1.Survey on Ideal Number of Children and Characteristics on Eligible Women in Rural Korea.

Recently, during several years the number of ideal children have not changed at all. Because the most of Korean people considered that son is necessary absolutely for dependence in old age inheritance of family line and holding ritual and so on. Therefore, without revolution of value system for children we could no more except to reduce fertility rate. The survey is intended to compare the characteristics between those women who want two or less number of ideal children and all married women(regardless the number of ideal children). The results showed as follows: The women who want small size of family were younger and little better educated than those of all married women. The age at marriage of women who want small size of family was older than that of all married women. The conducted rate of induced abortion and acceptance rate of contraception among those women who want small size of family were higher than those of all married women. the rate of those who want less than 2 children so-called ideal No. among all married women was 3.9 percent.

2.Flow Cytometric DNA Analysis in Papillary Carcinoma of Thyroid Gland: comparison with Ki-67 immunohistochemical staining.

Nuclear DNA content was measured using a flow cytometric method to analyze 36 paraffin- embedded and 7 fresh tissues of 43 papillary carcinomas of thyroid gland. DNA aneuploidy was found in 3 cases(6.9%) and diploidy in 40 cases(93.1%). But there were no suggestive findings in clinical history, and cytological and morphological features for aneuploidy. In 40 diploid cases, S-phase fraction(SPF) were analyzed with regard to sex, age, tumor size, presence or absence of capsular invasion, lymph node involvement and ground glass nuclei. Among the multiple factors, only the tumor size, especially the larger sized-group(above 2cm in tumor diameter) was found to have a statistically significant higher SPF than the smaller sized-group (p<0.05). And high SPF groups relatively well corresponded to the high risk group. Thirty nine cases of papillary carcinoma have also been evaluated for proliferative activity with Ki-67 monoclonal antibody. The average Ki-67 labeling index was 0.36% in total cases, and that of the aneuploid cases was 0.73%, which was higher than that of the diploid cases(0.33%). So. We think that the low aneuploid rate and low Ki-67 labeling index relatively well represent the usual good clinical course of this tumor and the high SPF is a suggestive finding for a high risk group.

4.Development of Endocrine Cells of Human Pancreases: A Morphologic and Immunohistochemical Study.

Pancreases from 123 human embryos and fetuses ranging from 3 weeks to 40 weeks of gestation age were studied. Thirty four cases were examined by immunohistochemical preparations to investigate the temporal development of the pancreatic islet particularly with reference of its origin, time of appearance and interrelation of various types of islet cells. Following results were made. 1) The endocrine cells and the acinar cells both originated from the primitive pancreatic duct cells. 2) Alpha, beta and delta cells began to appear almost simultaneously at 10 weeks og gestation. A, B and D cells, in decreasing order of frequency, were all found in the entire areas of the pancreas. 3) Four types of fetal islets i.e., preimtive, "homologous", bipolar and mantle, could be recognized during development. The latter 3 types first appeared simultaneously at 17 weeks of gestation. They continued to increase in sized and number throughout the fetal life and constituted permanent islets. 4) Two types of islet cells could be distinguished with routine H&E staining after the second trimester. 5) The size of fetal pancreatic islets were rapidly increased between early (64 micrometer) and mid-term (113 micrometer) (p<0.05). 6) The primitive islets were composed of 37.3% of alpha cells, 41.3% of beta cells and 13.4% of delta cells. At mid-term the mantle islets were composed of 36.4% of alpha cell, 42.7% of beta cell and 25.6% of delta cell. At term beta cell was higher value (52.6%) than alpha cell (29.3%) and delta cell (17.6%) (p<0.05).

5.Differential Expression of Cytokine Genes in Cultured Epithelial Cells Stimulated with Uropathogenic Escherichia coli of Different Adherence Patterns.

To investigate the inflammatory cytokine production of human epithelial cell lines stimulated with uropathogenic E. coli strains showing 3 different adherence patterns, differential expression of inflammatory cytokine (IL-1a, IL-lB, IL-8, TNFa, and TGFB) mRNA were detected by RT-PCR. IL-1a, IL-1B, IL-8, and TGFB mRNAs constitutively expressed in epithelial cell lines, but not TNFa. The expression of IL-1a and IL-1B mRNA was increased in J-82 cells stimulated with E. coli strains showing DA, LA, or AggA pattern. The expression of IL-8 mRNA was increased, whereas TGFj3 mRNA was decreased in J-82 cells stimulated with E. coli strain showing AggA.pattern. Treatment with crude bacterial adhesins (CBA) isolated from E. coli strains showing DA or LA pattern increased IL-la, IL-lB, IL-S, and TGFj3 mRNA expressions in J-82 cells and HeLa cells. IL-la, IL-lB, and TGFB mRNA expressions were decreased in epitheUal cells stimulated with CBA from E. coli strain showing AggA pattern, whereas IL-8 mRNA expression was significantly increased. The expressions of cytokine mRNAs showed little differences between epithelial ceRs used, but great differences between CBA from DA or LA and AggA strain. LPS stimulation was little changed cytokine mRNA expressions in epithelial cells. This study suggests that cytokine gene expression of epithelial cells by the bacterial stimulation mainly depends on the bacterial adhesins recognized by the respective receptors of epithelial cells.

6.Histopathological Differences between Silicone Granuloma and Paraffinoma.

During the past two decades, silicone (polydimethylsiloxane) has become one of the most extensively applied biomaterials. Although pure silicone is relatively inert and usually causes only minimal tissue reactions, it has been reported to evoke a definite foreign body reaction. We studied five cases of silicone-induced granulomas in various sites; two in the breast, one in the breast and axillary lymph nodes, one in the subcutis of the abdomen, back and extremities and one in the eyeball, to illustrate the salient histopathologic features of reactions to silicone with particular emphasis to its differences from paraffin granuloma. For this, 17 paraffinomas were also studied. Tissue reaction to silicone liquid and gel was characterized by numerous round to oval empty cystic vacuoles, mild to moderate fat necrosis, foreign body reaction, a variable degree of mononuclear inflammatory cell infiltration and mild focal fibrosis. The cystic spaces were relatively uniform and showed a snow-man like appearance. In contrast to the silicone granulomas, the paraffinomas, also refered to as sclerosing lipogranulomas showed diffuse sclerosis and frequent calcification around the cystic vacuoles. The cystic spaces in paraffinomas were swiss cheese-like configuration, and the content of the cystic spaces was dirty and frequently calcified. However, there were certain similarities between these two types of granulomas particularly in the early phases of the reaction, therefore, the history of silicone injection or implant, is sometimes critical to the diagnosis of silicone granuloma. Despite great technologic advances in the manufacturing of prostheses and medical equipment, droplets and/or particles of silicone still escape into the body tissues in a variety of ways; therefores, the pathologist should always wonder whether the histologic reaction observed is due to silicone or to some other foreign material including paraffin.

9.Rhabdomyoma of the Orbit: A report of a case.

The patient was a 7 months old Korean girl, who was admitted to Department of Ophthalmology of the Seoul National University Hospital, with 6 months history of the left eyelid swelling and exophthalmos. Visual acuity and ocular movement were unaffected. Orbit CT revealed 1.5x.5 cm sized well circumscribed intraconal mass in the left retrobulbar space with pressure erosion of adjacent bone. At surgery, the mass was pinkish gray and firm, and was adherent to adjacent tissue. The whole mass could not be removed, and a local excision was done.
