1.A study of serum and spinal fluid enzyme such as CPK and LDH as predictors of neurologic disability following perinatal asphyxia.

Perinatal asphyxia is an insult to fetus or newborn due to lack of oxygen or perfusion to various organs, especially to brain, resulting in the important complication known as hypoxic ischemic encephalopathy. A clinical study was conducted on 30 newborns with perinatal asphyxia, who were admitted to Pediatric department of Ewha Women's University Hospital during 18 months period from Sep. 1990 to Feb. 1992. CPK, LDH and isoenzymes were measured in 30 asphyxated neonates and compared with control group. The results were as follows: 1) The serum CPK values at 1st and 7th day were not different in the asphyxia group and control group. 2) The serum CPK-BB and MB fraction at 1st day were highly significant in the asphyxia group. 3) The CSF CPK value at 1st day was highly significant in the asphyxia group. 4) The serum LDH values and the serum LDH isoenzyme 3 were highly significant in asphyxia group at 1st day. 5) The CSF LDH values and the CSF LDH isoenzyme 2 and 3 were highly significant in asphyxia group at 1st day. So, we concluded the serum CPK-BB fraction, the CSF CPK values, the serum LDH values, the serum LDH 2 fraction, the CSF LDH values and the CSF LDH 2,3 fraction as predictors of neurologic disability following perinatal asphyxia.

2.A Study on the Educational Effects on Child-Raising Knowledge and Satisfaction with Out-Patient Care of Mothers with Ill-Child.

The purpose of this study were to find out the practical way to enlarged child-raising knowledge and to enhance their satisfaction with out-patient care by evaluating how effectively the education is done by nurses for mothers with ill-child and how their satisfaction with out-patient care changed. This study was designed as a Nonequivalent Control Group study. The subjects studied were consisted of the experimental and control group. Each consisted of 50 mothers with ill-child in pediatric department at one university hospital in Seoul. The period of this study is from May 20, 1996 to June 28, 1996. The first data were collected fro both of experimental and control groups in which mothers with ill-child come to the hospital for the first time. After this being done, the experimental group had been educated by the planned program and then the second data were collected from them. On the contrary, as for the control group, there had been no education and the second data were collected on the same method. The data analysis was done by SPSS program. The results of this study are as follow, 1. The child-raising knowledge level of mothers with education was higher than that of with no education.(t=18.84, df=49, p=0.000) 2. The satisfaction with out-patient care level of mothers with education was higher than that of no education.(t=10.51, df=49, p=0.000) Based on these results, I suggest as follow, 1. The research on the patients and their family should be made not only in pediatric department, but in every out-patient department. 2. For more effective education, it is required for all out-patient nurses to research the education demand of patients and their family. 3. To increase the effect of education, there must be the consultation room in out-patient department. 4. The meetings with the mothers with ill-child of the same illness have to be established and periodical education must be executed. 5. Audio-visual education programs like video tapes are needed to make use of waiting time for the medical treatment. 6. On-line consulting program are needed.

3.Tumor Angiogenesis and Vascular Endothelial Growth Factor Expression in Cervical Intraepithelial Neoplasia.

Angiogenesis is an essential requirement for development, progression, and metastasis of malignant tumors. Vascular endothelial growth factor (VEGF) is one of the important angiogenic factors. Recently the role of angiogenesis has been known in premalignant lesions. This study was performed to determine whether the angiogenesis and VEGF expression were increased in association with histological grade of cervical intraepithelial neoplasia (CIN) and to see the relationship between the angiogenesis and VEGF. Immunostainings for factor VIII and VEGF were performed on 52 cases of cervical neoplasia (12 cases of CIN I, 11 cases of CIN II, 15 cases of CIN III, 7 cases of microinvasive squamous cell carcinoma, and 7 cases of invasive carcinoma) and 5 cases of normal cervix. The results showed a significant increase of microvessel count from normal cervix through CIN grades to invasive squamous cell cacinoma. VEGF expression was increased in proportion to the CIN grades. There was no significant correlation between microvessel count and VEGF expression. In conclusion, the tumor angiogenesis is an early event in tumorigenesis of uterine cervix. In addition, no significant relationship between the microvessel count and VEGF expression in CIN suggests the possibility of other growth factors affecting mainly angiogenesis of premalignant lesion of uterine cervix.

