December 19, 1995 is the 80th anniversary of Alois Alzheimer's death. Recently, dementing illness including Alzheimer's disease has become the center of public interest in Korea. However, there are not much information about him or historical background of Alzheimer's disease. It is even misunderstanding. It seems to be meaningful to introduce the lifetime of the Alois Alzheimer and the historical background of Alzheimer's disease, especially in the anniversary of his death.

5.Meningioma, Hemangiopericytic Type, with Bone Metastasis: A report of a case.

Since Lebert stated, in 1851, that fibroblastic and sarcomatous tumors of the intracranial cavity had different survival times, many neuropathologists tried to set up histopathological grading system predicting the prognosis of the tumor of the nervous system. Especially, the histological assessment of the aggressiveness of meningiomas has not always been useful in conforming the biological characteristics of these tumors, and the definition of malignancy is still subject to controversy. However, metastases from meningioma to remote sites have always been considered rarities. The authors experienced a case of metastatic meningioma, hemangiopericytic type, with a history of recurrence. A forty years old male patient received a craniotomy to remove a mass in right parasagittal region, in June 1981. He had suffered from Jacksonian type of seizure started from left foot throuth leg, thigh, and arm to face. During operation, a well defined mass was noted in left parasagittal region with cleavage plane. The tumor was completely removed with cauterization of invasion site. In microscopic examination, the mass had typical findings of hemangiopericytic type of meningioma. In September 1987, he received a repeat operation to remove a recurred tumor mass in vertex, at age of forty-six. The removed tumor revealed same histologic features as those seen in primary tumor. In January 1990, at age of forty-nine, he noted pain and tenderness on the right iliac crest and the left iliac tuberosity, Plain pelvis AP x-ray ad Pelvis CT scan revealed osteolytic expansile mass lesions involving both iliac bones, the body of second sacral vertebra, and the right superior ramus of the pelvic bone. An open biopsy of the iliac bone was performed. The microscopic findings of the bone lesion were same as reccruent lesion of the brain.

7.Comparative Study in DNA-mediated Vaccination Efficaency Among the Plasmids with Different Promoters.

Plasmid vectors with either RSV or CMV promoter are frequently used for DNA- mediated immunization due to the availability in commercial. Consequently, influence of the vector constituents, such as promoter, enhancer and transcription termination signal etc. on vaccination efficiency is not studied extensively. As an initial attempt to develop an efficient vector system for DNA-rnediated immunization, influence of promoter for antigen gene expression on vaccination efficiency has been analyzed. Initially, plasmids with either B-actin or muscle creatine kinase (MCK) promoter were constructed from the plasmid with prototype CMV promoter. In addition, ovalbumin (OVA) antigen gene has been cloned into each vectors to generate the plasmid vectors with different promoters for induction of the anti-OVA immune responses. Antigen protein expression in antigen gene transfected mouse muscle myoblast cells showed that the level from MCK promoter containing plasmid was slightly higher than those from either CMV or B-actin promoter containing plasmids. Also, the same plasmid turned out to be slightly more efficient than other plasmids in antibody imrnune response induction in vivo, when they were applied both through intramuscularly and intradermally. These results suggest that the commonly used CMV promoter containing plasmid vector could be further modified to develop an efficient vector for DNA-mediated immunization.

8.Expression of p53 and Matrix Metalloproteinase-2 Proteins in Colorectal Adenocarcinoma.

The p53 gene is believed to play an important role through the mutation and overexpression in the progression of various human malignant tumors. The type IV collagenase (matrix metalloproteinase: MMP-2) initiates the degradation of the extracellular matrix, and consequently may play a role in the tumor invasion and metastasis. To investigate the correlation between clinicopathologic features of the colorectal adenocarcinomas and benign tumors and expression of p53 and MMP-2 proteins, we performed an immunohistochemical study on 40 colorectal adenocarcinomas, 20 adenomas and 20 hyperplastic polyps by using the antibodies to p53 and MMP-2 proteins. The positive expression rate of the p53 protein in adenocarcinomas was 62.5% and significantly higher than in benign tumors. The positive expression rate of the MMP-2 protein was 47.5% in adenocarcinomas, but there was no expression of MMP-2 protein in benign tumors. The difference in p53 and MMP-2 expression rates between malignant and non-malignant tumors was statistically significant. The positive expression rate of p53 protein in the non-metastatic and metastatic adenocarcinomas was 59.1 and 66.7%, respectively. The positive expression rate of MMP-2 protein in the non-metastatic and metastatic adencarcinomas was 45.5 and 50.0%, respectively. The correlation between several clinicopathologic features and expression of p53 and MMP-2 protein was not statistically significant, but the rate of positive MMP-2 immunoreactivity showed a statistically significant difference between Astler-Coller stage B1 C1 group and B2 C2 group of adenocarcinoma (p=0.0431). We concluded that the expression of p53 and MMP-2 protein contributes to the cancer development and MMP-2 may play a certain role in the invasiveness of the colorectal tumor. p53 and MMP-2 protein expression is not correlated with lymph node metastasis.

10.The Efficacy of Combination Therapy of 5 alpha -Reductase Inhibitor and of-Adrenergic Blocker in Benign Prostate Hyperplasia.

PURPOSE: Benign prostate hyperplasia(BPH) can be treated with alpha1-adrenergic blocker that relaxes prostate smooth muscle or 5 alpha-reductase inhibitor that reduces serum dirtydrotestosterone. The efficacy of the combination of 5 alpha -reductase inhibitor(finasteride) and alpha1-adrenergic blocker(doxazosin) was evaluated in patients with benign prostate hyperplasia. MATERIALS AND METHODS: Eighty five patients with BPH was treated and followed over 6 months and divided into three groups: Group 1(doxazosin 3mg/day), Group 2(finasteride 5mg) and Group 3(combination of both drugs). Initially, all patients were evaluated by international Prostatic Symptom Score(IPSS: irritative, obstructive, sum, life quality), uroflowmetry, residual urine, serum prostate specific antigen(PSA) and prostate weight by transrectal ultrasonography. IPSS, uroflowmetry and complications were evaluated every month. Residual urine and PSA were assessed at every 3 months, prostate weight at every 6 months. RESULTS: In Group 1 and 3, IPSS were more decreased than In Group 2 immediately(p < 0.001). In Group 1 and 3, maximal flow rate was more increased than in group 2 immediately(p < 0.001). There was no difference of mean change of residual urine among three group. In Group 2 and 3, serum prostate specific antigen and prostate weight were more decreased than in Group 1 (p < 0.001). CONCLUSIONS: In medical treatment of BPH, the combination therapy of alpha1-adrenergic blocker and 5alpha-reductase inhibitor shows early symptomatic improvement and decreased prostate weight without significant complications.
