A case of solitary glomuys tumor affection 43 year-old woman, who has been suffered from bluish purple colored, smail, tener nodule on the left scapular area, was presented. Surgical excision of lesion was enough for relieving the whole subjective symptoms.

With recent progres ion in knowledge of immunology, the roles of eosinophils and basophils in immediate and delaye3 hypersnsitivity reatioons have been partly elucidated and generally they are known as marker cells in various allergic disorders. Changes in number of eosinophils and basophils in the infiltrates of the involved tissue or in circulating blood. in various allergic disordcrs have been reported by several investigators, however, there are only a fever ports about skin disorders, especially for basophils because staining and conting method of the basophils devcloped only recently by James and Moore. The present study has been performed to detect the alteration in absolute number of circulating eosinophils and basophils and to find out any correation between the changes of hoth cell numbers in various allergic dermatoses. A total of 84 pati,.nts with various allergic dermatoses and 50 healthy adults as control group were selected for this study at National Medical Center during the period of July to September 1976. The method of staining and counting of circulating eosinophils and basophils was essentially same with the originaI method of Randolph for eosinophils and James and Moore for basophils with only a minor modifications of both. All the blood specimens were taken between 9:00 and l2:00 a.m. and all the experiments were done within 6 hours after sampling. The results were as follows. 1. The mean value+1 S.D. for eosinophil, basophil and total leukocyte number in 50 normal control was 126.5+80.4, 31. 413.6, and 5,928.3+1,536.9, respectively. 2. In 20 cases of acute urticaria (duration of illness, less than 2 days), the mean value+1 S.D. for eosinophil, basophil, and total leukocyte number was 107.4+95.4, 9.3+7.3 and 9,620+3,240.5, respectively and the result showed a significant decrease of basophils(p<0.001). In 20 cases af chronic urticaria (duration of illness, more than 4 weeks and dermographism included), the mean value 1 S.D. for eosinophil, basophil and total leukocyte number was 237.3+202.9, 31.1+12.3 and 7,748.5+1,989,7, respect!vely and the result showed a significant increase of eosinophils(p<0.01). 4. In 15 cases of atopic dermatitis, the mean value+1 S.D. for eosinophil, basophil and total leukocyte number was 309.7+402.8, 28.8+16.5 and 7,694.5+3,221.9, respectively and the result showed a significant increase of eosinophils (p<0.01). 5. !n 10 cases of acute allergic contact dermatitis, the mean value+1 S.D. for eosinophil, basophil and total leukocyte number was 381.5+269.5, 53.6+34.0 and 7,979.4+1,126.0 respectively and the result showed a significant increase of eosinophils and basophils, respectively(p<0.001, p<0.0.1) 6. In 5 ceases of allergic drug eruption, the mcan value 1 S. D. for eosinophil, basophil and total leukocyte number was 520.0+367.5, 19.6+17.6 and 12,390+2,783.3, respectively and the result showed a significant in.rease of eosinophils (p<0. 001) and decrease of basophils. 7. In 10 cases of scabies, the mean value+1 S.D. for eosinophil, basophil and totai leukocyte number was 299.3+216.6, 30.4+11.5 and 8,081.6+3,304.4, respectively and the result showed a significant increase of eosinophils (p<0.01). 8. In 3 cases of tinea pedis with id eruption, the mean value+1 S.D. for eosinophil, basophil and total leukocyte number was 288.9+79.7, 29.6+5.2 and 8,916.7+2,739.2, respectively and the result shovred slight increase of eosinophils. 9. In order to observe the sequential changes in the number of circulating eosinophils and basophils, a patient with wart was sensitized and challenged with DNCB, and serial determinations were performed. During sensitization period the number of circulating eosinophils was not changed, but the basophils increased gradually, After spontaneous flare-up the number of eosinophils increased with the decrease toward normal of basophils. In the present study, we confirmed that the circulating eosinophils and basophils are definitely related to allergic dermatoses and found out the possibility that the difference in absolute number might be related to the causal agents and duration of disease.

The mite fauna of the house-dust, collected from 39 houses of Seoul, were examined and the results are as follows. 1) Mites were present in 29 samples (74.3%) and the number of mites collected was 993 in total. 2) The families of the mites distinguished were Pyroglyphidae, Acaridae, Cheyletidae, Phytoseiidae and Smariidae, and the identified species were Dermatophagoides farinae, Deromatophagoides pteroassinus, Tyrophagus purtrecentiae, Landoglyphus konoi and Cheyletus malacensis. 3) D. farinae (53.6%) showed higher frequency rate of occurance than any other mites; C. malacensis (12.8%), T. putrecentiae (7.7%), D. pteronyssinus (5.0%) and I. konoi (2.7%).

