1.Clinical Effects of Venitol(R) on Complications after Hemorrhoidectomy Prospective Randomized and Placebo-controlled Trial.

Fifty four patients who received closed hemorrhoidectomy were randomized into two parallel groups and treated with Venitol(R)(a micronized flavonoidic fraction containing diosmin 450 mg and hesperidin 50 mg) (group 1) or placebo (group 2). Venitol(R) was administered at the dosage of three toblets b.i.d. the first four days and two tablets b.i.d following three days. Postoperative analgesia and laxative prescription as well as hospital stay were same in two groups. Though there is no difference of symptoms at D1, improvement of symptoms of complications was greater in group 1 than in group 2 at D18. The clinical severity of postoperative spot-bleeding, pain and anal discharge diminished in both groups but to a greater extent in group 1 (P<0.005). There was no side-effects in using Venitol(R). In summary, Venitol(R) is effective in reducing complications after hemorrhoidectomy.

We investigated the effect of 2% dorzolamide and 0.12% UF-021 on intraocular pressure and peripapillary and optic nerve head microcirclation with Heidelberg Retina Flowmeter. The 40 eyes of 40 normal subjects were divided 2 groups. 2% Borzolamide and 0.12% UF-021 were instilled in each group. The IOP and microcirculation of peripillary retina and optic nerve head were measured before and 90min, 4, 8, 12, 24 hours after instillation. The IOP was decreased at 90 minutes, 4, 8 hours after instillation of 2% dorzolamide and maximal reduction of IOP was observed at 90 minutes (P<0.05). The IOP was decreased at 90min, 4, 8, 12 hours after instillation of 0.12% UF-021 and maximal reduction of IOP was observed at 4 hours(P<0.05). But, there was no statistically significant changes of microciculation in both groups(P>0.05). We suggest that 2% dorzolamide and 0.12% UF-021 reduce the IOP, but do not alter the mictocirculation of peripapillary retina and optic nerve head.

3.Diagnostic Accuracy in Endoscopic Gastric Biopsy - Part 1. Analysis of preoperative gastric biopsies in 236 cases with neoplastic lesions based on gastretomy and histotopographic investigation.

Two hundreds and forty six preoperative endoseopic gastric biopsies from 236 conaecutive cases were subjected to the retrospective analyais of diagnostic accuracy among 597 tumor- bearing gastrectomy specimens confirmed by extended histotopographic investigations during a period of 1980 to 1981 at Seoul National University Hospital. The overall diagnostic accuracy of endoscopic gastric biopsy was 93.1% with 0.8% false positivity and 3.5% of false negative caaes; of those, the concordance rate of preoperative biopsy diagnosis based on the histotopographic investigation of the gastrectomy remained in 85.5%, whereas 75% of diagnostic discrepancy wae reaulted from inappropriate biopsy sampling or failure of neoplastic tissue obtainability; and the rest of 25% was mostly false negative cases. 5.Incidence of Anti - Ro Antibodies in Patients with Systemic Lupus Crythematosus.

We examined the incidence of anti-Ro antibodies with sera from 60 Korean patients with systemic lupus erythematosus(SLE) by double immunodiffusion. Among these 60 sera tested, 31(51.7%) showed evident immunoprecipitations against Ro antigen on the Ouchterlony plates. Several recent studies indieate that the anti-Ro antibody occurs much less frequently in Occidental lupus patients(around 30%) than in Japanese lupus patients (50%). These data may be suggestive that the racial or ethnic baekground might be an important factor determining the serological, and perhaps the clinical features of SLE.

8.Detection and characterization of excretory/secretory proteins from Toxoplasma gondii by monoclonal antibodies.

Excretory/secretory proteins (ESP) from Toxoplasma gondii were analyzed to define the function in the penetration process into host cells. Whole ESP obtained at 37 degrees C were composed of 15 bands with molecular mass of 110, 97, 86, 80, 70, 60, 54, 42, 40, 36, 30, 28, 26, 22, and 19 kDa. Five ESP of 86, 80, 42, 36, and 28 kDa were reacted with monoclonal antibodies (mAb), named as Tg386 (microneme), Tg485 (surface membrane), Tg786 (rhoptry), Tg378, and Tg556 (both dense granules), respectively. The ESP was released by a temperature-dependent/-independent manner and all at once whenever ready to pour out except Tg786. Each ESP was not exhausted within the parasite but the amount was limited. Tg786 was released continuously with increment, whereas Tg378 and Tg556 were ceased to release after 3 and 4 hr. Dense granular Tg378 and Tg556 were released spontaneously and constitutively before the entry into host cells also. The entry of T. gondii was inhibited by all the mAbs differentially. And the parasite deprived of ESP was inhibited to enter exponentially up to 90.1%. It is suggested that ESP play an essential function to provide appropriate environment for the entry of the parasite into host cells.
