2.Clinical Observation of Sporotrichosis during a Twelve Year Period (1967 - 1978).

Observations of 50 sporotrichosis patients, who were confirmed by dinical, histopathological and mycological examination, were made in the department of Dermatology at Chon-nam University Hospital for the twelve year period from 1967 to 1978. The results obtained were as follows; 1. During this period, out of a total of 39, 594 outpatients, there were 50 cases of sporotrichosis for an average of 0. 13 percent. The highest incidence was in 1970 with 7 cases, however, there was no period with a particularly marked increase in the occurrence of the disease. 2. Half of all the cases occurred in the third and fourth decades of life. The male to female ratio was 2. 3:1. 3. Occupationally, the greatest percent were farmers, 36 cases(72%), and the next highest, unemployed. 4. In Kwang-ju an.d surrounding 6 counties, there were 25 cases which were widely scattered thraughout Chonnam province and the southern tip of Chonbuk province. 5. The initial lesion appeared on the upper extremities in 39 cases, particularly the right hand, and the lower extremities in 9 cases. 6. Of the total patients, there were 24 cases(48%) with a history of trauma before the onset of the disease. Of those cases, the disease appeared between I and 2 weeks. after trauma in 10 cases(41.7%) and between 2 and 3 weeks in 8 cases(33.3%). 7. The duration of the skin lesions before initial examination was 2 months in 12 cases(24%) and 3 months in 14 cases(28%). 8. Forty six percent of the total cases visited our department in spring, showing a higher incidence than any other season. The highest monthly incidence was in May, when there were 13 cases. 9, Of the total, 42 cases(84%) were cutaneous lymphatic type, while 8 cases(16%), localized cutaneous type.

3.A Case Report of Pleuropulmonary Blastoma in Childhood.

Classic pulmonary blastoma is a variant of carcinosarcoma which is seen almost exclusively in adults. By contrast, most cases of pulmonary blastoma in children have been described as having an exclusive mesenchymal composition, which was proposed as pleuropulmonary blastoma. Recently we experienced a case of pleuropulmonary blastoma, type 1. This 27-month-old male baby was transferred to our hospital due to the left tension pneumothorax. Chest CT revealed a subpleural pulmonary cystic lesion on the left upper lung and an open cystectomy was performed. Histologically the lesion was composed of variable-sized cystic structures lined with a single layer of respiratory-type epithelium. the underlying stroma was composed of sheets of small, round to oval, primitive tumor cells. Some of them had eccentric, eosinophilic cytoplasm, suggestive of rhabdomyoblastic differentiation. These rhabdomyoblastic cells were fuchsinophilic and positive with desmin and vimentin on immunohistochemistry.

4.Treatment of Bone and Joint Exposure of Finger

Severe injuries of hands frequently expose the bone and joint due to the defect of crushing of soft tissues. Although the several methods are introduced, the treatment of bone and joint exposure of finger and hand is very difficult to control satisfactorily. From October 1986 to February 1988, we performed the pedicled skin flap in 11 cases; 9 abdominal skin flaps and 2 pectoral skin flaps. Follow up stuides showed good results. The successful surviving of flaps was accomplished in all cases and the dead bones were regenerated by the creeping substitution. Therefore this pedicled skin flap operation can be considered to be a simple and effective method in treating the bone and joint exposure of fingers.

5.Variable complementary combined radiologic imaging methods for breast diseases

Radiographic examination of the breast has been so improved that it became a routine complement to physicalexamination. From November 1, 1983 through Sept.30, 1984, 684 patient with complaints of various breast problemwere examined by low-dose film mammography at Yong Done Hospital, Yonesei University. Among them, a comparativestudies, independently conducted physical examination, 97 cases of film mammography, 35 cases of ultrasoundmammography, 16 cases of aspiration cytology, and 3 cases of galactography were performed for our pathologicallyproven 98 cases of breast disease. Combined and complementary studies for breast diseases were analyzed in 37proven cases and authors found that specificity of those combined immediate complementary study techniques forbreast disease were 94% under colose cooperation with surgeon in Yong Dong Hospital, Yonsei University.

9.Study on the Development of a Rapid Detection Method for Pathogenic Yersinia enterocolitica and Yersinia pseudotuberculosis by Nested Polymerase Chain Reaction.

A nested polymerase chain reaction (PCR) was applied to detect and identify pathogenic Yersinia enterocolitica and Y. pseudotuberculosis. We used photochemical postamplification procedure with 8-methoxypsoralen to control carryover contamination. Using the ail and inv gene, the sensitivity and specificity of DNA amplification by nested PCR was considerably improved. The amplified fragment sizes were 298 bp for the ail gene and 295 bp for the inv gene. Amplification was successful when the template was derived from three sources: purified DNA, aliquots of boiled bacterial suspension and aliquots of lysed bacterial suspension. The detection limits were 10 fg of DNA and 2 * 10 colony forming units (CFU) for Y. enterocolitica and 10 fg DNA and 2 CFU for Y. pseudotuberculosis.
