3.A Study on Patent Ductus Arteriosus in premature Infants.

A Prospective study was done about incidence and treatment of hemodynamically significant' patent ductus arteriosus in premature infants admitted to neonatal intensive care unit of Ewha University Hospital from Feb. 1984 for one year. The results were as follows; 1) Among 46 infants with birth weight less than 2,000gm and gestational period 36 wks, 15(32.6%) developed a hemodynamically significant PDA. 2) The incidence of respiratory distress syndrome was six times higher in PDA group than that of non-PDA group(73.3% vs 12.9%). 3) The need for ventilatory assistance was five times higher in PDA group than that of non-PDA group(80% vs 16.1%). 4) The mortality rate was 40% in PDA group, 15% in non-PDA group. 5) The ratio of left atrium to aorta(LA/AO) in M-mode echocardiography was significantly higher in PDA group than that of non-PDA group(1.21+/-0.18 vs 0.93+/-0.23). 6) Indomethacin treated with usual medical therapy resulted in ductal closure in 80%, 20% with usual medical therapy only. 7) The mean LA/AO ratio was significantly decreased after indomethacin treatment(1.15+/-0.13 vs 1.21+/-0.18). 8) The change of the value of plasma sodium and potassium was not significant during indomethacin therapy, whereas BUN, creatinine value was increased during treatment, but that was reversible.

4.Measurement of Cognitive Functions of Elderly.

To examine the cognitive function of elderly, the study examined the performance of Korean normal elderly population whose age over 65 using neuropsychological instruments. It was predicted that the performance of the Korean population would be different from the U, S. mainly due to their difference in language, culture, and education. Korean elderly people from the Chungchung and Daejeon Metropolitan city(n=97) participated. Two age scores were developed: below 74 years and over 75 years. The effect of age, gender and education was examined, which yield significant age, gender and education effect. The score of DSF, DSB, TMTA, and TMTB are expected to be utilized for research purposes, such as basic, clinical, epidemiological studies, as well as practice purposes such as diagnosis and assessment of the progression of cognitive decline and dementia with MMSE-K.

5.Monocular Axial Myopia following Neonatal Eyelid Suture in The Rabbits.

The exact etiology and pathogenesis of myopia are not known yet, although various studies of the development of myopia in experimental animals have been made. Authors attemtped whether rabbits developed axial myopia by fusing their lids at birth in one eye, or not. The monocular vision was deprived by suture of upper and lower lids in 12 rabbits which were 5 day-old. The other eye was unsutured as the control. Ten months later, we assessed refractive error, corneal curvature, axial length, and intraocular pressure and enucleation was done. We observed the histologic change of sclera with light microscope. The eyes with lid suture were more myopic(-3.35 +/- 0.99D) than the controlled eyes(-0.21 +/- 0.46D) (p<0.01). The corneal curvature and intraocular pressure were not significantly different between two groups. The axial lengths of the sutured eves were increased(21.27 +/- 1.70mm) in comparison with those of the controlled(17.39 +/- 1.83mm)(p<0.01). We conclude the difference in axial length was caused by the elongation of the posterior segment of eyeball. since lens thickness, depth of anterior chamber, and corneal curvature were identical in both groups. These were correlated with histologic change that showed thinner posterior parts of sclera of the sutured eyes than those of the controlled eyes. The results of this study demonstrate that monocular lid fusion in rabbits produced an expenmental axial myopia.

6.Radiologic analysis & diagnostic value of lateral tomography on ossification of posterior longitudinal ligamentof c-spine

The ossification of posterior longitudinal ligament of the spine(OPLL) is newly recongnized clinical entity,although compression of the spinal cord by an OPLL was reported by key as early as in 1839 in Guy's HospitalReport. OPLL was noticeable in lateral tomography as an abnormal dense radiopacity along the posterior margins ofthe vertebral body. Authors retrospectively analysed the diagnostic values and findings of lateral tomography ofthe cervical spine in 11 cases at Kang Nam General Hospital Public Corporation during 1 yr from July 1984 to June1985. The results were as follows; 1. Among suspected 11 cases of OPLL, 9 cases were confiremd as OPLL on lateraltomogram. 2. Age range was 25 years old to 55 years old and more prevalent age was over 5th decades & male wasmore involved than female. 3. Frequent involvement was C2-C5 level and number of vertebral bodies involved was 3.6in average. 4. This ossification developed 4 modes, a continuous type 11%, segmental 33%, mixed type 33%,circumscribed type in 22%. 5. OPLL thickness were from 2mm to 4.5mm and spnal canal narrowing ratio were form 25%to 44% and there were norational relationships between clinical symptom and thicknness of OPLL. 6. On diagnosis ofOPLL, lateral tomography is accurate and recommendable screening study due to easy, noninvasive, indisipensable and less harmful technique, compared to those of myelography or computed tomography.

