2.A Comparative Study of Autograft and Tutoplast Processed Allograft in Posterolateral Lumbar Spine Fusion.

Fresh autogenous bone is regarded as the most effective biologic resource for repair or reconstruction of the skeletal system. Disadvantages, however, are related to doner site morbidity and include the risk of wound infection, increased blood loss, prolonged anesthesia time, and additional postoperative discomfort. To avoid the disadvantages of fresh autogenous bone graft, we started to use Tutoplast processed allograft to the operation of spinal disorders of elderly patients. The purpose of this article is to evaluate the incorporation ability of the allograft and possibility of it for suitable substitutes for autogenous bone graft. From March 1995 to October 1996, twenty-seven elderly patients at Nowon Eulji hospital received operation for mainly stenotic lumbar spine disorders. Surgical procedures to all patients consisted of massive decompression, posterior instrumentation and bilateral posterolateral fusion and were done by same surgeon. The types of bone graft used for 54 cases of posterolateral lumbar spine fusion included 40 cases of autograft alone, 8 cases of Tutoplast processed allograft alone and 6 cases of a combination of autograft and the allograft. Of the 40 cases using the autograft only, 37 cases(92.5%) showed good bone incorporation and fusion mass formation but all eight cases using only Tutoplast processed allograft showed complete resorption within postoperative three to five months. Of the six cases using mixture of autograft and the allograft, three had incomplete but largely appropriate bone mass formation. There was great statistical difference(p<0.001) between autograft and the allograft in bone incorporation ability and concluded that Tutoplast processed allograft was not appropriate for the purpose of solid bone mass formation on posterolateral lumbar spine fusion.

4.Intraoperative Coagulation Management by TEG in a Patient with Aplastic Anemia: A case report.

This case was an acquired aplastic anemia patient who required a major operation for excision of a mediastinal mass. The authors previewed that the coagulation abnormalities would be developed due to major operation. Thus we decided to monitoring the coagulation function using the thromboelastography during the perioperative period and checked the complete blood count, concommitantly. The total blood volume lost during operation was 1800 ml, so we gave him a transfusion of 10 U's of platelet concentrate, 10 U's of pheretic platelet rich plasma and 5 U's of whole blood. The thromboelastography was a good guide that helped us to avoid excessive treatment of the coagulation abnormalities. We concluded that the thromboelastograhy was a reliable and effective monitoring system at the intraoperative coagulation management.

6.THE ROLE OF REGULATED PROTEINS RELATED TO CELL CYCIE IN CARCINOGENESIS.

The cell cycle is composed of G1, S, G2 and M phase. The transitions between different phases are regulated at checkpoint such as Start(restriction), S phase and mitotic checkpoint. These checkpoints are regulated by specific cyclins and Cdks(cyclin-dependent kinases). Especially, Start checkpoint in late G1 is though to be very important in control of cell cycle. In this study, it was shown various CDKN2(p16ink4A) alteration, including deletions, mutations, down regulations, and performed differential expression of p53, Cdk4, PCNA and pRb in stomach cancer tissues. 1. The frequency of CDKN2 mutations was not observed in the 19 primary stomach cancer tissues. In contrast to the mutations of CDKN2, mRNA levels was showed by Northern blot analysis that expression of CDKN2 was absent or decreased in 10 of the 19(53%) primary stomach adenocarcinoma. Western blot analysis was performed to determine the differential expression of p53, Cdk4, PCNA related to Start checkpoint. Overexpression of p53 was shown 38%, Cdk4 was expressed in all each specimens, and expression of PCNA was not shown. 2. As the other method to determine the differential expression of p53, Cdk4, PCNA and pRb, immunohistochemical analyses were performed on each 14 formalin-fixed and paraffin embedded tumor tissues of stomach adenocarcinoma. p53 overexpression was showed to clear nuclear staining only in tumor cells not in nonneoplastic cells. In staining for cdk4, the tumor was considered to be cdk4 positive if there was nuclear staining in tumor cells, regardless of cytoplasmic staining. PCNA staining for carcinoma tissues showed more intense nuclear staining in tumor cells than in nonneoplastic cells. pRb overexpression was show in tumor cells. Significant differences were observed in the expression of the proteins among the cancers from different anatomic site. Overexpression of adenocarcinomas had high rate of p53(57.1%) and pRb(71.4% ), and low late of cdk4(7.1% ) and PCNA(14.3% ), As these results, deletion of CDKN2 gene in human stomach cancer was not observed but mRNA expression was down regulated in restriction checkpoint, G1 phase. Inactivation of the CDKN2 gene due to hypermethylation may play an important role in development of cancer. And one of the abnormalities in p53, Cdk4, PCNA or pRb function occurs very common in various cancers, especially oral adenocarcinoma, osteosarcoma and squemous cell carcinoma, suggest that components in restriction checkpoint also play an critical role in the carcinogenesis and progression of cancers.