4.Occupational Stress of Women Workers in Service Industries depending on Their Working Conditions.

PURPOSE: The purpose of this study is to discover the influence of the working conditions on women workers' occupational stress. METHODS: Descriptive method is used to identify women's stress depending on their general working features and conditions by conducting a survey of them in women-dominated service industries. SPSS 18.0 program is used for data analysis and descriptive statistics is presented with standard deviation, frequency and percentage. chi2-test is used as an analysis tool. To figure out factors that influence their stress, logistic regression analysis is used for general features and working conditions as independent variables, and occupational stress as a dependent variable. RESULTS: As a result, among the independent variables, employment status, weekly working hours, career, shiftwork, and work-family-balance are meaningful factors that influence their stress. Temporary workers' stress is 3.65 times higher (p<.001), and workers working over 48 hours a week have 1.97 times higher stress (p<.003). Workers with over 5 years' career are under 1.73 times higher stress (p<.046) and shift workers are under 3.51 times higher stress (p<.001). Work family balance results in 1.93 times higher stress (p<.009). CONCLUSION: It is necessary to seek how to prevent and manage women workers' stress considering features.

5.Flow Cytometric DNA Content Analysis in Breast Cancer Comparison study of fresh and paraffin-embedded tissues.

DNA content of 25 cases of breast carcinoma was analyzed by flow cytometry in both fresh and formalin-fixed, paraffin-embedded tissue. Aneuploidy in fresh tissue and paraffin-embedded tissues was 72% and 32%, respectively. There was a 52% agreement in analysis of DNA ploidy between fresh and paraffin-embedded tissues. Most of the discrepancies resulted from loss of aneuploid peaks on the histograms of paraffin-embedded tissue. Mean S-phase fraction was slightly higher in a paraffin-embedded tissue than that in the fresh tissue; 19.2 9.1% versus 16.1 8.8% and there was no significant correlation between the S-phase fractions. In statistical analysis, the histologic and nuclear grades were not correlated with ploidy or mean S-phase fraction. Therefore it is strongly recommended to use the fresh tissue in flow cytometric DNA content analysis of breast cancer.

7.Combined Mucinous Tumor and Carcinoid of Appendix Associated with Mucinous Tumor of Ovary and Pseudomyxoma Peritonei: A case report .

We report a case of mucinous cystadenoma of uncertain malignant potential and carcinoid of appendix associated with bilateral mucinous cystadenoma of ovary and pseudomyxoma peritonei. The patient was a 46-year-old female. She suffered from dyspnea and lower abdominal palpable masses for several months. Ultrasonogram showed multilocular huge ovarian cysts. Appendectomy, transabdominal hysterectomy, bilateral salphingo-oophorectomy and biopsy of omentum were performed. The bilateral ovaries measured 16 11 cm and 7X5 cm in size, respectively. The both ovaries showed multilocular cysts filled with thick mucus material. The ovarian cysts were covered by a single layer of columnar epithelium with focal proliferation. Mucus materials dissected through the ovarian stroma (pseudomyxoma ovarii). The tip of appendix was dilated and covered by mucus material. The cut surface showed a cyst and a yellowish solid mass proximal to the cyst. Microscopically, the appendiceal cyst was lined by stratified columnar epithelium with moderate cytologic atypia. Mucus material dissected through the wall. In the proximal portion of the appendix, a classic carcinoid with focal tubular form was present in submucosa and muscle layer. The omentum was covered by thick mucus material. Microscopically, the omentum showed mucinous epithelium and mucus material (pseudomyxoma peritonei).

Renal cell carcinoma is the most common primary cancer of the kidney. The tumor stage is a reliable prognostic marker in renal cell carcinoma which is significantly associated with patient survival. But assessment of other prognostic factors has produced varying and often conflicting results. We reevaluated the significance of varied prognostic parameters in 33 cases of renal cell carcinoma; clinical stage, cell type, histologic pattern, DNA ploidy, Ki-67 labeling index, and bcl-2 oncoprotein expression. We could not statistically prove that DNA ploidy and bcl-2 expression were related to any examined parameters. Cell type was not related to clinical stage nor nuclear grade but there was a significant correlation (p=0.002) between cell type and histologic pattern. Nuclear grade (p=0.007) and Ki-67 labeling index (p=0.036) were significantly related to clinical stage, suggesting their value as complementary prognostic markers for renal cell carcinoma.