7.The Effects of the Nurses' Knowledge to Breast-Feeding on the Nursing Activities for Breast-Feeding.

This study was attempted to identify the knowledge to breast-feeding and the performance of nursing activities for breast-feeding and to test "how does the degree of knowledge to breast-feeding influence the nursing activities for breast-feeding?" The subjects were 180 nurses working in delivery rooms, nursery, obstetrics & pediatrics wards or OPD of obstetrics & gynecology of 8 general hospitals in Pusan as of August 3 through 13, 1996. The results are abstracted as follows : 1) Subject nurses' age : 25-29 was 45.6%(the major), education levels ; graduates from junior college were 95.0%, unmarried status was 62.2%, 76.5% of married nurses had children, 39.7% in-service education for Breast-Feeding, 337% did nursing activities for breast-feeding actively, the reason for inadequate activities for breast-feeding was "too much other tasks." 2) The degree of knowledge to breast-feeding ; mean score was 13.54, the degree of performance of nursing activities for breast-feeding ; 92.38+/-20.93 points out of possible 145 points(3.19+/-.74 out of possible 5 points) meaned that it was a low level. 3) The hypothesis "the nurses who have higher degrees of knowledge to breast-feeding will show higher degrees of performance of nursing activities for breast-feeding than the nurses who have lowe degrees of knowledge to breast-feeding" was tested by t-test(t=-.01, P=.9888), but rejected because it turned out statistically not significant at the level of P<.05) Above results suggested the degrees of knowledge to breast-feeding and the degrees of performance of nursing activities for breast-feeding were generally low and the degree of knowledge didn't influence the nursing activities. Researchers believe that the education for breast-feeding by the nurses need to be performed systemically & practically and new-method of breast-feeding education program need to include hospital managers as well as nurses related mothers and their family. In addition, researchers propose the introduction of "lactation specialist system", for the specialist can change the attitude of feeding-mothers positively with their specialty and authority.

8.Photochemical Treatment Utilizing Mercury Out-door Illumination Lamp.

In previous paper we reported on the radiation spectrum of commercially ava ilable out-door illumination lamp and suggested the use of this lamp as long ultraviolet light source until such time we have adquate medical equipments availahle. We tried to dectermine the erythema dose of above lamps in conjunction with topical psoralen and tar. The skin lamp distance was kept 50cm. The test sites were the backs of 30 young volunters. 200 watt bulbs were used. l. 30 minutes topical application of 0.25% Meladoxen solution required 2 to 6 minutes of exposure to inducs erythema in 48 hours. 2. 6 hours of topical tar application needed 2 to 8 minutes of exposure. 3. With 0.25% Meladoxen solution; 30 minutes of topical application produced more phototoxic effect tnan 2 or 24 hours. 4. With 5% Tar ointment; the longer the duration of topical application the stronger was the phototoxic effect. But topical application time exceeding 6 hours failed to increase phototoxic effect furtber. 5. With 0.3% Meladoxen cream the thicker the applied cream the stronger was the phototoxic effect up to 25mg per cm, over which phototoxic effect decreased, probably due to the screening effect of the cream, It is concluded that in the photochemical treatment of vitiligo and psoriasis psoralen sould be applied for 80 minutes, the tar for 6 hours or ovemight before irradiating the lesions with long wave ultraviolet rays. In this way the time required for each session of photochemical treatment of psoriasis or vitiligo will be greatly reduced while obtaining maximum effect. This is more significant in the view of long term treatmant required for the management of these embarrasing dermatoses.

10.A Study on the Tzanck Test of Vesicobullous Skin Diseases.

For the more practical and convenient application of the Tzanck test, we evaluated several staining methods and cytological findings 98 cases of vesicobullous skin diseases. Fixation by air-drying of specimen aad modified Giemsa staining method were thought to be the most appropriate procedure in clinical use. The disease studied were chickenpox, herpes zoster & simplex, contact dermatitis, Id reaction, tinea pedis, urticaria pigmentosa, incontinentia pigmenti, farnilial benign pemphigus, hand-foot-mouth disease, erytherna multiforme, toxic epiderrzal necrolysis, fixed drug eruption, and septic vesicle. Among these, herpes and chickenpox vesicles shawed the most characteristic and specific findings. And degenerating giant cell could be added as an useful diagnostic marker for these diseases in relatively old lesions. In the other diseases, cytological findings were neither charcteristic nor specific. But these findings suggested the possibility of picking up useful findings by more elaborated study including more large number of cases.
