1.The Kawasaki disease in the National Institute of Pediatry

33 patients with the Kawasaki disease diagnosed in the National Institute of Pediatry, with ages of 3-4.5 (male: 22; female: 11) during 2/1995- 4/2000. The results have shown that this was not a rarely disease in our country, current difficult to diagnose. The symptoms comprised pyrexia over 4 days, rash, red conjunctivitis, periostomatous change or the nodules in the maxillary should be attributed to the Kawasaki disease. The cardiovascular problem were comon in the progress of the disease: The coronary lesion: 12/30; coronary aneurysm 23.3%. There is no complication of the coronary aneurysm in patients who were administrated the gamaglobulin within the 2 weeks of the disease

2.Doppler echocardiography in diagnosis of valvular open in the rheumatism without clinical carditis

A study on 51 patients with rheumatism of the heart and without clinical carditis, ages of 5-15 in the Institute of Pediatry during 1996-2000 has shown that 23 cases (45,1%) had symptoms of pathological valvular open by Doppler of which mitral open in the 3 cardiac valves. The extend of valvular open reduced when disease was stable. The Doppler can detect carditis without clinical symptoms due to rheumatism of the heart.

6.Cervical Prevertebral Hematoma - a Rare Complication of Acupuncture Therapy: A Case Report.

A 36-year old man with neck pain developed an acute cervical prevertebral hematoma after acupuncture therapy at an oriental medicine hospital. MR imaging demonstrated a fluid collection, and this suggested a diagnosis of retropharyngeal hematoma, and the patient was managed conservatively. We report here on a case of a retropharyngeal hematoma following acupuncture therapy and we review the relevant literature.

7.Studies on Total Exchangeable Sodium and Plasma Volume in Hypertension.

Total exchangeable sodium and plasma volume were measured to evaluate role of sodium in hypertension by radioisotope 24 Na and Evans blue in 25 patients with essential hypertension, 10 with malignant hypertension, 5 with renal hypertension and 15 normal controls. The results obtained are as follows. 1. The total exchangeable sodium content was 35.8+/-3.5mEq/kg in normal controls, 35.6+/-2.7mEq/kg in essential hypertension and 36.2+/-3.5mEq/kg in renal hypertension revealing no statistical significance in difference. In malignant hypertension, it was 44.9+/-2.0mEq/kg and it was markedly elevated than in normal controls. 2. Plasma volume in normal controls was 44.0+/-4.7ml/kg. In essential hypertension there was two groups, one (group II, 35.2+/-5.6ml/kg) was slightly lower than the other (group I, 43.2+/-4.8ml/kg). In malignant hypertension, it was 56.9+/-7.5ml/kg revealing significant increase than in normal controls. 3. Changes of the total exchangeable sodium content was observed after the administration of hydrochlorothiazide 50mg in normal controls and essential hypertension. In normal controls there was no statistical significance in reduction. In essential hypertension, initially low salt group (group II) showed no significant reduction and the other group (group I) showed significant reduction with depression on blood pressure. 4. Long term therapy with hydrochlorothiazide 50mg for 2~3 months in the group of hypertension who responded well with short term therapy (group I), the contents of exchangeable sodium reduced significantly than normal controls and the antihypertensive effect was sustained. 5. The reduction of plasma volume with hydrochlorothiazide in essential hypertension are similar as changes of total exchangeable sodium. 6. In cases of low salt diet 4gm of daily salt intake, there was similar results as diuretic therapy in exchangeable sodium. 7. In patients who responded well to antihypertensive effect of hydrochlorothiazide or low salt diet (group I), salt loading of 10gm daily showed an increase of total exchangeable sodium and an elevation of blood pressure with statistical significance.

9.The Treatment of Open Tibial Fractures by Ender's Nailing

The treatment of open tibial fracture has become one of the most controversial subjects in orthopedic surgery. Recently flexible intramedullary nails have been used successfully in treatment of Type I, II open fractures of the tibial shaft. In this paper we are reporting our experience with the method in 20 open fractures of the tibia in 18 patients. The results were ss follows. 1. The patients were predominantly more common in male(90%) and the average age was 42.7 years old and the most common cause of fracture was traffic accident. 2. The most common site and shape were at midshaft and comminuted fracture. 3. With early operation the incidence of infection was greatly reduced and wound healing was relatively rapid. 4. The patients were applied with long leg or P.T.B. cast for at least 4 weeks in order to the prevention of re-displacement. 5. The average healing time was 17.7 weeks in 14 cases and union was most rapid at upper third and Type I fractures. 6. The complication was developed in 14 cases, 3 cases of delayed union, nonunion and ankle stiffness was seen respectively. 7. Infection rate was greatly reduced because Ender's nailing procedure was easy to perform, operation time was shortened, good fixation and alignment of fracture fragment was done without periosteal stripping. 8. Ender's nailing is considered to be a good method in the treatment of open tibial fracture associated with soft tissue injurv and multiole iniuries.
