid chromatography coupled with tandem mass spectrometry (LC-MS/MS) is developed and validated for quantification of venlafaxine in human plasma with simple liquid-liquid extraction step consisted of extraction with ether and dichloromethane for 10 min and mixing with 1 M sodium acetate in human plasma using fluoxetine as an internal standard (IS). The analyte are separated using an isocratic mobile phase consisted of acetonitrile and 5 mM ammonium formate (4/3, v/v) on a isocratic YMC hydrosphere C18 (2.0x50.0 mm, 3.0 microm) column and analyzed by MS/MS in the multiple reaction monitoring (MRM) mode using the transitions of respective [M+H](+) ions, m/z 278.2-->260.3 and m/z 310.1-->148.1 for quantification of venlafaxine and IS, respectively. The standard calibration curves showed good linearity within the range of 1.0-200.0 ng/mL (r2=0.9986, 1/chi2 weighting). The lower limit of quantification (LLOQ) was 1.0 ng/mL. The retention times of venlafaxine and IS were 0.6 min and 0.7 min that means the potential for the high-throughput potential of the proposed method. In addition, no significant metabolic compounds were found to interfere with the analysis. Acceptable precision and accuracy were obtained for the concentrations over the standard curve range. The validated method was successfully applied to bioequivalence study after 75-mg of venlafaxine sustained-release (SR) capsule in 24 healthy Korean subjects.

8.A Case of Venous Hemangioma Occuring on The Interdigital Web.

We present a case of venous hemangioma which is seen beneath a clavus (corn) developed in the interdigital area of the foot. Clinically, a violaceous papule was located on the third and fourth interdigital spaces under an overlying corn. Histopathologic examination showed a non-encapsulated dermal mass composed of thick-walled and thin walled vessels lined by a single layer of endothelial cells. Some thick-walled vessels had protruding endothelial cells into the lumen and through Verhoeff-van Gieson and Masson-trichrome stain of the tissue section, we found that the vascular mass was composed of vein.

9.Complications after Intravitreal Triamcinolone Acetonide Injection: Incidence and Risk Factors.

PURPOSE: To reduce complications and provide proper management for better clinical outcomes in the treatment of macular edema through a large-scale analysis of the incidence and risk factors for complications of intravitreal triamcinolone injection. METHODS: The medical records of 490 eyes that received intravitreal triamcinolone (4.0 mg) injection for treatment of macular edema caused by various diseases were retrospectively reviewed, and relevant parameters were included in a multivariate regression model. RESULTS: Vitrectomized (p < 0.001) and pseudophakic (p = 0.054) eyes were less likely to exhibit an intraocular pressure increase, but glaucoma (p < 0.001) and young age (p = 0.073) were found to be significant risk factors for this complication. Vitrectomized eyes (p = 0.011), diabetic macular edema (p < 0.001), secondary ocular hypertension (p = 0.029) and old age (p = 0.059) were associated with cataract progression. Three consecutive case of sterile endophthalmitis, which differs from bacterial endophthalmitis in clinical course and prognosis, occurred during the same period. CONCLUSIONS: Risk factors and the incidence of complications after intravitreal triamcinolone injection should be evaluated through regular follow-up evaluation to ensure proper management and a better prognosis.

10.Prognostic Factors to Final Results after Conservative or Surgical Treatment of Thoracolumbar Burst Fractures.

STUDY DESIGN: This study assessed the final functional results after treatment of thoracolumbar burst fractures and compared the relationship between the results and the parameters of reduction and state of the fractures. OBJECTIVES: To define prognostic factors affecting the final results and to present some precautions to minimize the treatment failure. SUMMARY OF LITERATURE REVIEW: In the treatment of the thoracolumbar burst fractures in which flexion loads are predominant, the sagittal contour is crucial to achieve permanent pain-free stability, but definitive therapeutic guidelines have remained a controversal topic. MATERIALS AND METHODS: We reviewed 37 thoracolumbar burst fractures with an average follow up period of 1.8 years: group 1 consisting of 20 cases treated conservatively and group 2 consisting of 17 cases treated surgically with posterior instrumentation. Finally functional results were analysed with the Denis'pain and work scores, and were compared between groups on anterior body height and local kyphosis. RESULTS: A satisfactory pain score less than or equal to P3 was in 15(75%) in group 1 and 15(88.2%) in group 2(p>0.05), but satisfactory work score less than or equal to W3 was in 12(60%) in group 1 and in 14(82.4%) in group 2(p<0.05). But, some loss of body height and local kyphosis in group 1 was not reversely related with functional outcomes. Eight cases in group 1 showing unsatisfactory result in work scores were analyzed as 4 osteoporosis(Jikei grade I, II/III), 3 associated compression fracture of the contiguous vertebra and one combined osteoporosis and compression fracture, showing significant loss of vertebral height and increase of kyphosis(p<0.01). CONCLUSIONS: Functional results of group 1 showing loss of vertebral height less than 50% and increased kyphosis less than 200 were comparable to those of group 2. The osteoporosis and associated compression fracture of adjacent vertebra were the risk group to develop posttraumatic kyphosis and might be added to the surgical indication of the thoracolumbar burst fractures.
