2.Trends of Ambulatory Leprosy Patientis Treated at the Chronic Disease Laboratory in the Past 10 Years ( 1965 - 1974 ).

This study is an epidemiological investigation of the trends of leprosy of the last 10 years by age, sex, place of birth, present address, type of disease, onset age and duration of disease hetween the onset and the treatment. The study was conducted on 766(550males, 216females) out-patients who visited the Chronic Disease Laboratory from January, l965 to December, 1974. The importance of leprosy control in the urban area has been discussed. The conclusions are as follows; 1. Average monthly registered numbers of new patients are 5. 2 in A group of patients and 7. 6 in B group of patient. The ratio of male to female is 2. 7 and 2.4 respectively. By age groups, both male and female, the highest age group is 25~34 years. This age group comprised 41. 3% of the 1965~1969 group (A group) of patients, and 34. 7% of the 1970~1974 group (B group) of patients. In the former group of patients, a significantly higher number of patient were under 35 years and in the latter group of patients, the patients of 35 years or more showed a significant increase. It can be concluded that rate of patients of 35 years of age or more is on the increase. 2. By geographical locations of birth, the number of patients from the southern part of korea where leprosy is widely prevalent was the largest. These comprised 59. 3% in the A group of patients and 16. 0% in the B group of patients, followed by 23. 9% and 23. 7% from the middle part of Korea for those periods. The northern part of Korea showed only 10. 6% and 11. 7% respectively. Meanwhile, by present addresses, the number of patients residing in the northern part was dominantly large, and 80.3% of the A group of patients and 79.8% of the B group of patients were found to be living in the vicinity of Seoul. 3. Among the patients from the northern part, the increase of patients of L-type and B-group was remarkable. From this point of view, leprosy control policy for the neighbouring areas of large cities ia urgent. 4. The onset age shows highest in the age braeket of 15-19years with 24.5% in the A group of patients, 20.0% in the B group of patients. This is folIowed by the age bracket of 20~21 years of age comprising 16.8% in the A group of patients, 18. 4% in the B group of patients. A Comparative study of the peroid from the onset of the disease to the beginning of the treatment, showed that patients of less than 5 years amounted 69. 3% in the A group of patients and 64. 0% in the B group of patients. Those of more than 10 years were 9. 4% in the A group of patients and 14. 7%, in the B group of patients. 5. The results of bacteriological examination showed a tendency to increase in the positive rate with 14.2% in the A group of patients and 28.7% in the B group of patients. This is attributable to the increase of patients of L-type and B-group.

3.Ichthyosis Hystrix Treated by Vitamin A acid Ointment Application.

Ichthyosis hystrix is a localized variant of Bullous Congenital Ichthyosiform Erythroderma by Broq, and also, localized form of Epidermolytic hyperkeratosis according to Frost and Van Scott's classification of ichthyosi-form dermatoses. Clinically it is generally considered to be widespread systematized epidermal nevus, and demonstrating an autosomal dominant pattem of inheritance, and characterized by verrucous scale at or shortly after birth with particular involve- ment of flexural areas. A case of 21 year old girl with ichthyosis hystrix was reported, The skin lesion showed multiple rice to pea sized linear verrucous papules on left forearm, neck, axilla and upper chest. The skin biopsy was done, the finding shows remarked laminated hyperkeratosis, acanthosis, papillomatosis, and vacuoliization on malpighian layer. Diagnosis was confirmed by clinicaI apperance and histopathologicaI finding. The method of treatment is 0. 05% Vitamin A acid ointment application and show marked improvement.

5.Asthma and Air Pollution in Korea.

