1.Effect of Oxygen on the Antidotal Action of Thiosulfate in Cyanide Poisoning.

Cyanide poisoning is expected to be antagonized by the administration of oxygen, when it is administered in combination with the conventional cyanide antidote, sodium,thiosulfate. However, the antidotal efficacy and its exact mechanism of oxygen in cyanide poisoning is still a controversial one. To test the effect of oxygen on the antidotal action of thiosulfate ,in cyanide poisoning, author designed this study on the dose-mortality patterns for potassium cyanide in mice. Potency ratios derived from LDso values were compared in groups of mice treated with sodium thiosulfate alone and sodium thiosulfate with oxygen. These results indicated that oxygen enhances the anti-dotal effect of sodium thiosulfate, effectively. This fact demonstrates that oxygen is of importance in the treatment of cyanide poisoning.

2.The effects of compound madecassol on the wound healing.

This study was designed to assess the effects of compound madecassol(madecassic acid with neomycin sulfate and hydrocortisone acetate) on the wound healing. Madecassol is a titrated extract of centella asiatica and the clinical effects of madecassol are stated to the enhancement of wound healing and the prevention and relief of excessive scar formation. Compound madecassol is composed of madecassol and neomycin sulfate and hydrocortisone acetate. Neomycin sulfate has antibiotic effect and hydrocortisone acetate has antiinflammatory and antiallergic effects. So, compound madecassol has a synergistic effect of madecassol as well as neomycin sulfate and hydrocortisone acetate. Using 54 rats, we compared the effect of compound madecassol on wound healing at 3rd, 7th, 14th, and 21th postoperative days. This study examined the histologic findings and the gross findings which were wound size, epithelization and quality of granulation tissue. In this study, compound madecassol showed lower degree of inflammatory infiltration, shorter inflammatory phases and less wound contraction. The number of the myofibroblast in the group of compound madecassol were fewer than other groups. Granulation tissue of the compound madecassol was relatively healthier than others. There were no significant difference of re-epithelization between compound madecassol and other groups. In conclusion, compound madecassol can reduce excessive wound contraction and promotes wound healing process.

4.A Clinical Observation on 112 Cases of Hypospadias.

A clinical observation was made on 112 cases of hypospadias which were treated at the Department of Urology, Seoul National University Hospital during the period June 1977 through May 1982. The results were as follows: 1. The ages were distributed from 7 months to 31 years and most of the cases (64.3%) was treated in the age group of less than 7 years. 2. Most of the types of hypospadias was penoscrotal type (43.8%). 3. Cryptorchism was the most frequent associated anomalies with 21 cases (18.8%). 4. The complication rate of other anomalies was greatest in the perineal type (81.8%). 5. Success rate of one-stage urethroplasty was 52.9%. 6. In two-stage operation, the first stage operation of 97 cases was successful except one of incomplete chordee release and the success rate of Thiersch-Duplay and Johanson second stage urethroplasty was 44.4% and 66.7% respectively. 7. As the urethral stent was used in the urethroplasty, the success rate of use of silastic tube was better than red rubber and feeding tube.

10.National Guidelines for Breast Cancer Screening.

Breast cancer is the school most common cancer in Korea women. The incidence of breast cancer is around 25 per 100,000 women, and more than 5,500 women are diagnosed as breast cancer annually. Epidemics show that the incidence and mortality of breast cancer are increasing due to rapid changes of women's life style and westernized food and so on. The risk factors for breast cancer include hormone-related factors (early menarche, late menopause, no or late birth, hormone replacement therapy) and genetic background. It is hard to change risk factors for breast cancer but early detection strategies are best for decreasing the mortality rate from breast cancer. Breast self examination, clinical breast examination, and mammography can be used for screening of breast cancer. Breast self examination is not sensitive enough to detect a small cancer but monthly exam makes women be awakening about breast cancer and feel changes of her breast. Meta-analysis shows annual mammography reduces breast cancer mortality around 35% in the ages over 50. Some randomized clinical trials also showed mortality reduction by mammography in the ages of 40s. Surprisingly, breast cancer is rapidly increasing in late thirties with a peak incidence in forties among Korean women. Whether the early peak makes mammography less accurate is controversial. However, data from qualified screening centers showed mammography has same sensitivity with that in westerns. Korean Breast Cancer Society and National Cancer Center organized the 1st Consensus meeting for the national guidelines for breast cancer screening on July 24, 2001. We recommend that women should have monthly self examination beginning at age 30, should receive biannual clinical breast exam from healthcare providers from age 35, and should receive clinical breast exam and mammography at 1~2-year intervals after age 40.
