2.Clinical Effect of Strontium 90 on Recalcitrant Benign Dermatoses.

Some of chronic benign recurrent dermatoses are markedly recalcitrant to the ordinary dermatological treatments. The author studied the therapeutic effect of beta ray on those of 30 patients including 6 cases of psoriasis, 3 of lichen simplex chronicus, 9 of nummular eczema, 8 of pustulosis palmaris et plantaris, and 4 of chronic eczema of the finger. All of the above were promptly recurred with ordinary treatments. Strontium 90 applicator was used as the source of beta ray and the total irradiation doses to each sites were ranged from 600 to 1500 rads in divided doses(usually weekly) over 2 to 3 weeks. The results were as follows, 1. In psoriasis patients with papular type were cleared by strontium 90, however, with chronic localized plaque type were poorly responded. The relapses of the cleared papular type were noticed between 13 to 24 days intervals, and it appeared that the larger doses did not influence significantly the clinical results. 2. Almost all of the chronic eczematous dermatoses including lichen simplex chronicus, nummular eczema, pustulosis Palmaris et plantaris, and chronic eczema of the finger, which were irradiated by beta rays following clearance with preliminary corticosteroid therapy, were recurred between 2 to 4 weeks intervals. However, larger doses resulted in prolonged recurrence intervals in a few patients. 3. No noticeable eariy hazards of irradiation were detected except for the mild hyperpigmentation on the irradiated skin.

3.Nutritional Risk and Its Contributing Factors in the Low-income Elderly in Urban Areas.

PURPOSE: This study was to evaluate the nutritional status of low-income elders in urban areas and factors affecting their nutritional risk. METHODS: A cross-sectional analysis was conducted. The subjects were 300 elders selected from home visiting clients of DongJack Public Health Center. Data were collected using a questionnaire containing questions on socio-demographic characteristics. health behavior and disease. dietary pattern. Nutritional Screening Initiative. Geriatric Depression Scale and Barthel Index for ADL. Collected data were analyzed through descriptive statistics. chi2-test and multiple regression analysis using SPSS. RESULTS: Of the subjects, 63% had high nutritional risk, 21.3% moderate nutritional risk, and 15.7% good nutritional risk. NSI score was significantly different according to economic status, subjective health condition, medication, dental health, depression. regularity of diet and meal with family. Multiple regression analysis revealed that depression, subjective health condition, dental health and regularity of diet and meal with family explain 38.1% of nutritional risk. CONCLUSION: It is necessary to evaluate nutrition status and to control nutritional risk factors such as depression, dental health, regularity of diet and meal with family for improving the health of the low-income elderly.

5.A Case of Imperigo herpetiformis.

Impetigo herpetiformis is a rare disease which has not been cleary defined in the European or American literature and is currently not fully appreciated or accepted as a clinical entity. According to Baker & Ryan, Impetigo herpetiformis was reffered to as the Exanthematous type of generalized pustular psoriasis. Impetigo herpetiformis starts suddenly without any preceding lesions of psoriasis as an extensive eruption of pustules on an erythematous base, and may. occur repeatedly during successive pregancies but may occur also without any known cause. A case of Impetigo herpetiformis affecting 30 year old 8 month pregnent woman, who has been suffering from generalized coaIesced pustular eruption with slight itching & buming sensation with fever & chill. 15days before, The Eruption began as an erythematous macule which developed pustules along the margin. There was extension by the development of fresh peripheral pustules and coalescence of these patches and central clearing. Diagnasis was confirmed by histopathologic finding and treated with cortico.teroid and Antibiotics, But improvement was not noted.

6.A Case of Klippel-Trenaunay-Weber Syndrome.