A case of progressive lipodystrophy affecting 19-year-old female, is reported. Two years ago, she accidentally discovered brownish pigmented, and depressed lesion on the periumbilical area. For one year after that, lipodystrophic process spread to the left lateral abdomen and chest wall in linear fashion and ceased spontaneously. Physically, she had no abnormalities on general conitions and fat distribution on whole body was also normal except the above mentioned area. Laboratory findings are with normal limits except incread ESR. Histopathologic findings showed abscence of subcutaneous adipose tissue.

Neurilemmoma is usually a solitary nerve sheath tumor, most often seen in adults. Tl: neurilemmomas of the oral cavity have been reported to develop on the tongue, buccal m7; Cosa, palate, gingiva, floor of the mouth, and lower lip, in order of frequency. We describe 7-year-old boy witlf an asymptomatic, solitary neurilemmoma on his upper lip with a brit: review of the literature. Neurilemmoma should be considered as one of the tumors that ca develop on children's lips.

STUDY DESIGN: A case report is presented of destructive lesion involuting intervertebral disc space and adjoing vertebral bodies in association with ankylosing spondylitis. OBJECTIVE: To report unusual clinical and radiological presentation of spondylodiscitis and its treatment wi th anterior interbody fusion. SUMMARY OF BACKGROUND DATA: Althought there is some mention in the literature of pathogenesis, diagnosis and treatment of spondylodiscitis in ankylosing spondylitis, we think that its occurrence is frequent enough to warrant this condition and we should differentiate it from other infectious or tumorous condition. RESULTS: Anterior debridement and anterior interbody fusion with structural bone graft were performed. The microscopic study showed nonspecific chronic inflammation and extensive necrosis. CONCLUSION: It was suggested that primary spondylodiscitis or pseudoarthrosis resulted from mechanical trauma be related to this condition rather than infectious spondylitis.

Itraconazole is the first orally active triazole antifungal agent. Authors performed multicenter trials to evaluate the therapeutic effect of itraconazole in dermatophytoses. During 6 months from September, 1988 to February, 1989, 246 patients with dermatophytoses were included in this study. 74 of these cases lost to follow-up and 3 cases developed drug eruption were excluded from analysis of therapeutic efficacy. Duration of therapy were 4 weeks in tinea pedis and tinea manus, and 2 weeks in tinea cruris and tinea corporis. The obtained results were as follows : 1. The everall cure rate of itraconazole in dermatophytoses were 81.3%, and the cure rate in each dermatophytosis were 80.3% in tinea pedis, 100.0% in tines manus, 85.3% in tinea cruris and 68.6% in tinea corporis. 2. The cure rate at two weeks after completion of the therapy(81.3%) was higher than that at the end of the therapy(74.3%). 3. Trichophyton rubrum(70.0%), Trichophyton mentagrophytes(81.3%), Microsporum canis(7.3%) and Epidermophyton floccosum(0.9%) were isolated from 110 cases among the enrolled patients. 4. Seven of the 190 patients experienced adverse effects of the treatment: abdominal discomfort(one); nusea(one); constipation(two); and drug eruption(three). Therefore we impressed that itraconazole is an effective and safe new oral therapeutic: agent for dermatophytoses.

Trichomonas tenax(T. tenax) and Trichomonas hominis (T. hominis) were collected, cultured and sampled for comparative microscopical studies using electron microscope. Both flagellates were oval in shape and surrounded by a distinct outer membrane. Five recurrent flagella and one anterior flagellum had, each, 9 paris of peripheral and 1 pair of central fibrils, Undulating membrane was curved over the recurrent flagella, and bended in the middle at right angles with cell surface. Cytostome, engulfing bacteria, was observed in T. hominis. In the cytoplasm, there were fine dense glycogen particles, and vacuoles containing ingested materials. Dense pigment rods were also observed in both flagellates, but the rods were not distributed around the vacuoles in T. hominis. In T. tenax axostyle appeared as a cup-shaped structure comprising a single row of 41 fibrils, each about 120 a in diameter. It enclosed glycogen particles, and the open side was faced to the nucleus. Endoplasmic reticulum was observed around the nucleus, but it was less developed in T. hominis. Nucleus was ovoid having double nuclear membrane, which was clearly defined in T. hominis. Blepharoplast, parabasal body, Golgi appartus and mitochondrion was not observed in both flagellates.

