1.Changes of Brachial Arterial Doppler Waveform during Immersion of the Hand of Young Men in Ice-cold Water.

PURPOSE: To evaluate the changes of brachial arterial Doppler waveform during immersion of the hand of young men in ice-cold water. MATERIALS AND METHODS: Doppler waveforms of brachial arteries in 11 young male patients were recorded before and during immersion of ipsilateral hand in ice-cold water(4--5 degree C). The procedure was repeated on separate days. Patterns of waveform during immersion were compared with the changes of pulsatility index. RESULTS: Four men showed high impedance waveforms, and 5 men showed low impedance waveforms during immersion both at the first and at the second study. Two men, however, showed high impedance waveforms at the first study and low impedance waveforms at the second study. The pulsatility index rose and fell in high and low impedance waveforms, respectively. CONCLUSION: The changes of brachial arterial Doppler waveforms could be classified into high and low impedance patterns, probably reflecting the acute changes in downstream impedance during immersion of hand in ice-cold water.

4.Doppler Sonographic Evaluation of Ophthalmic Arterial Flow Pattern in Hypertensive Patients.

PURPOSE: To compare the Doppler velocity waveform pattern of ophthalmic artery of hypertensive patients with that of normotensive subjects. MATERIALS AND METHODS: Doppler velocity waveform was obtained from ophthalmic artery in 45 hypertensive patients and 60 normotensive subjects. Both hypertensives and normotensive subjects were classified according to age into those younger than and those older than 45 years. Doppler indices[pulsatility index(PI), resistance index(RI), the first systolic peak/the second systolic peak(S1/S2), the first sytolic peak/diastolic peak (S1/D)] measured in hypertensive patients were compared with normotensive subjects. RESULTS: Among the various doppler indices, only S1/S2 showed significant difference(p < 0.05) between the hypertensive patients and normotensive subjects younger than 45 years. Doppler velocity waveform of hypertensive patients older than 45 years showed no significant difference from that of normotensive subjects with corresponding age. CONCLUSION: Doppler velocity waveform of ophthalmic artery in hyopertensive patients younger than 45 years shows pattern with S2 higher than that of normotensive subjects. High S2 component(reflective-wave) may represent increased vascular impedance ~ue to vasococonstriction of retinal arterioles in hypertensve patients.

5.Prevalence of Penicillinase-Producing Neisseria Gonorrhoeae (PPNG) in Seoul (1995).

BACKGROUND: In recent years gonorrhea has been pandemic and remains one of the most common sexually transmitted diseases(STD) in the world, especially in developing countries. OBJECTIVE & METHOD: For the detection of a more effective therapeutic regimen and to assess the prevalence of penicillinase-producing Neisseria gonorrhoeae (PPNG), we have been studying patients who have visited the VD Clinic of Choong-ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. RESULTS: In 1995, 97 strains of N. gonorrhoeae were isolated, among which 41(42.3%) were PPNG. CONCLUSION: Since the prevalence of PPNG was reported as 21.9% in 1981, the rate fluctuated. However, in 1993, there was an increase up to 74.3%. Thereafter, the prevalence rate decreased to 64.3% in 1994 and 42.3% in 1995. These changes will therefore need continuous observation.

7.Drug-Drug Interactions: Mood Stabilizers and Anti-Anxiety Drugs.

Pharmacotherapy of bipolar disorder is a rapidly evolving field. Mood stabilizers and anticonvulsants have varying biochemical profiles which may predispose them to different adverse effects and drug-drug interactions. Several of the new anticonvulsants appear less likely to have the problems with drug-drug interaction. To provide more effective combination pharmacotherapies, clinicians should be allowed to anticipate and avoid pharmacokinetic and pharmacodynamic drug-drug interactions. We reviewed the role of cytochrome P450 isozymes in the metabolism of the drugs and their interactions. The drug-drug interactions of several classes of drugs which used as mood stabilizers and new anticonvulsants, some of which may have psychotropic profiles, are discussed mainly in this article. Finally, potential pharmacokinetic interactions between the benzodiazepines and other coadministered drugs are discussed briefly.

8.The Study of Talar Tilt Angle of Ankles in Korean Adult

An appreciation of the normal variation of the Talar Tilt angle under stress roentgenogram on the ankle joint may throw some light on the diagnosis and treatment in an ankle joint injuries. The purpose of this study is to show the normal range of the Talar Tilt angleof the ankle joint under various circumstances and has been to compare the results of each angle. The author have undertaken the neutral ankle mortise view and inversion and eversion stress roentgenogram in all ankles and measured its Talar Tilt angle. The data were based on study of free chosen 50 normal Korean adult male 25 and female 25 respectively. The results were summerized as follows; 1. Talar Tilt angle was O degree in neutral ankle mortise view. 2. In inversion stress roentgenogram, Talar Tilt angle ranged from 0 to 15 degrees, 97% of cases were less than 10 degrees. 3. In eversion stress roentgenogram, Talar Tilt angle ranged from 0 to 7 degrees. 4. In inversion stress roentgenoram, the difference between right and left of same male and female ranged from 0 to 7 degrees. 5. It is very important to check accurately Talar Tilt angle under the same method and same conditions.

9.Salmonella Osteomyelitis of the patella: A Case Report

A 48 years old male was visited with complaining of painful swelling of the left knee joint for 2 weeks duration. On radiological finding of the patella, there was found local rarefaction and osteolytic change in the inferior pole. It was diagnosed as Salmonella osteomyelitis of the patella and was confirmed by bacteriological study. Osteomyelitis of the patella caused by Salmonella group C as an etiological factor has not been reported. It was treated by conservative method with satisfactory result.

10.The Change of the Mechanoreceptor fo Anterior Cruciate Ligament after Injuries of Medial Articular Cartilage and Ligament in the Rabbit.

The aim of this study was to determine whether there are quantitative changes in the mechanoreceptors in the anterior cruciate ligament of unstable knee with injured medial articular cartilage or medial ligament. Twenty specimens of anterior cruciate ligament were obtained from medial articular cartilage injury group and medial ligament injury group, respectively. Forty anterior cruciate ligaments of the other side of knee were used as controls. The ligaments were transected and stained with a modified gold-chloride method. Mechanoreceptors were located almost exclusively in the periligamentous synovial tissue. The results were as follows; 1. Three kinds of mechanoreceptors were found at the surface of each ligament beneath the synovial membrane. 2. The type 2 mechanoreceptor was more common than any other types in all groups. 3. In the medial articular cartilage injury group, the number of mechanoreceptor was significantly decreased and morphologically degradated with synovial degeneration than the control group. The same findings were noted in the medial ligament injury group(P<0.003). 4. There was no statistical difference between the two experimental groups in numher of the mechanoreceptor. In conclusion, the mechanoreceptor of the anterior cruciate ligament of unstable knee with injured medial articular cartilage and ligament in the rabbit were decreased in its numher and were degradated morphologically than the control group. And it was suggested that reduced proprioception in unstable knee might he an another cause of development of osteoarthritis.