With rapid industrialization and urbanization, environmental pollution has become an urgent health problem. Asthma attack is a common and severe disease, and can be a sentinel event of worsening air pollution. There are, however, few studies about the epidemiolgy of asthma and the association between asthma and low level air pollution in Korea. This study was conducted to describe the trend and pattern of asthma attack and to examine the effect of air pollutants on admission in Korea. Combined data of this study are medical insurance claims for asthma, data from telemetry system of the Ministry of Environment for air pollutants, and air temperatures from monthly weather reports of the Meteorological Administration. To describe the temporal trends of asthma in Korea, insurance claims since 1985 were analized. Age specific asthma visit/admissions per 1000 eligible persons, as well as ratio of asthma visit/admissions over all visit/admissions showed a steady increase in asthma attack in Korea. Asthma prevalence was relatively high among under 5 years and over 50 years of age, and in these age groups asthma was more prevalent among males. The effects of relatively low level air pollution and weather condition on the number of patients who had asthma attacks admitted to hospitals in Seoul area were studied from April 2 to April 24, 1993. Asthma admissions were examined throughout the study period (392 cases), except sunday and holiday(354 cases) and except sunday, holiday and saturday (293 cases), while grouped into three age categories (3 12, over 40, and all age). Following results were obtained through correlation and multiple regression analysis. The number of adimission were significantly correlated with ambient air concentrations of total suspended particles (TSP), sulfur dioxide (SO2) and carbon monoxide(CO). 1) The number of admissions on the same day were significantly correlated with ambient air maximal concentrations of total suspended particcles among persons who were above 40 year old. 2) The effects of maximal daily concentrations of sulfer dioxide(SO2) among persons who were of 3- 12 age were significant when one day lagged asthma admission was compared with the air pollution on the previous day. 3) The number of admissions on 1-day lag were significantly correlated with ambient air maximal daily concentrations of carbon monoxide(CO) among persons who were above 40 year old. The associations between temperatures and number of admissions were not significant in this study. Levels of pollutants were fairly low, the monthly mean being 0.022 ppm for SO2, 0.032 ppm for NO2, 3.2 ppm for CO, 0.015 ppm for 03, and 240 micro gram/m3 for TSP. The mean temperature was 10.6degrees C. These results suggest that concentration of air pollutants lower than those given as guidelines may increase the incidence of asthma attacks.

6.Osteoid Osteoma of the patella: a case report.

Osteoid osteoma is a distinctive benign lesion characterized by a less then 2 cm, pea-like mass of abnormal bone (nidus). The common sites of osteoid osteoma are femur, tibia, humerus, spine and talus. Mastoid tip, clavicle, acetabulum, coccyx, rib, hamate, scapula and scaphoid are rare sites of osteoid osteoma. Osteoid osteoma of the patella is also very rare and on review of literatures we could find only one reported case which was treated by surgical excision. We also experienced one case of osteoid osteoma located at the patella, so we report this case with review of literatures.

7.The Clinical Study on 28 Patients with the Pericardiac Effusion.

The clinical study was performed to 28 patients with the pericardiac effusion who were admitted in Busan university Hospital and Paik Hospital, In-Je Medical College, Busan, korea during March 1976 and July 1981. The results were as following; 1) 12 cases(42.9%) of total 28 pericardiac effusion occured in 3rd decade, 8(28.5%) in 2nd decade and 5 cases(17.9%) in 4th decade. Sex distribution showed male to female 2.5:1. 2) With etiological distribution there were 15 cases(53.6%) tuberculous, 5(17.9%) nonspecific, 3(10.7%) malignant, 2(7.1%) pyogenic, 2 traumatic and 1(3.6%) rheumatic. 3) On admission 28 cases(100%) complained of dyspnea, 24(85.7%) distant heart sound, 20(71.4%) each engorged jugular vein and edema, 18(64.3%) each cough and abdominal fullness, 17(60.7%) each sputum and chest discomfort and pain, 12(42.9%) fever, 11(39.1%) orthopnea, 10(35.7%) chilling, 7(25%) oliguria, 4(14.6%) palpitation and 3(17.2%) headache. Increased cardiac dullness was observed in 26 cases(71.4%), hepatomegaly 18(64.3%), pulmonary rales 17(60.7%), ascites 13(46.1%), fever 12(42.9%), pericardiac friction rub 10(35.7%), splenomegaly 7(25%), paradoxical pulse 6(21.4%) and jaundice 2(7.1%). 4) On amission SGOT and SGPT level was increased each in 15(53.6%) and 13(46.7%), TTT and CCF abnormal in 10(35.7%), and 8(28.5%), serum NPN, BUN, creatinine increased in each 10(35.7%), 11(39.1%), 8(28.5%), WBC increased in 6(21.4%), RBC decreased in 8(28.5%) and ESR increased in 16(57.1%). 5) Characteristics of pericardiac effusion showed hemorrhagic 17(60.7%), serosangeous 6(21.4%), sangeous 3(17.2%) and pyogenic 2 cases(7.1%). 6) The ECG on admission revealed low voltage in 25(89.1%), depressed ST segment in 13(46.1%) and inverted T wave in 12(42.9%). 7) Chest X-ray showing cardiomegaly 28(100%), pleural effusion in 20(71.4%), and pulmonary congestion in 16(57.1%). 8) The m mode and 2-D Echocardiogram showed echo free space in 28 cases and after pericardiocentesis the space was reduced or disappeared. 9) 19 cases of 28(67.5%) were improved clinically, while 5 cases(17.9%) were not improved and 4 cases(14.6%) died.

