1.Diagnostic Accuracy of Low Tesla MR Imaging in the Internal Derangement of the Knee.

PURPOSE: This study is for the evaluation of low tesla(0.064T) MR imaging diagnostic accuracy in the internal derangement of the knee. MATERIALS AND METHODS: We retrospectively analysed the MR images of 36 injured knees of 35 patients. The presence of tear was determined by arthroscopy or surgery in all cases. RESULTS: The sensitivity, specificity, accuracy, positive predictive value, negative predictive value of low tesla MRI for the diagnosis of anterior cruciate ligament injury were 83%, 88%, 86%, 77%, 91%, for the posterior cruciate ligament 75%, 95%, 86%, 92%, 83%, for the medial collateral ligament 83%, 96%, 92%, 91%, 92%, for the lateral collateral ligament 67%, 97%, 94%, 67%, 97%, for the menisci 75%, 93%, 89%, 75%, 93%. CONCLUSION: The low tesla MRI is an accurate method in detection and evaluation of the internal derangement of the knee.

2.Ultrasonographic findings of ovarian tumors

The ultrasound is easily available and noninvasive diagnostic method without radiation hazard. We can approachto differential diagnosis of ovarian tumors by gray sale ultrasound which can display the detailed structure ofsoft tissue. In department of radiology, Korea Veterans Hospital, 27 cases of histologically confiremd ovariantumors collected from March 1984 to February 1986 were analized. The results were as follows: 1. The agedistribution was from 13 to 72 years. 2. The histologic diagnosis of ovarian tumors(27 cases) are as follows:teratoma(14), mucinous cystadenoma(8), mucinous cystadenocarcinoma(1), serous cystadenoma (2), Krukenburg tumor(1)and malignant thecoma (1). 3. The long diameter of ovarian tumors ranges from 4cm to 28cm. In these, mucinouscystadenoma is the largest and their size are as follows; less than 10cm; 25%, 11–15 cm; 12%, 16–20 cm; 25% andmore than 20cm; 38%. 4. The ultrasonographic findings of teratoma(14) were mainly cystic (8) or echogenic(6).There were echogenic solid components showing distal acoustic shadowing or fat-fluid level within cystic masses.Echogenecity of solid masses was heterogenous. 5. The ultrasonographic findings of mucinous cystadenoma&cystadenocarcinoma(9) were unilocular cystic (3) or multilocular cystic(6). There were variable amount ofechogenic component within cystic masses. 6. Those of serous cystadenoma(2) were unilocular cystic (1) ormultilocular cystic (1). 7. Krukenburg tumor(1) and Malignant thecoma(1) were heterogenously echogenic.

3.CT and Fluoroscopy Guided Celiac Ganglion Block.

PURPOSE: To evaluate the effects and usefulness of fiuroscopy guided celiac ganglion block after marking of needle path with CT scan. MATERIALS AND METHODS: Celiac ganglion block with 100% ethyl alcohol was performed in 50 cancer patients who were inoperable and had intractable abdominal pain. Duration and degree of pain relief after the procedure and its complication were analyzed. RESULTS: Early pain relief was observed in 98% and long term relief in 68% without serious complication. CONCLUSION: Fluoroscopy guided celiac ganglion block after marking of needle path with CT scan was a safe and valuable procedure in relieving intractable pain in terminal cancer patients and reduced the time in the CT room.

4.A Case of Intrahepatic Cholangiocarcinoma in Polycystic Liver Disease.

Cholangiocarcinoma has been associated with various fibrocystic diseases of liver and biliary tract, but cholangiocarcinoma in polycystic liver disease (PLD) was extremely rare. It was reported that the prognosis of cholangiocarcinoma associated PLD was very poor and distant metastases were common. We report a rare case of intrahepatic cholangiocarcinoma associated with PLD in 58- year- old female who presented vague abdominal pain. She had also polycystic kidneys with normal renal function. MRI showed well demonstrated tumor with central scar and a adjacent large cyst in the left lobe of the liver. She underwent extended left lobectomy. On microscopic examination, the tumor was moderately differentiated cholangiocarcinoma having abundant fibrous stroma and necrosis and the adjacent cyst showed focal in situ carcinomatous changes in the lining epithelium.

5.Spinal Cord Infarction following Off-pump Coronary Artery Bypass Surgery.

A 62-year-old woman with ischemic heart disease, hypertension and hypercholesterolemia had developed spinal cord infarction after off-pump coronary artery bypass (OPCAB). The incidence of postoperative neurological complications is well known to be less in OPCAB than that of conventional coronary bypass surgery. Furthermore, spinal cord infarction is an uncommon clinical event after coronary bypass surgery. Here we report a case of spinal cord infarction following OPCAB, discuss possible mechanism of spinal cord infarction with relate literatures.

