3.A Study for Safe Storage Time and Cost Analysis for Sterilized Products.

BACKGROUND: The shelf life should be determined by health care facility's infection control program. The current standards for preserving the sterility can be changing. So, to measure and compare the recontamination time of sterile packs among wrappers and storage conditions, and develop a practical standard for the shelf life and storage conditions of sterile products. The purpose of this study was placed on measurement of the recontamination time in packs wrapped in all kinds of wrapping materials used in Il-San hospital and extra-costs. METHODS: The first phase study was from June to November 2002 and tile second phase study was planned to extend further the shelf life of the pouch bag from December 2002 to June 2003. Total 504 sterile sample packs containing cotton ball (5 cm-length) prepared and wrapped in four wrapping materials (randomized reprocessed two folds of lines, a fold of new craft paper, a fold of new nonwoven fabric, a fold of new pouch bag) and stored in three location (open shell of one of typical wards, open shelf of one of ICUs, open shelf of CSSD). Each type of packs were opened weekly and the cotton balls inoculated into thioglycollate broth at 35degrees C for seven days by infection control nurse. The temperature and relative humidity was monitored monthly. RESULTS: There was on growth found for any types of the pack and storage conditions studied. The temperature and relative humidity of storage locations were 24.8degrees C and 35.5 %. The extra-cost of reprocessing in Ethylene-Oxide gas sterilization was \32,800,000 and Steam sterilization was \36,900,000 by year. CONCLUSION: This study was conducted in attempt to identify the possibility of extending the current shelf life for sterile goods we use in a sense of cost effectiveness. It was possible to extend shelf life from four weeks to six months in the study hospital. We are going to continue this study and take all steps need to apply this new concept to our practice, and then trying to extend to other hospitals.

4.Relationship between Radiation Induced Activation of DNA Repair Genes and Radiation Induced Apoptosis in Human Cell Line A431.

PURPOSE: The purpose of this study was to evaluate the relationship between radiation-induced activation of DNA repair genes and radiation induced apoptosis in A431 cell line. MATERALS AND METHODS: Five and 25 Gys of gamma radiation were given to A431 cells by a Cs-137 cell irradiator. Apoptosis was evaluated by flow cytometry using annexin V-fluorescein isothiocyanate and propidium iodide staining. The expression of DNA repair genes was evaluated by both Northern and Western blot analyses. RESULTS: The number of apoptotic cells increased with the increased radiation dose. It increased most significantly at 12 hours after irradiation. Expression of p53, p21, and hRAD50 reached the highest level at 12 hours after 5 Gy irradiation. In response to 25 Gy irradiation, hRAD50 and p21 were expressed maximally at 12 hours, but p53 and GADD45 genes showed the highest expression level after 12 hours. CONCLUSION: Induction of apoptosis and DNA repair by ionizing radiation were closely correlated. The peak time of inducing apoptosis and DNA repair was 12 hours in this study model. hRAD50, a recently discovered DNA repair gene, was also associated with radiation-induced apoptosis.

5.A Clinical Observation on Meningitis in Infancy and Chilhdood.

A clinical observation and comparision was carried out on 466 cases of meningitis in infancy and childhood : 135 cases of purulent meningitis ; 208 cases of aseptic meningitis, and 123 cases of tuberculous meningitis. They were admitted to Jeonju Presbyterian Medical Center from January, 1973 to December, 1979. The results were as follows; 1) Male to female ratio was 2.1:1 in purulent meningitis, 3.2:1 in aseptic meningitis, and 2.4:1 in tuberculous meningitis. 2) The seasonal peak incidence was Spring and Summer for purulent and tuberculous menigitis and Summer for aseptic meningitis. 3) The most predilectional age which resulted in meningitis was infancy in purulent meningitis, 4 to 12 years o fage in aseptic meningitis and 1 to 4 years of age in tuberculous meningitis. 4) Fever was the most frequent symptom ; and vomiting, convulsion, unconsciousness, and headache followed in that order in the three kinds of meningitis. The most frequent neurologic finding was stiff neck; and Kernig's sign, Brudzinski's sign, and Babinski's sign followed in that order in the three kinds of meningitis. 5) In CSF examination on admission, cell counts were most frequently under 1,000/mm3 in purulent meningitis(49.6%), under 50/mm3 in aseptic menigitis (35.6%) and 100 to 300/mm3 in tuberculous meningitis (52.0%). The sugar level in 74.1% of purulent meningitis and in 84.5% of tuberculous meningitis was under 50mg/dl. In aseptic meningitis the sugar level was over 50mg/dl in 72.6%. The protein level was most frequently 30 to 150mg/dl in all three kinds of meningitis. 6) The mortality rate was 14.1% in purulent meningitis, 0.5% in asepic meningitis and 10.6% in tuberculous meningits. 7) The younger the patient in purulent and tuberculous meningitis, the worse the prognosis. The mortality rate of tuberculous meningitis was 9.1% in stage II and 50% in stage III.