8.Differential diagnosis of pelvic masses by gray-scale sonography

Ultrasongraphy is a safe, noninvasive examination which should be the first procedure in the workup of apatient with a definite or suspected pelvic masses. The diagnostic schemes were derived from correlating the sonographic features with histomorphology in 230 surgically proven pelvic masses. Besides separating pelvic masses into the conventional categories of cystic, complex and solid, grayscale sonographic features of a pelvic mass canbe used to subcategorized these masses into a more useful differential diagnosis. The results are as follows; 1.The most characteristic finding of uterine myoma was midly to mederately echogenic uterine enlargement (90.7%)with a lobulated uterine margin (62.8%), and often less echogenic than the normal uterine echoes. 2. The typical ultrasonographic finding of H-mole was uterine enlargement with multiple small vesicular patterns of intrauterincontents(93%). 3. The most frequent finding of cystic teratoma was cystic mass with echogenic foci(48%), but the echogenic appearance of the lesions was extremely variable. 4. The ultrasonographic findings of ectopic pregnancywere adnexal mass(comlex or cystic), decidual proliferation of the uterus, enlargement of uterine size, fluid incul-de-sac, deveiation of uterus by adnexal mass, and psuedointrauterine appearance. 5. Ultrasound provided information leading to the correct diagnosis in 57% of cases, contributory data in 21.3%, and non-specific information in 10.4%. Errors occured in 3.9% and false-negative in 7.4%.

10.Ultrastructural In Situ Hybridization Method for Detection of Oncogenic Viruses.

In situ hybridization (ISH) is a standard method for localizing DNA or RNA sequences in tissue or cell preperation. The technique was developed at the electron-microscopic level, and enables the precise subcellular localization. A method was developed for detection of specific viral DNA. We have tested various methods and technique to detect specific viral DNA through ISH at the electron microscopic level. Postembedding method of ultrastructural ISH was developed and successfully applied for the detection of human papillomavirus type 16 in squamous cell carcinoma of the uterine cervix and Epstein-Barr virus in EBV-infected leukemia cell line. The following results are made. The best results were obtained using 0.2% glutaraldehyde and 4% paraformaldehyde fixed tissue or cell block. The labelling was best observed on Unicryl resin and Lowicryl K4M resin sections. Epon sections showed no reactivity. Thin sections of Unicryl resin were more easier than Lowicryl K4M resin. Enzymatic predigestion with proteinase K, pepsin and trypsind gave good results. However, high concentration of these produce poor results due to excessive destruction of the cellular components. Alkali treatment with 0.5N sodium hydroxide produced successful results in denaturation of target DNA. The labelling density of gold particles was independent of incubation time or temperature in hybridization step. The viral DNA labelling was localized mainly within the nucleus, both within and at the edge of electron dense regions, and below the nuclear membrane. And the labelling was seen in the form of a dense, roughly spherical shape. In conclusion, the best results are obtained by the conditions that tissue fixed by 0.2% glutaraldehyde and 4% paraformaldehyde solution, embedded with Unicryl resin, protein denaturation by 0.1ul/ml proteinase K, DNA denaturation by 0.5N sodium hydroxide, and reaction with DNA probe.
