4.DNA Sequencing of p53 Gene Mutation in Colorectal Carcinomas.

Mutations in the p53 gene occur during the development of colorectal carcinomas, and play an important role in the conversion of adenoma into carcinoma. To detect the p53 gene mutation and its pattern of expression in colorectal carcinomas, polymerase chain reaction for exons 5, 6, 7, and 8, recombinant gene cloning, and automated DNA sequencing were performed with 30 fresh colorectal carcinomas. Each tissue was also analyzed by immunohistochemical staining for p53 protein. p53 protein was detected in 25 of 30 (83.3%) colorectal carcinomas by immunohistochemical study. p53 mutation was detected in 4 of 30 (13.3%) colorectal carcinomas. The distribution of these mutations among these exons investigated was as follows: Three mutations in exon 5 (66.7%) and 1 mutation in exon 7 (33.3%). One case with mutation in exon 5 had mutations at three different codons. Mutations in exon 5 were found at codon 153 (GGG to AGG: Gly to Arg), 170 (TGC to GGC: Cys to Gly), 186 (CTA to TTA: silent mutation), 158 (GCG to ACG: Ala to Thr), and 176 (ACG to ATG: Thr to Met). Mutation in exon 7 was found at codon 248 (AGG to AGA: silent mutation). Four of them were missense mutations. Two of 6 mutations were silent mutations. Five transition mutations and 1 transversion mutation were also detected. All cases with mutations by automated DNA sequencing showed positive p53 protein immunohistochemical stainining. In conclusion, p53 gene mutation was detected in 4 of 30 (13.3%) colorectal carcinomas, located in codon 153, 158, 170, 176, and 186 of exon 5 and codon 248 of exon 7. Further studies are needed to evaluate the significance of the codon 153 mutation which was not recognized in other studies on colorectal carcinomas.

5.Effect of a Diabetic Camp Program on the Fasting Blood Sugar Level in Type 2 Diabetic Patients.

This study was conducted to test the effect of a diabetic camp program on the fasting blood sugar in type 2 diabetic patients. The subjects of the study consisted of 33 diabetic patients who had participated with a diabetic camp program in the YangPung area from August 12 to August 15, 1998. Wilcoxon signed rank test was used for the significance of the differences between values before and after the diabetic camp program. The fasting blood glucose level, before of the closing day at the diabetic camp, was lower than that of the opening day. When comparing the blood glucose levels before meals, between the early and later part of the diabetic camp, fasting blood glucose levels before breakfast by the 4th day in the later part of the diabetic camp were lower than those of the 2nd day in the early part. Blood glucose level before lunch by the 4th day, in the later part of the diabetic camp, were lower than those of the 2nd day in the early part. Blood glucose levels before dinner by the 3rd day, in the later part of the diabetic camp, were lower than those of the 1st day in the early part. Blood glucose levels before sleep by the 3rd day, in the later part of the diabetic camp, were lower than those of the 1st day in the early part.

6.Expression of Oncogene Product in the Colorectal Carcinoma.

The expression of p62"' and p2l can be seen in many solid tumor, but the pattern and incidence of expression were different according to organ, countries, and examiners, thus it is not definitely defined. Total 67 colorectal carcinoma in paraffin sections are analysed by immunohistochemically for evaluation of the p62and p21- expression according to the age, sex, chief complaints, location, differentiation, modified Dukes stage, using the specific monoclonal antibodies. The results were summarized as follows : The age of patients ranged from 32 years to 82 years. The mean age was 57.6 years. The expression of p 62y' and p21 was not correlated with age. Male was 29 cases(43.3%) and female was 38 cases(56.7%). The male to female ratio was 1:1.31. The and 95.0% in moderately differentiated adenocarcinoma, 90.0% and 90.0% in poorly differentiated <,;, adenocarcmoma, 100.0% and 100.0% in mucinous carcinoma. The positive rate of p62!and and p2l' was 94.1% and 88.2% in Dukes stage B,, 96.0% and 96.0% in Dukes stage B,, 100.0% and 100.0% in Dukes stage C,, 100.0% and 88.9% in Dukes stage C, and 100.0% and 100. o% in Dukes stage D. The expression of p62 y' in metastatic colorectal carcinoma showed diffuse and strongly positive reaction than primary colorectal carcinoma. The expression of p21 in primary colorectal carcinoma showed diffuse and strongly positive reaction than metastatic colorectal carcinoma.

7.A Case of Complete Resolution of Aortic Dissection in the Descending Thoracic Aorta Treated with Endovascular Stent-Graft Implantation.

The aortic dissection is an acute aortic syndrome, caused by an intimal tear and subsequent splitting of the media by the pulsatile blood flow. Though there would be differences in the origin of aortic dissection and therapeutic modalities, the intermediate and long-term prognoses are poor. Endovascular Stent-graft implantation is a revolutionary technique in the treatment of aortic dissection. The endovascular stent grafting in aortic dissection is less invasive and feasible method and is an effective tool for closing the entry site and promoting clot formation, reducing the size of the false lumen. Therefore, endovascular Stent-graft implantation makes possible the desirable remodelling of aorta. We report 33 year-old male with aortic dissection in the thoracic aorta, which was treated with endovascular Stent-graft implantation. Over the favorable remodelling, his dissection was healed completely by the endovascular treatment using Stent-graft.

8.Current Status of Research on Liver Cancer in Korea.

