2.Mycological findings of Trichophyton tonsurans isolated in New Orleans area.

There had been no reports of trichophyton tonsurans infection in Korea yet. We have much chances to import the organism through international travels and exchanges. But we, Korean, have no experiences to observe T. tonsurans. Author tried mycological studies with T. tonsurans insolated in New Orleans, USA. The results are as follows: Gross findings of T. tonsurans showed that fine granular surface with light yellow hue or white color and grooving in the central area on the front side and mahogany brown color on the reverse side. On the urease test, various reddish discoloration was noted. Microscopic findings showed that septated hyphae, macroconidia, chlamydoconidia and microconidia. Macroconidia were 305 septated, smooth-surfaced, and were found more frequently in the whitish colony. Microconidia were characteristic in their arrangement and shape; round or oval shaped microconidia laterally to hyphae, some were match-head like terminal swelling. Hair perforation test showed positive results in 16 strains among 19 tested strains. All these findings are similar to T. rubrum and T. menatgrophytes, the most frequent isolates of dermatophytes in Korea, and we have to pay an attention to differentiate carefully.

3.Prevalence of Capillaria hepatica among house rat in Seoul.

During May-July, 1978 a survey on the prevalence of C. hepatica among house rat(Rattus norvegicus Erxl.) and wild mice (Mus musailus molossinus and Microtus mandarinus) in two districts in Seoul was carried out. Approximately 38 percent of 1,000 house rats examined were infected. Rat population as an established group in Ahyundong, neighbouring the city, revealed 39.2 percent of infection among 660 house rats, and that as newly formed group in Shinrimdong, a developing residential quarter on the southern outskirts of Seoul showed 35.9 percent among 340 rats. Infection rate of females and males was 38.9 percent and 37.2 percent respectively. Differences of infection rates by locality and sex were not statistically significant. It was very interesting that none of 64 wild mice were infected with this parasite in both districts. Some probable factors which are related on C. hepatica infection among rats were discussed briefly.

7.A Simulation Model for Strategic Planning in a Community Hospital.

In this paper strategic planning is applied for effective management to improve hospital income. Though community hospital has been set up for improving the health status of community, it must be trying to increase income for continuing with referral hospital in the community. The most useful tool of management to do this purpose is thought of strategic planning. Therefore this paper developed strategic planning a typical community hospital, which for it presenting the strategic planning process and developing the simulation model for evaluation of strategic alternatives. In the result it is represented that admission rate is the most sensitive to hospital operating income among the important variables in the model. And simulation by changing the parameter could estimate the yearly hospital income in the response of alternatives. This strategic planning simulation model has had constraints as number of date but also significant such as evaluating the alternatives by quantitative model and at first applying the hospital in our country.

8.Studies on the incidence of the intestinal protopozoa in the mountainous area of Chollapuk-do.

