2.Update on Medications in Schizophrenia

Antipsychotics are a pharmacologically heterogeneous group of compounds, but all act as D2 dopamine receptor antagonists, an action linked to their antipsychotic effect. Today, sixty years on since 1952, we have the FGAs and the SGAs. These medications continue to be useful, and continue to have some troubling adverse effects. As a class, the FGAs are more likely to be associated with EPS but this is primarily true of medications that bind tightly with D2 neuroreceptors, such as haloperidol, and less true of medications that bind weakly, such as chlorpromazine. Anticholinergic effects are especially prominent with weaker-binding FGAs, as well as with the SGA clozapine. As a class, the SGAs, especially clozapine and olanzapine generally tend to cause more problems relating to the metabolic syndrome, such as obesity and type 2 diabetes mellitus. All antipsychotic medications are associated with an increased likelihood of sedation, sexual dysfunction, postural hypotension, prolonged QT interval and sudden death. Primary care physicians need to be familiar with the individual adverse effect profiles of these medications.
3.Effects of the Reticuloendothelial System on the Formation of Nonspecific Reactive Hepatitis in Rats.

This study was designed to evaluate the effects of altered reticuloendothelial function on the formation of nospecific reactive hepatitis in rats. Increased activity of reticuloendothelial system was evoked by methylcellulose treatment (20 mg/100 g). Partial depression of reticuloendothelial system was induced by splenectomy. Experimental nospecific reactive hepatitis was made by Escherichia coli treatment, artificial stomach perforation ileum ligation, respectively. The nonspecific reactive hepatitis thus made evaluated by liver function tests such as AST ALT and alkaline phosphatase. Histological and electron microscopical observation were also done. The results obtained were as follows: 1. Functional and histological changes of nospecific reactive hepatitis deffered with the disign employed. 2. Partial reduction of reticuloendothelial activity by splenectomy didn't influence the serverity of nonspecific reactive hepatitis. 3. Increase of reticuloendothelial activity by methylcellulose treatmetns reduced the severity of nonspecific reactive hepatitis lesion developed by E. coli treatment and stomach perforation, both functionally and morphologically. 4. Changes of nonspecific reactive hepatitis by lieum ligations, however, were aggrevated by increased recticuloendothelial activity. These results showed the divers effect of altered reticuloendothelial function on the degree of nonspecific reactive hepatitis according to the employed experimental designs.
4.Multiple Subcutaneous Granuloma Pyogenicum Combined with Cherry Angioma.

A 51-year-old man had multiple nonspecific subcutaneous nodules and small red hemorrhagic papules on his trunk. Histopathologically, the subcutaneous nodules were shown to be lobular capillary hemangiomas with edematous stroma throughout the entire subcutaneous fat. Multiple subcutaneous granuloma pyogenicum is a rare variant of granuloma pyogenicum. The addition of cherry angioma in the same patient also makes it a very unusual case.

6.The Study of Changes in Arterial Blood Gas Finding with Time Interval.

BACKGROUND: Arterial blood gas analysis are highly susceptible to preanalytic error due to improper method of obtaining or handling the blood sample before analysis. The error in measurement of blood gas analysis are loss of CO2 by exposure to atmospheric air, effect of anticoagulant itself, temperature difference between the experimental subject and the measuring electrode and metabolic change which occur between blood sampling and measurement. METHOD: To study the effect of the delay in estimation of blood gas and drawn blood on values of blood gas partial pressure and pH. Blood sample were divided into 2 groups according to the method of storage, group I stored at 24~25degrees C(room temperature) under anaerobic condition. ;group II stored at 0~4degrees C(refrigerator) under anaerobic condition. The samples were analyzed by time interval through 180 minutes in each group. RESULTS: The result were as follows: 1) PaO2 decreased significantly after 10 mins in group I, whereas it decreased significantly after 20 mins in group II. 2) PaO2 increased significantly after 20 mins in group I, whereas it increased significantly after 120 mins in group II. 3) pH decreased significantly after 60 mins in group I, whereas it decreased significantly after 120 mins in group II. 4) No significant changes of bicarbonate and SaO2 were noted in each group CONCLUSION: From above results, it would be advisable to analyze the sample in a short period of time or to store in a refrigerator when the measuring will be delayed. So we highly recommend that blood gas analysis should be performed as soon as possible after sampling, especially within 10 minutes.