A case of Klippel-Trenaunay-Weber syndrome described originally by Klippel and Trenaunay on 1900, was presented with a brief review of literatures. The patient, 30 year-old male, said that he had noticed a reddish plaque in his right leg at the age of about 10 and had felt the asymmetricity of the limbs about 5 years later. On physical examination, he had an ulcerated sclerosing hemangioma on his right limb which is extending from medial aspect of upper one third of the lower leg to the foot. It was also associated with the varicosities just above the hemangioma, edema and hypertrophy around the right ankle, and a visible elongation of the right limb. A plane X-ray revealed osteohypertrophy of tibia and fibula without any periosteal reaction, and a soft tissue hypertrophy was also noticed around the right ankle. Histological findings of the hemangioma revealed moderate fibrosis around the proliferated vascular tissue and two of the remarkable thrombosis in the median sized vessels.

7.cDNA Microarray in Psychiatry.

The development of inexpensive high throughput methods to identify individual DNA sequences is important to the future growth of medical genetics. This has become increasingly apparent as psychiatric geneticists focus more attention on the molecular basis of complex multifactorial diseases at which most of psychiatric disease is estimated. Furthermore, candidate gene approaches used in identifying disease associated genes necessitate screening large sequence blocks for changes tracking with the disease state. Even after such genes are isolated, large scale mutational analysis will often be needed for risk assessment studies to define the likely medical consequences of carrying a mutated gene. This review provide basic knowledge of up-to-date technology, cDNA microarray which enables above mentioned various research themes.

8.The Effects of Solution-Focused Group Counseling on the Stress Response and Coping Strategies in the Delinquent Juveniles.

PURPOSE: This study was conducted to examine the effects of solution-focused group counseling on the general stress, stress response and coping in the delinquent juveniles. METHOD: The subjects consisted of a convenience samlple of 60 delinquent juveniles who had been placed under probation in Kwang-ju, and recruited from Oct., 2000 to Dec., 2000. Solution-focused group counseling was conducted for the experimental group once a week for 120 to 180 minutes for 6 week. The control group received no group session. For both groups the level of general stress, stress response and coping were measured before and after the experiment. RESULT: General stress significantly decreased in the experimental group and increased in the control group, but showed no significant difference between the two groups. Stress response decreased significantly in the experimental group and increased in the control group, but showed no significant difference between the two groups. The problem-focused coping significantly increased in the experimental group and decreased in the control group, and showed a significant difference between the two groups. The emotion-focused coping significantly decreased in the experimental group and increased in the control group, and showed no significant difference between the two groups. CONCLUSION: This study showed that solution-focused group-counselling has a positive effect on variables like the general stress, stress response, emotion-focused coping and problem-focused coping. Further study needs to be conducted to verify results.
9.Characterization of cytoplasmic Form of Human CTLA - 4 Molecule.

CTLA-4 (=CD152), a T cell activation antigen, has been known to be homologous to CD28 in its molecular and genomic structure. Both of these two molecules are sharing their counterreceptors, B7 (CDSO) and B7-2 (CD86) and are known to play a crucial role in T cell activation. In previous our study it was reported that there are 2 forms of CTLA-4 antigen in activated human T cells, 30 kD membrane-bound form and 34 kD cytosolic-sequestered form and the former was less than 5 % of total of this antigen induced. Aims of this study are to confirm previous finding by using flow cytometry and to characterize the cytoplasmic form of human CTLA-4 by using ultrafiltration and immunoprecipitation techniques. In PHA stimulated peripheral blood lymphocyte surface expression of CTLA-4 was less than 2.1% of any of CD4+, CD8+ and CD56+ subsets. And the 34 kD form of CTLA-4 was detected in CDS+ subset only. This discrepancy confirms that 34 kD antigen is the cytoplasmic form of human CTLA-4. In ultrafiltration and subsequent Western blot analysis study this 34 kD antigen was detected in >100 kD fraction only. And in non-reducing condition this antigen formed high molecular weght complex (MW > 350 kD). In immunoprecipitation study using anti-peptide A antibody it was found that this high molecular weight complex consists of the 34 kD cytoplasmic form of CTLA-4 and previously unknown 54 kD antigen and 46 kD antigen at 1:1:8-10 ratio. And none of these 3 molecules were identified in membrane fraction of activated human T cell. The result of this study implies that CTLA-4 molecule induced upon T cell activation mainly sequestered in cytoplasrn and another signal is necessary to target this antigen on the activated T cell surface.