The detection of renal disorders now involves a series of complicated diagnostic procedure such as radiologic pyelography, function studies in laboratory and others These add up to a painful, expensive, and time consuming experience for the patient, others may be spared the discomfort of unnecessary. Consequently, the development of methods for preliminary screening of patients is significant both with regard to the diagnostic information obtained and to the selection of those patients requiring complete urologic work up. In particular, a reliable screening test is important in establishing or ruling out renal pathology as an underlying factor in hypertension the finding of a disorder of one kidney in the hypertensive patient may be at indication for nephrectomy or other surgical measures of renal artery. A positive preliminary investigation warrants precise diagnosis by aortography and confirmation by individual renal clearance tests. Finally the primary objective of the screening test is the restriction of more exacting procedures such as aortography with its inherent risk of serious complications to the preoperative evaluation of a demonstrated lesions. The visualization of an internal structure by determining the spatial distribution of a gamma radiation-emitting isotope was started in 1950 when Cassen and his associates utilized newly developed scintillation counters to print mechanically the spatial distribution of radioactive iodine in the thyroid gland. Visual and functional informations referable to each kidney separately may be obtained by radioisotopes and tracer or by scanning techniques rapidly and without trauma. The renogram or tracing of the renal uptake and excretion of a labeled contrast medium is a clinically useful measure of renal vascularity, tubular cell function, and the patency of the upper urinary passage, and especially valuable as a screening test for the hypertensive patient with suspected unilateral disease of the kidney. Similarly, scanning images are achieving an important place in medical diagnosis of irregularities effecting the kidneys and other specific organs. Radioactive contrast media, such as I131 labeled diodrast, miokon, Urokon, or hippuran have been widely used for renal function studies. They are entirety unsatisfactory, however, for renal scintillation scanning because they are rapidly excreted by the kidneys without retention in the renal parenchyma in contrast but mercurial diuretics labeled with radioactive mercury are concentrated to a high degree in the renal tubular cells, chiefly in the cortex, prior to their appearance in the urine (Borghgraef 1956.MaAfee and Wagner 1960). Following the report of these letter writers, scintillation scanning of the kidney after the intravenous injection of Hg Neohydrin have been widely used in urological clinics for preliminary screening method of renal disease such as delineating the parenchyma of the kidney and space-occupying lesions by Krevitz, Berk, and others(Reley, Blahd, Mins, Morgan, Kuhl, Bender, Simmons, Blau, Kim) and remained several problems which is anticipated to improvements in resolution for further using of renal scan. The main purposes of this experimental study were: 1) to learn at which period of time it is best to visualize the renal parenchyma following the intravenous administration of Hg Neohydrin, 2) to learn the minimal detectable size of space-occupying lesion within the kidney phantom, and 3) to compare the renal scan with aortography and pyelography on various period of hydronephrosis for visualization of renal parenchyma, 4) to compare the renal scan with aortography on various degree of narrowing of the renal artery for visualization of renal parenchyma. MATERIALS AND METHODS: These experiments have been performed on 17 normal adult mongrel dogs anesthetized with intravenous injection of sodium amycal. Ofthese 3 dogs were used for determining the optimal time of visualization of the renal parenchyma. 8 dogs for the visualization of the parenchyma of hydronephrotic kidneys as created by partial ligation of the left ureter for the period of 1. 2. 3 and 4 weeks as devided in 4 groups, and 6 dogs were used for the visualization of the narrowed renal artery produced by partial obstruction to various degrees. A wax phantom of the kidney was utilized for the study of detectable minimal size of the intrarenal space occupying lesion, the phantom measures 12.5 X 6.2 X 3.5cm. All animals underwent renal scintiacan received 80 microcuri of Hg neohydrin intravenously, they were placed in a prone position and the study was carried out using a Nuclear-Chicago dual scanner with a 62-hole coarse focusing collimator containing a 3x3 inch sodium iodide crystal, the collimator was passed back and forth over the target are at a rate of 12-15cm, per minute. The scan usually required 30 minutes for complete of scan. The pyelography was performed in retrograde way with 30% diodrast injected through ureteral catheter and the amount of dye injected was adjusted to replace the urine from renal pelvis. The aortography was performed by transfemoral arterial catheterization technique as described by Seldinge. RESULTS: 1. The optimal opacification period of renal parenchyma after injection of Hg 203 neohydrin was found to range from 1 to 6 hours 2. By kidney phantom study it was demonstrated that the space-occupying lesion measuring 2cm. in diameter can not be detected on scan. While the lesion measuring 3cm, or more can be successfully visualized. 3. The effect of partial obstruction of the ureter upon the renal scintiscan for visualization of renal parenchyma was not. dependable, the renal scintiscan continued to appear up to at least the 2 weeks after partial obstruction of ureter whereas the retrogredepyelography and aortography showed the definite picture of hydronephrosis to various degree. 4. The renal scan is inconsistent in many instances of renal artery stenosis, the scan showed a diminished concentration of mercury in the kidney with stenotic main renal artery and complete failure of visualization of the renal substance in cases with completely obstructed renal artery. 5. It is possible to demonstrate the value and usefulness of renal scan of Hg neohydrin as a routine preliminary screening test for renal pathology, the alteration of the concentration of the mercury can might be expected to give additional information.