7.Alterations of Cerebral Blood Flow and Cerebrovascular Reserve in Patients with Chronic Traumatic Brain Injury Accompanying Deteriorated Intelligence.

PURPOSE: The purpose of this study was to evaluate alterations of regional cerebral blood flow (CBF) and cerebrovascular reserve (CVR), and correlation between these alternations and cognitive dysfunction in patients with chronic traumatic brain injury (TRI) and normal brain MRI findings. MATERIALS AND METHODS: Thirty TBI patients and 19 healthy volunteers underwen1 rest/acerazolamide brain SPECT using Tc-99m HMPAO. Korean-Wechsler Adult Intelligence scale test was also performed in the patient group. Statistical analysis was performed with statistical parametric mapping software (SPM97), RESULTS: CBF was diminished in the left hemisphere including Wernicke's area in all patients with lower verbal scale scores. In addition, a reduction in CBF in the right frontal, temporal and parietal cortices was related with depressed scores in information, digital span, arithmetic and similarities, In patients with lower performance scale scores. CBF was mainly diminished in the right hemisphere including superior temporal and supramarginal gyri, premotor, primary somatomotor and a port of prefrontal cortices, left frontal lobe and supramarginal gyrus. CVR was diminished in sixty-four Brodmann's areas compared to control. A reduction in CVR was demonstrated bilaterally in the frontal and temporal Iobes in patients with lower scores in both verbal and performance tests, and in addition, both inferior parietal and occipital lobes in information subset. CONCLUSION: Alterations of CBF and CVR were demonstrated in the symptomatic TBI patients with normal MRI finding. These alterations were correlated with the change of intelligence, of which the complex functions are subserved by multiple interconnected cortical structures.

The myoepithelioma is a rare salivary gland tumor composed nearly exclusively of myoepithelial cells. A case occuring on the parotid gland of a 51 year-old female is reported. Light microscopy finding revealed plasmacytoid cells. Ultrastructurally, the most of the neoplastic cells had polygonal outlines and showed eccentric, round to ovoid nuclei with even contours, peripherally distributed heterochromatin and to three small nuceoli. Their cytoplasms were almost completly filled with thin filaments, arranged in parallel strains and exhibiting "dense bodies". The neoplastic cells showed positive immunostaining for S-100 protein and cytokeratin.

10.Intrahepatic Arterioportal Shunt:A Mechanism of Hypovascular Hepatocellular Carcinoma.

PURPOSE: To prove whether the arterioportal shunt, especially transvasal shunt is one of the cause of the hypovascular hepatocellular carcinoma. MATERIALS AND METHODS: We evaluated the early phase images of table incremental dynamic CT and hepatic angiography in 20 cases of hepatoceltular carcinomas with transvasal arterioportal shunt. RESULTS: In hepatic arteriography, 18 cases were hypovascular and the remained 2 cases showed hypervascular tumor staining than surrounding normal hepatic parenchyme. In the early phase dynamic CT, 18 cases were hypodense(including 4 cases of focal hyperdensity in hypodense background), one was isodense and remaining one was hyperdense. CONCLUSION: Arterioportal shunt, especially transvasal shunt may make originally hypervasular hepato-cellular carcinoma to hypovascular lesion in the early phase dynamic CT or hepatic arteriography. In attempt to differentiate hepatic masses by tumor vascularity in recently widely used table incremental dynamic CT, the vascular patterns of the mass should be considered by close evaluation of vascular pattern of the liver, such as morphology of perfusion abnormality and arterioportal shunt, etc.
