4.A Case of Penile Carcinoma Transformed from Giant Condyloma Acuminata.

Giant condyloma and carcinoma-like condyloma is relatively rare. Although it has histological similarities to condyloma acuminata, it is believed by most authorities to be a clearly defined variant of epidermoid carcinoma. Clinically it is easily mistaken for the more aggressive metastasing epidermoid carcinoma. The legion was first described by Buschke in the German literature in 1896. as a varient of condyloma acuminata. A case of malignancy of the penis which have apparently been transformed from giant condyloma acuminata of the penis are herein presented with review of literature.

5.Clinical Analysis of Common Peroneal Nerve Repair

Common peroneal nerve injury has been reported as the most frequent lower extremity peripheral nerve injury. The peroneal nerve may be particulary prone to direct or indirect injury by penetrating trauma or external compression by it's superficial route to bony prominence of fibular neck. By the development of microsurgical technique, recently epineural repair, fascicular group repair, and interfascicular graft were tried. The authors have experienced five cases microsurgical repair of common peroneal nerve injury around the knee joint from December 1990 to November 1991, and motor and sensory result was relatively good.

7.Office Blood Pressure is Higher than Home Blood Pressure, and Digital Electronic Sphygmomanometer is Useful for Self-Monitoring of Blood Pressure in Hypertensive Patients.

BACKGROUND: To investigate the possibie difference, if any, between office blood pressure(BP) and home BP may be important in the diagnosis and treatment of hypersensive patients. This report deails the difference between the two BP's and the usefulness of digital electronic sphygmomanometer(DES) for self-monitoring of home BP. METHODS: The BP's of 14 patients with essential hypertension were measured with mercury sphygmomanometers at outpatient department by physician and with DES at home(twice a day) by the patients. Patients were followed up every 2 weeks for 4 weeks and previous 2 weeks' average home BP's were compared with the office BP's of each 2 weeks' end. RESULTS: There was a significant difference between office and home BP(both systolic and diastolic) ; office average BP(151/95mmHg) was higher than home average BP(136/86mmHg). CONCLUSION: Caution may be needed in the interpretation of office BP unless it is measured several times after adequate rest.

8.Growth Hormone Non-dependent Insulin-like Growth Factor-I of Kidney.

Recent studies have revealed that IGF- I produced in kidney are of two fractions; GH dependent and GH nondependent IGF-I. The role of GH nondependent IGF-I is interesting in renal hypertrophy and glomerulosclerosis because GH is clearly related with hypertrophy accompanying glomerulosclerosis is not seen in GH deficient animal. The relationship of the high protein diet and the IGF- I production under the deprivation of GH was studied. In hypophysectomized Sprague-Dawley rat, the level of serum IGF-I was measured using radioimmunoassay, and renal IGF- I production evaluated by immunohistochemistry during both normal and high protein diet. Serum IGF-I of rats on high protein diet was significantly higher than that fed normal protein diet. After unilateral nephrectomy, the level of serum IGF-I was significantly increased in both normal and high protein diet groups. Henle's loop, distal convoluted tubule and collecting duct were weakly stained with normal protein diet. With high protein diet, the staining intensities increased at these portion , and distal part of proximal convoluted tubule and straight tubule were weakly stained. After unilateral nephrectomy, distal convoluted tubule and collecting duct were densely stained with normal protein diet. With high protein diet, the staining intensities increased in distal part of proximal convoluted tubule and Henle's loop. Regardless of the types of protein diet, the specific difference between unilateral nephrectomized rats and sham- operated rats was immunoreactivity of the distal convoluted tubule.. In conclusion, it is suggested that GH non-dependent IGF-I is mainly produced in distal convoluted tubule during compensatory renal hypertrophy, and protein diet mainly affect IGF- I production of distal part of proximal convoluted tubule and Henle's loop.

9.MR evaluation of visceroatrial situs abnormality.

Thirteen patients with visceroatrial situs abnormalities were evaluated by magnetic resonance(MR) imaging. Eleven patients were confirmed surgically. Two patitnts were diagnosed by MRI and cardiac catheterization. Right isomerism was found in seven patients, left isomerism in two, and situs inversus in four. For the determination of situs, we evaluated the morphology of atrial appendages and main bronchi, the relationship between abdominal aorta and inferior vena cava(IVC), and the status of upper abdominal viscera. The bilateral atrial morphology was differentiated in 8 of 12 patients. The bronchial situs was determined in 11 of 12 patients. Juxtaposition of abdominal aorta and IVC was found in 6 of 7 with right isomerism. IVC interruption with azygos continuation was found in all two with left isomerism. Incidentally three cases of short pancreas were found. MR imaging showed all structures relevant for the assessment of situs, thus obvrating the need for performing additional diagnostic procedures. MR imaging, therefore, is a valuable tool in the clinical management of patients who are suspected of having a situs abnormality.