Liver cancer is one of the leading causes of cancer-related death in Korea. Liver cancer imposes a considerable societal burden due to its high incidence and high mortality rate in younger patients, as compared to other cancers. However, interest in liver cancer among researchers and health-policy makers is low. In this review, recent trends in the number of published articles on liver cancer in Korea and internationally were analyzed. The key finding is that the rate of growth in the number of published articles on liver cancer is slowly decreasing and financial investment for research into liver cancer is very limited, despite the increasing research and development investment budget in Korea. Meanwhile, the rate of growth of research into liver cancer in China has recently increased markedly. Therefore, the scale and rate of growth of research into liver cancer in Korea should be enhanced.

9.Effect of Varicocelectomy on Spermatogenesis.

A total of 37 randomized patients with left sided varicocele were investigated for the past 5 years to evaluate the effects of varicocelectomy on semen quality and fecundity. They were 14% of infertile males who hospitalized for surgical treatment except vasectomy reversal cases. According to severity of varicocele, patients were divided into three grades such as Grade I (Valsalva positive, consisted of 8 cases). Grade II (palpable, consisted of 18 cases) and Grade III (visible, consisted of 11 cases). Their, mean age was 29 years(20-40 years) (Table 1), and duration of marital life, 3 years(1-15 years). Mean levels of plasma FSH(6. 39 IU/l), LH(6. 25 IU/I) and testosterone (6. 97 ng/ml) were within normal ranges. There were no significant differences in hormonal levels among the grades of varicocele (Table 2), Oligospermias and normospermias were distributed in these grades of varicoceles almost the same extent. This means that the size or grade of varicocele was not closely correlated with degree of spermatogenic failure. Spermiogram of total patients was as follows: mean volume. 2. 9 ml; motility, 34%; normal shape, 68%; and azoospermia in 3 cases; counts of less than 20 X 10`/ml in 21 cases; and more than 20 x 10 6/ml in 13 cases with mean count of 26 x 10 6/ml(Tables 3 and 4). Of these, 6 subjects with sperm counts of more than 41 x10 6/ml were submitted to control series without surgery and the remaining 31 patients with sperm counts of less than 40 x 10 6/ml underwent varicocelectomy by high ligation of internal spermatic vein. After the operation, sperm appeared in 1 of the 3 azoospermia. Numbers of patients with sperm counts of less than 20 x 10 6/ml decreased from 21 to 19 cases, and those with more than 21 x 10 6/ml increased from 7 to 10 cases following the surgery. No significant changes in volume, motility and morphology could be demonstrated after varicocelectomy(Tables 5. 6 and 7). When judging by tentative criteria of fertility unit (total sperm counts( 10 6) x motility(%) x morphology(%) / 10 6) of more than 1 unit, improvement was observed in only 9 patients(29%) after the operation(Table 5). Three conceptions (10%) occurred in those submitted to varicocelectomy in 1 to 36 months after the operation and 1 pregnancy(17%) resulted in control series. In the present series of patients with varicocele, no significant beneficial effects were observed on semen quality and pregnancy rate after varicocelectomy(Tables 5 and 6).

10.Drying time of tray adhesive for adequate tensile bond strength between polyvinylsiloxane impression and tray resin material.

STATEMENT OF PROBLEM: Use of custom tray and tray adhesive is clinically recommended for elastomeric impression material. However there is not clear mention of drying time of tray adhesive in achieving appropriate bonding strength of tray material and impression material. PURPOSE: This study is to investigate an appropriate drying time of tray adhesives by evaluating tensile bonding strength between two types of polyvinylsiloxane impression materials and resin tray, according to various drying time intervals of tray adhesives, and with different manufacturing company combination of impression material and tray adhesive. MATERIAL AND METHODS: Adhesives used in this study were Silfix (Dentsply Caulk, Milford, Del, USA) and VPS Tray Adhesive (3M ESPE, Seefeld, Germany) and impression materials were Aquasil Ultra (monophase regular set, Dentsply Caulk, Milford, Del, USA) and Imprint II Garant (regular body, 3M ESPE, Seefeld, Germany). They were used combinations from the same manufacture and exchanged combinations of the two. The drying time was designed to air dry, 5 minutes, 10 minutes, 15 minutes, 20 minutes, and 25 minutes. Total 240 of test specimens were prepared by auto-polymerizing tray material (Instant Tray Mix, Lang, Wheeling, Il, USA) with 10 specimens in each group. The specimens were placed in the Universal Testing machine (Instron, model 3366, Instron Corp, University avenue, Nowood, MA, USA) to perform the tensile test (cross head speed 5 mm/min). The statistically efficient drying time was evaluated through ANOVA and Scheffe test. All the tests were performed at 95% confidence level. RESULTS: The results revealed that at least 10 minutes is needed for Silfix-Aquasil, and 15 minutes for VPS Tray Adhesive-Imprint II, to attain an appropriate tensile bonding strength. VPS Tray Adhesive-Imprint II had a superior tensile bonding strength when compared to Silfix-Aquasil over 15 minutes. Silfix-Aquasil had a superior bonding strength to VPS Tray Adhesive-Aquasil, and VPS Tray Adhesive-Imprint II had a superior tensile bonding strength to Silfix-Imprint II at all drying periods. CONCLUSION: Significant increase in tensile bonding strength with Silfix-Aquasil and VPS Tray adhesive-Imprint II combination until 10 and 15 minutes respectively. Tray adhesive-impression material combination from the same company presented higher tensile bonding strength at all drying time intervals than when using tray adhesive-impression material of different manufactures.