6.Studies on the Radiation Induced Apoptosis by Morphological and Biochemical Analysis in A431 Cells.

PURPOSE: We performed this study to evaluate the process of radiation induced apoptosis in A431 skin epithelial cancer cell line. MATERIALS AND METHODS: Low to high dose radiation (0, 2, 5, 10, 25 Gy) was given to A431 cells by Cs-137 cell irradiator. Apoptosis was evaluated by cell morphology, dye exclusion test, and DNA laddering. RESULTS: Cell viability decreased as the radiation dose increased. Number of apoptotic bodies increased as radiation dose increased. It increased most significantly at 12 hours after irradiation. Lactate dehydrogenase activity in culture medium increased according to radiation dose and time after irradiation. CONCLUSION:: Radiation-induced apoptosis which was the main course of cell death in A431 cells could be analyzed quantitatively by counting apoptotic bodies under microscope. Apoptosis increased as radiation dose increased.

8.Evaluation of The Neck Mass.

As public awareness of the various warning signs of malignancy increases, so does the concern evoked by the self identified finding of mass in the head and neck area. Not all the palpable masses are always significantly abnormal, but any nontender mass especially to the adult is significant enough to warrant further full investigation and follow up, the object of which should be to determine the possibility of malignancy and urgency of treatment. Approach to the diagnosis of the neck mass is so important in that it affects decision regarding further evaluation would lead to the determination of the most efficacious mode of therapy, eventually to the good prognosis. So, it should be emphasized that approach to the diagnosis of neck mass should be planned, systematic and thorough, this begins with the taking careful history following performance of complete examination of the head and neck especially to the nasopharynx, tongue base, pyriform sinus, palatine tonsil and larynx. Then a number of laboratory and radiologic studies are available, following triple endoscopy under general anesthesia and blind biopsy if needed. The most important rule to keep is that any biopsy procedures should be delayed to the last modality of effort to the diagnosis and if it should be done, under the plan of radical neck dissection.

9.Syringocystadenoma Papilliferum Giganteum: Report of A Case.

Syringocystadenoma papilliferum is s rare tumor of disputed origin that was first reorted by peterson in 1892. Theclinical features of syringocystadenoma papilliferum are not constant. It may be present at birth or develop later in life. Its appearance varies from an irregular, flat, grey or reddish area to a grey or dark brown. raised and cauliflower-like surface. Sometimes it is moist, flesh or pedunculated and occasionally a verrucous plaque, or cyst may be seen. The typical lesion occurs most frequently on the scalp, and is a hairless smooth plaque until puberty, when a nodular verrucous transformation occurs. A 39 years old male patient had egg sized (3*5cm) and yellowish red colored verrucous tumor mass on his epigastric area, which had grown up slowly for 7 years. The diagnosis of syringocysyadenoma papilliferum was confirmed by histological examination. We repoet a case of syringocystadenoma papilliferum in unusually large size on the epigastric area.

10.Ultrastructural In Situ Hybridization Method for Detection of Oncogenic Viruses.

In situ hybridization (ISH) is a standard method for localizing DNA or RNA sequences in tissue or cell preperation. The technique was developed at the electron-microscopic level, and enables the precise subcellular localization. A method was developed for detection of specific viral DNA. We have tested various methods and technique to detect specific viral DNA through ISH at the electron microscopic level. Postembedding method of ultrastructural ISH was developed and successfully applied for the detection of human papillomavirus type 16 in squamous cell carcinoma of the uterine cervix and Epstein-Barr virus in EBV-infected leukemia cell line. The following results are made. The best results were obtained using 0.2% glutaraldehyde and 4% paraformaldehyde fixed tissue or cell block. The labelling was best observed on Unicryl resin and Lowicryl K4M resin sections. Epon sections showed no reactivity. Thin sections of Unicryl resin were more easier than Lowicryl K4M resin. Enzymatic predigestion with proteinase K, pepsin and trypsind gave good results. However, high concentration of these produce poor results due to excessive destruction of the cellular components. Alkali treatment with 0.5N sodium hydroxide produced successful results in denaturation of target DNA. The labelling density of gold particles was independent of incubation time or temperature in hybridization step. The viral DNA labelling was localized mainly within the nucleus, both within and at the edge of electron dense regions, and below the nuclear membrane. And the labelling was seen in the form of a dense, roughly spherical shape. In conclusion, the best results are obtained by the conditions that tissue fixed by 0.2% glutaraldehyde and 4% paraformaldehyde solution, embedded with Unicryl resin, protein denaturation by 0.1ul/ml proteinase K, DNA denaturation by 0.5N sodium hydroxide, and reaction with DNA probe.
