2.A Case of Toxic Epidermal Necrolysis.

A case of Toxic epidermal necrolysis in 19 th years old man is presented. This patient has been suffering from erythematous patches and bullae over entire body including vesicles, erosions, and fissures on both lips since 2 days ago after taking some drugs (analgetics, aspirin, chloramphenicol, sulfisoxazole, erythromycine.) for treatment of tonsillitis. Furthermore, it is very difficult for us to say why the disease has occured. Toxic Epidermal Necrolysis (TEN) by talking drugs or Staphylococcal Scalded Skin Syndrpme (4S) by staphylococcus aurues because staphylococcus aureus was found on bacteriai culture from his throat swab. He was treated with corticosteroid, antibiotics and fluids for 20 days with good result. Literature was briefly reviewed.

3.Effect of Growth Hormone on Death of Sertoli Cell.

PURPOSE: Growth hormone(GH) receptors have been detected in Sertoli cells and clinical observations suggested the involvement of GH action(either indirectly via IGF-I or directly) on testicular function. This study is aimed at assessing the effect of GH on cell death during Sertoli cell culture. METHODS: Sertoli cells(TM4, mouse, male, ATCC, USA) were cultured by using DMEM(Dulbecco' Modified Eagle' Medium, Biowhitteker, USA) and Fetal Bovine Serum(10%, GIBCO BRL, USA) with or without human recombinant growth hormone(Eutropin, LGCI, Korea) of different concentrations(10, 50, 100, 500 microgram/mL of media), in 34degress C, 5% CO2 incubator. After cultivation during 5 days, Sertoli cells were examined under light microscope(x400). RESULTS: Number of survival Sertoli cell was remarkably increased in the GH treated(50 microgram/mL in media) group(62.6+/-12.4) compare to control(22.4+/-4.8)(P<0.05). Degree of survival Sertoli cell was increased proportionaly until 50 microgram/mL in concentration of GH in media, but decreased in higher than it. CONCLUSION: GH has the protective effect on death of Sertoli cell and it depends on GH concentration.

4.A Case of Pulsating Exophthalmos.

A case of pulsating exophthalmos is reported in a 50 years old Korean Woman. The complaints of this case were exophthalmos, chemosis on Rt. eye and intracranial bruit. The etiology of this case is considered due to toxemia of pregnancy and hypertension without trauma. A brief review of the literatures related with the etiology, symptom and therapy is made.

5.A case of Bronchogenic Cyst in Diaphragm.

Isolated bronchogenic cysts of the diaphragm are rare abnormalities. They are usually asymptomatic unless secondarily infected or large enough to cause compression of vital structures. The patient was a 31-year-old man and had no symptoms except mild postprandial epigastric discomfort. The chest X-ray and chest C-T examination revealed a mediastinal mass at the left cardio-vertebro-phrenic angle. We performed the operation under the impression of solid mass at mediastinum. We revealed that the mass was bronchogenic cyst in diaphragm. Therefore we report this case with review of literatures.

6.Dupuytren's Contracture associated with Dlabetes Mellitus.

Dupuytren's contracture is a idiopathic hypertrophy of palmar aponeurosis and consequent flexion deformity of distal palms and fingers. This patient is a 58-year-old man who has been suffering from diabetes mllitus since last 20 years. Skin lesion developed from one year ago. Skin lesion is a well defined, yellowish brown, nontender, firm, pea-sized nodule at distal palmar crease proximal to ring finger on both hands. On physical examination, he is within normal limit except skin lesion and flexion deformity on both hands. Skin biopsy on nodule revealed non-specific fibrosis. Literature was briefly reviewed.

7.Report of Two Cases of Hypomelanosis of Vitiligo Type Probably due to Benoquin.

Two cases of vitiligo-type hypomelanosis followed by the local application of Benoquin ointment for the treatment for the treatment of melasma are reported. These two cases of hypomelanosis are unwanted and unintended side effects probably caused by the application of monobenzyl ethers of hydrcquinone (monobenzone). Monobenzone blocks the enzyme tyrosinase and thereby prevents the conversion of tyrosine to dihydroxyphenylanine, a precursor of melanin, and since this does not destroy melanocyte or facilitate the loss of melanin. The skin contact with the material will not occur, repigmentation will slow but eventuaIly complete.

9.A Case of Lupus Vulgaris on the Patella Region of the Right Knee Joint.

A case of lupus vulgaris who has the typical skin lesion on the patella region of the right knee was presented. The patient is 9 years old, the generally healthy male, and is no special finding of chest X-ray but tuberculin test was strong positive. The skin lesion started about 2 years ago as baby's palm sized erythematous patch with apple jelly nodule on the right knee joint region. Diagnosis was made by characteristic of clinical appearance and histulagial finding including other laboratory data. The treatment was done with INAH, PAS, streptomycin and pyridoxine for 3 months. Now,The skin lesion is almost healed and still continue treatment and observation go on.

10.Clinical and Histopathological Observation on Pityriasis Rosea.

The present study was performed to evaluate clinical manifestations and histopatological findings of 95 patients with pityriasis roses who visited the Department of Dermatology in National Medical Center from October of 1979 to September of 1982. The results are as follows, 1. Clinical Manifestations: 1. The number of patients with pityriasis roses were 95, which occupied 0. 98/o of new out-patients in the department of dermatology in the same period. 2. In sex distribution, males were 36 and females were 59, with male to female ratio of I: l. 6. R. Age of onset varied from 13 months to 58 years with a mean of 26. 0 years and 68% of cases belonged to 15 years to 84 years. 4. Seasonal incidence was 32% in winter, 27% in summer, 22% in autumn and l9% in spring. 5. iMild prodromal symptoms developed in 8 cases(%) and itching was noted in 58 cases (61%), which was mild to moderate exeept for severe itching in 2 cases. 6. Herald patch was found in 42 cases (44%) and the predilection sites were the trunk and the thighs. 7. Daughter lesions could be classified into macular type 69 cases (73g), pa.pular type 22 cases (23%), pustular type 2 cases (2%), urticarial type 1 case (I,3 and purpuric type 1 case (1%) They appeared on the back (79%), the chest (74%), the abdomen(73%), the thighs (47%), the upper arms (40%), the lower legs (24%),the forearrns (22%), the neck (16%), the face (7 %), dorsa of the hands (4%) and dorsa of the feet(2%).