This study was undertaken in order to elucidate the prevalences of the intestinal protozoan infection according to localities and the mode of the infestation in the mountainous area of Chollapuk-do, Korea during the year 1970-1971. 8,508 of fecal samples were collected among the inhabitants of the mountainous area, which covered 18 Myens(=Township) from 6 Guns(=County) in the eastern part of Chollapuk-do. Most of the inhabitants of this area were engaged in farming and living under lower economic and poor sanitary conditions. The samples were examined twice repeatedly by iodine stained direct fecal smear method. For the study on the mode of infestation, with the exception of drinking water, the zinc sulfate floatation technic was applied for the examinations of hog-feces, manure heap, garden soil, sewage, dishtowel, dust in room and floor, house fly and house rat. The results were summarized as follows: Among total 8,508 fecal specimens examined, 3,152(37.6 percent) were positive for the intestinal protozoa: E. histolytica 9.8 percent, E. coli 25.6 percent, E. nana 7.2 percent, G lamblia 6.8 percent, I. butschlii 0.6 percent, and C. mensnili 0.7 percent. A single infection of the intestinal protozoa was the highest(68.3 percent), and double infection(22.3 percent) triple, quadruple and quintuple infections were in decreasing order. The highest distribution (39.8 percent) was observed in the group aging 41-50, and the lowest (27.3 percent) in the group above 61. The groups under 10 years showed a relatively high positive rate among all age groups. Sexual distribution of protozoan infection showed slightly higher rate (38.6 percent) in male than that (35.4 percent) of female. Jangsoo-gun showed the highest positive rate (42.9 percent) and Namwon-gun was the lowest (32.5 percent). The prevalence of E. histolytica showed the highest (11.6 percent) in Imsil-gun and the lowest (6.9 percent) in Jangsoo-gun. E. histolytica cysts were found 9.1 percent among total 99 samples of hog feces, 4.8 percent among 78 samples of manure heap, 0.6 percent among 164 samples of fingernail dirt, 5.3 percent among 57 samples of drinking water, 15.8 percent among 19 samples of sewage, 7.1 percent among 42 samples of dishtowel, 0.1 percent among 780 samples of house fly and 5.0 percent among 40 samples of house rat.

9.Direct Measurement of Low-Density Lipoprotein Cholesterol with Immunoseparation Method.

BACKGROUND: Most of clinical laboratories currently estimate low-density lipoprotein cholesterol (LDL-C) using the Friedewald equation (FLDL-C) , which requires fasting specimens and is inaccurate with increasing triglyceride (TG) levels. The author evaluated a new assay which directly measures LDL-C (DLDL-C) using the direct LDL immunoseparation reagent and subsequent measurement of cholesterol by conventional method. METHODS: Direct LDL-cholesterol assay (Sigma Diagnostics, St. Louis, MO) was analyzed in 110 fresh serum samples from fasting patients for physical examination at Ewha Womans University Tongdaemun Hospital. In FLDL-C, total cholesterol and TG were measured by enzymatic methods and HDL-C by direct method. Lp (a) , ape-A and apo-B were measured by nephelometry (Array 360, Bookman, USA). The paired t-test, Pearson product-moment correlation coefficients and linear regression were calculated by Microsoft Excel. Within-run precision and between-run precision were determined by two level reagent control sera containing normal and high concentration. RESULTS: Precision studies were provided within and between-run CVs in the range of 2.9-5.8% and 6.5-12.1%, respectively. On the comparison of the bias between FLDL-C and DLDL-C, there was no significant difference between the two methods in 0.37-6.48 g/L 7G and the same in less than 4.0 and 2.0 g/L TG. But the results of DLDL-C were higher than FLDL-C (P(0.05) in 2.0-4.0 g/L TG. The correlation coefficient between the two was 0.7637 and Y(DLDL-C)= 0.87X(FLDL-C) + 0.16 at a TG range of 0.37-6.48 g/L. The concordance of low-density lipoprotein classification for DLDL-C compared to FLDL-G at the NCEP cut-point was 91.2% below 1.30 g/L, 73.3% at 1.30-1.59 g/L and 100% above 1.60 g/L FLDL-C. The LDL-C was highly correlated with total cholesterol and ape-B concentrations. CONCLUSIONS: There was a high positive correlation between DLDL-C and FLDL-C below TG 4.0 g/L and direct determination is necessary above 2.0 g/L of TG and will require proper calibrators in measuring the DLDL-C.

10.A case of contact dermatitis due to oak moss.

Oak moss is a lichen extract obtained from Evernia prunastri and Pseudovernia furfuracea. It is the potent,ial allergen in contact dermatitis to perfumes. Sensitivity to oak moss has been atributed mainly to atranorin, evernic acid, usnic acid and fumarprotocetraric acid. We report a 44-year-old female patient presenting with features of contact dermatitis due to oak moss in perfumes. Patch test results revealed positive reactions to fragrance mix, oak moss, atranorin and evernic acid.