6.A Case of Inadvertent Anterior Chamber and Corneal Stromal Injection with Antibiotics during Cataract Operation.

PURPOSE: To report a case of inadvertent anterior chamber and cornea stromal injection with high dose antibiotics and steroids during cataract operation. METHODS: During cataract operation on a 78 year-old female patient, high dose gentamicin (20 mg/0.5 ml) and dexamethasone (2 mg/0.5 ml) were inadvertently injected into the anterior chamber and cornea stroma when making cornea edema for sealing of the incision sites. Anterior chamber irrigation with balanced salt solution (BSS) was immediately administered. On postoperative day one, extensive cornea edema was noted, and best-corrected visual acuity was 0.2. Descemet's membrane folds were observed around the corneal incision sites. Topical 5% NaCl and 1% prednisolone were started. RESULTS: Four weeks postoperatively, corneal edema began to reduce significantly. At four months postoperatively, corneal edema fully resolved, and best-corrected visual acuity was 0.8. However, some Descemet's membrane folds still remained, and a decrease in the number of endothelial cells was noted by specular microscope. CONCLUSIONS: In this case involving anterior chamber and cornea stromal injection with high dose antibiotics and steroids, immediate anterior chamber irrigation with balanced salt solution seemed an appropriate management, and the patient's long-term visual acuity appears good. To prevent such mistakes, precise labeling of all solutions and use of different syringe needles should be considered.

7.The Concentration of Tra n s forming Growth Factor-beta2 from Rabbit Eye by Laser Retinal Photocogulation.

Laser photocoagulation is commonly used for the treatment of retinal neo-vascularization in ischemic retinal diseases. Transforming growth factor-beta2 is known to be produced by RPE cells after laser photocoagulation and to be related to both regression of neovascularization and acceleration of preretinal fibrosis. In this study, we investgated the safe extent of laser photocoagulation not promoting the fibrosis by measuring intravitreal TGF-beta2 released following treatment. In gray rabbit eyes, trans pars plana vitrectomies and laser photocoagulations[1/8, 1/4, 1/2 of the fundus]were performed. The each eyeball was enucleated and TGF-beta2 concentration was measured at 12, 24, 72 hours after laser photocoagulation. And fibrotic activity was observed by the subconjunctival injection of fibrocyte and TGF-beta2 [0.1, 1, 10ng/milliliter]. The results were as follows:1. When TGF-beta2 was injected subconjunctivally along with fibrocytes, fibrocytic activity was observed found at the concentration of 1 or 10ng/milliliter of TGF-beta2. 2.The intravitreal concentration of TGF-beta2 after laser photocoagulation to the 1/8, 1/4, 1/2 of fundus area were 0.05, 0.1, 0.2ng/milliliter respectively. From the above results, we suggest to treat limit the area of laser photo-coagulation to 1/8 of fundus area in order to prevent the retinal fibrosis.

10.Plasma Levels of Brain Natriuretic Peptide Predict Postoperative Atrial Fibrillation in Patients Undergoing Heart Surgery.

BACKGROUND: The brain natriuretic peptide (BNP) level has been reported in some studies to be associated with the occurrence of atrial fibrillation (AF). The aim of this study is to evaluate the potential usefulness of the BNP level as a predictor of the occurrence of postoperative (postop) AF and to assess the relationship of the BNP level with the onset of AF and the restoration of sinus rhythm. MATERIAL AND METHOD: From January 1, 2005 to February 28, 2006, 82 patients without a history of atrial arrhythmia that had undergone cardiac surgery were enrolled in the study. Blood samples for plasma BNP were drawn daily for all these patients from the preoperative (preop) day to the 7th postop day. The patient records were reviewed and postop EKGs were checked daily for AF until the time of discharge. RESULT: Patients were divided into two groups based on development of postop AF. Postoperative AF developed in 26 patients (31.7%). There was no significant statistical difference in age, sex distribution, preop left ventricle ejection fraction, hypertension, left ventricular hypertrophy, or the use of beta blockers between the non-postop AF and postop AF group. More patients in the AF group had undergone valve surgery (39.3% versus 76.9%, p=0.002). The preop left atrium size was significantly larger in the AF patients (43.8+/-10.3 mm versus 49.8+/-11.5 mm, p=0.029). The preop plasma BNP levels were higher in the postop AF patients (144.1+/-20.8 pg/mL versus 267.5+/-68 pg/mL, p=0.034). In the postop AF group, the plasma BNP level was the highest on the 3rd postop day. Postop AF developed in most patients by the 3rd postop day; restored sinus rhythm developed by the 7th postop day. CONCLUSION: Elevated plasma BNP levels may lead to the occurrence of postop AF in patients undergoing cardiac surgery. Patients who have a high risk of postop AF should be considered for aggressive prophylactic antiarrhythmic therapy.
