4.Clinical Study of Cementless Femoral Stem: Comparative Study between Porous Coating and Uncoating in Superolateral Portion of the Femoral Stem

The authors analyzed clinical and radiographic results of the 87 cases which had done THRA with porous coated femoral stem followed up more than 2 years to study the difference of clinical and radiologic findings between the absence (group 1 ; 53 cases) and presence(group 2 ; 34 cases) of the porous coating in superolateral portion of the femoral stem. The following results were obtained : 1. Harris hip scores were improved 34.4 points from 56.5 points preoperatively to 90.9 points postoperatively in group Ι, and 32.7 points from 61 points preoperatively to 93.7 points postoperatively in group 2. The clinical results of the group 1 were Excellent in 45 cases, Good in 5 cases and Fair in 3 cases, and clinical result of the group 2 were Excellent in 33 cases and Good in 1 case. 2. The changes of femoral stem angle were below 2 degrees in 7 cases, above 2 degrees in 2 cases in group 1, and below 2 degrees in 5 cases, but not found more than 2 degrees of change of the femoral stem angle in group 2. 3. The osteolysis around femoral stem were 15 cases(28%) in Group 1, and 5 cases(15%) in Group 2. Diffuse osteolysis were 3 cases in Group 1, but was not observed in Group 2. 4. The vertical subsidence more than 2 mm was 3 cases in group 1, but was not observed in group 2. 5. The stable femoral stem were 51 cases in group 1, and 34 cases in group 2. The unstable femoral stem were 3 cases in group 1, but was not observed in group 2. Above results suggest that the porous coating in the superolateral portion of the femoral stem showed better results in radiologic and clinical findings in short term follow up.

Keratoacanthoma is a benign neoplasm that develops as a protruding mass especially on the central part of face, We experienced a case of keratoacanthoma that developed on the nasal vestibule after the eradication of verruca vulgaris on the same site by surgical intervention 3 months before. 6.Republication: Sudden Postpartum Death from Pulmonary Arterial Hypertension.

We present here a brief literature review and a case study of postpartum pulmonary arterial hypertension, a rare and progressive disease with a high maternal mortality rate. The patient, a 32-year-old woman, presented with dyspnea and shivering immediately following an uncomplicated vaginal delivery. The patient had no remarkable individual or familial history. She died 4 hours after her delivery, and an autopsy revealed atherosclerosis of the main pulmonary arteries and characteristic plexiform lesions on the lung. The remaining organs showed no remarkable pathology. We reported pulmonary arterial hypertension associated with pregnancy as the cause of death.

7.Surgical Treatment of the Ruptured Abdominal Aortic Aneurysm Complicated with Abdominal Compartment Syndrome and Colon Ischemia .

A 76-years old man with a ruptured abdominal aortic aneurysm underwent an emergency abdominal aortic replacement with artificial graft. The patient developed abdominal compartment syndrome at the day of the operation and he received secondary decompression operation the next day. At 45 hours after the second operation the patient was returned to operation room to close the abdominal fascia, and sigmoid colon necrosis was found so we performed sigmoid colectomy with colostomy. After 22 days from the last operation, the abdominal wound was closed completely and the patient was discharged at the 42nd postoperative day with a colostomy state. We report here on this complex case together with a review of the recent articles.

8.A Case of Atypical Fibroxanthoma.

The lesion showed typical findings of atypical fibroxanthoma and was located on the lesion of solar keratosis, So sunlight may be the most important one among the various insults to atypical fibroxanthoma of the skin.

10.A Case of Malignant Melanoma.

The patient, 67 years old male, had thumb tip sized two tumor masses with linear arcuate spread on left upper back near spine of 3 months duration, and metastatic lesion on right axillary lymphnode.