8.A Case of Sparganosis in the Calf.

Sparganosis is a tissue-invading disease caused hy plerocercoid of Spirometra mansoni91. Usually, uncooked snakes or flogs and infected water are primary sources of sparganosis. Sparganosis found in the lower extremities, abdominal wall, breast, inguinal region and scrotum. Simple radiographs show linear or elongated calcification. Serodiagnostic tests have also heen used widely. Confirmative diagnosis of sparganosis is made by surgical removal of the worm. We experienced a case of sparganosis in a 48-year old male in the calf. We report a case and review related articles hriefly.

9.The Result of Compression-Plate Fixation in Forearm Fractures

We meet much difficult problem to solve in the treatment of the forearm fractures which are not encountered in the treatment of fractures of the other long bone and there are many reports on the results of treatment of forearm fractures and many methods have been introduced. In the late 1950's ASIF compression plate was invented and developed by Muller, Allgower, and Willenegger and it has shown excellent union rate and functional results in the treatment of forearm fractures. From Jan. 1971 to Dec. 1979, we have experienced 71 cases of fresh or old forearm fractures treated by different methods and devices and among them, 53 patients, those were treated with compression plate and other various internal fixation devices, were possible to trace for more than 3 months. The Author divided the traceable patients into two groups, the one was the group treated with compression plate and the othtr one was the group treated with other various internal fixations, and compared the results in the aspect of healing time and functional results. The results were as follows; 1. Among 71 patients, 45 patients (36.6%) were fresh and 26 were old cases. Of 61 adult patients 21 cases (34.4%) exhibited severe soft tissue injury due to crushing machinary injury. 2. There was 24 (33.8%) cases of open fractures and the most common fracture site was middle one-third of both radius and ulna. 3. The period between operation and exercise was 7.3 weeks in fresh cases which were treated with compression-plate fixation and 10.3 weeks in old cases with other fixation devices. 4. The time of radlological union was comparatively rapid in compression-plate fixation group, acute cases and radial fractures when compared it with those treated with other fixation devices, old cases and ulnar fractures, respectively. Radiological union time in average was as follows; Radius, compression-plate fixation: 12.1 weeks ulna, compression-plate fixation: 12.4 weeks redius, other fixation devices 14.9 weeks ulna, other fixation devices 15.5 weeks 5. By Anderson's functional criteria, the ratio of excellent or good results was as follows, Acute compression-plate fixation: 87% Acute, other fixation devices 67% Old, Compression-plate flxation: 67% Old, Other fixation devices 23% 6. Achieved bony union in all cases in compression-plate fixation group and experienced 3 cases of non-union in the group treated with other fixation devices. Among 3, two cases of non-union were due to post-operative infection and technical failure and the other one was a solitary ulnar fracture which was treated with rush pin.

10.Extraskeletal Neoplasm Resembling Ewing's Sarcoma: Case Report

Ewing's sarcoma had never been described as a primary tumor outside bone, although other malignant skeletal tumors, such as osteogenic sarcoma & chondrosarcoma, are known to arise from extraskeletal soft tissues. In 1975, Angervall & F.M. Enzinger reported 39 cases of small, round or oval cell sarcomas occuring in the soft tissues and considered histologically indistinguishable from Ewing's sarcoma of bone. Recently, We experienced one case of extraskeletal neoplasm resembling Ewing's sarcoma of bone which it was located deeply in the calf area of young female patient and the case review has been followed until the death, approxlmately 10.5 months after removal.
