2.A Development of the Social Network Model for the Maternal Role of First-time Mother.

PURPOSE: The purpose of this study was to evaluate the factors which are related to the maternal role performance of first-time mother to improve the health of infant. Specifically a basic hypothetical model was developed based on the previous study about a model of social networks. METHOD: The survey was done from January to February in 2001. Total 257 mothers who have four to twelve month old first-time baby was interviewed in five community health center around country(Seoul, Choung-ju, Asan, Cheon-an, Jeju). Finally 247 data was analyzed. Data analysis was done with LISREL 8.20 program for covariance structural analysis. RESULTS: Compared to the hypothetical model, the revised model has become parsimonious and had a better fit to the data(X(2)=167.55(p=.00), X(2)/df=1.48, GFI=0.97, AGFI=0.95, RMR=0.049, NFI=0.98, NNFI=0.99, CN= 222.53). All predictive variables of the maternal role of first-time mother explained 30% of total variance in model. Social network structural characteristics and social network interactional characteristics had significant effect on the emotional support and the information support. And social network interactional characteristics had significant effect on the service support, material support and social companionship support. The service support and social companionship support had significant effect on the maternal role strain. The emotional support and the social companionship support had significant effect on the maternal role of first-time mother. CONCLUSION: As the conclusion of this study, there is in need of the developing the programmes focussed on the social network for the first-time mother.

3.A Case of Cutaneous Larva Migrans.

A 58-year-old male presented with a pruritic, erythematous hread-like eruption on the right chest, which appeared as a small papule since 6 months prior to his visit. Histopathologic examination showed a burrow containing numerous eosinophiles and a few mononuclear cells in the epidermis. There was focal spongiosis unde the burrow. In the dermis, there was a diffuse and occasionally dense perivascular and periadnx 1 inflammatory cell infiltration consisting of eosinophiles and some lymphocytes. Multiple histologic sections were made without success in deizo strating parasitic tissue in the epidermis, however, the clinical and histopathologic features we onsistent with cutaneous larva migrans.

5.Radiological Study of Spondylolysis in Korean Laborer

The spondylolysis is a defective ossification in the pars interarticularis of the neural arches without displacement of the vertebral body. The authors analysed 810 lumbar spines of adult Korean laborers on the basis examined to determine the anatomic characteristics of spondylolysis. The following results were obtained. 1. The overall incidence in laborer was 6.8%. 2. The most frequent site of the involvement was the fifth lumbar vertebra. of radiologic study and 3. Spina bifida in spodylolysis was approximately 1.7 times as frequent as is normally expected. 4. Lumbarization in spondylolysis was approximately 5.8 times as frequent as is normally expected. 5. Sacralization was not found in 55 cases of spondylolysis.

7.The Operative Treatment of Patellar Fracture Using Indirect Reduction.

The goal of treatment of fracture of the patella is to gain accurate fracture reduction, secure fracture fixation, and early range of motion. In severely comminuted fractures of patella, open reduction requires extensive soft tissue dissection and high risk of infection. Furthermore, it is very difficult to get rigid fixation enough to allow early joint motion. An alternative in the management of severely comminuted patellar fractures is the technique of indirect reduction, in which the soft-tissue attachments of the individual fragments are preserved and used to reduce the fracture. Thirteen patients with fracture of the patella were treated by indirect method. Clinical analysis was done by using anterior knee pain score by Shelboume, Lysholm score, and radiological finding. The average Lysholm score was 88.3 and average anterior knee pain score 87.3. Nearly anatomic reduction was obtained in all cases. Indirect reduction seemed to be useful for the treatment of comminuted patella fracture due to minimal soft tissue damage, moderate rigid fixation to allow early joint motion.

9.The Impact of Long-term Care Insurance on Medical Utilization and Medical Cost in South Korea

Background:
This study aimed to analyze changes in medical utilization and cost before and after long-term care (LTC) implementation.
Methods:
We used the National Health Information Database from National Health Insurance Service. The participants were selected who had a new LTC grade (grade 1–5) for 2015. Medical utilization was analyzed before and after LTC implementation. Segmented regression analysis of interrupted time series was conducted to evaluate the overall effect of the LTC implementation on medical costs.
Results:
The total number of participants was 41,726. A major reason for hospitalization in grade 1 was cerebrovascular diseases, and dementia was the top priority in grade 5. The proportion of hospitalization in grade 1 increased sharply before LTC implementation and then decreased. In grade 5, it increased before LTC implementation, but there was no significant difference after LTC implementation. As for medical cost, in grades 1 to 4, the total cost increased sharply before the LTC implementation, but thereafter, changes in level and trend tended to decrease statistically, and for grade 5, immediately after LTC implementation, the level change was decreasing, but thereafter, the trend change was increasing.
Conclusion
Long-term care grades showed different medical utilization and cost changes. Long-term care beneficiaries would improve their quality of life by adequately resolving their medical needs by their grades.
10.A Case of Asymmetric Septal Hypertrophy Combined with Conn's Syndrome.

We report a case of a 44 year old femele with unilateral aldosterone-proudcing adrenal adenoma characterized by hypertension, plasma aldosterone excess, and low plasma renin, commonly but not invariably with hypokalemia. She also had asymmetric septal hypertrophy of left ventricle established with two-dimensional echocardiography. The electrocardiogram showed inverted T wave and prominent U wave with high QRS voltage on precordial leads. In the case of this patient, we are not sure whether asymmetric septal hypertrophy was caused by secondary hypertension and chronic aldosterone excess of primary aldosteronism, or hypertrophic cardiomyopathy per se, so further long=term follow-up is required to determine it. Following the successful unilateral adrenalectomy, however, the systemic pressure fell down to the normal level and electrolyte abnormalities were corrected immediaterly within a few days and the modest regression in septal hypertrophy was noted in one year, suggesting that the promary aldosteronism contributes to the development or porgression of asymmetric septal hepertrophy.
