1.Efficacy and safety of BiPAP ventilator combined with nikethamide for the chronic obstructive pulmonary disease complicated with type II respiratory failure

Objective To observe the efficacy and safety of BiPAP ventilator combined with nikethamide for the chronic obstructive pulmonary disease (COPD)complicated with type II respiratory failure.Methods 76 COPD patients complicated with type II respiratory failure were selected as the research subjects,they were randomly divided into two groups by single blind and randomized method,38 cases in each group.The control group was treated with nikethamide therapy,while the observation group used BiPAP combined with mechanical ventilation therapy.The clinical therapeutic effect and adverse reactions were analyzed and compared in the two groups.Results The total effective rate in the observation group was 92.11%,which was significantly higher than 68.42% in the control group (χ2 =10.54,P <0.05).After treatment,the HR and RR of the observation group were (94.28 ±7.62)/min, (13.27 ±4.41)/min,which were significantly decreased than before treatment (t =8.02,8.04,all P <0.05),and which were significantly lower than those in the control group (t =7.54,6.88,P <0.05).After treatment,the pH (7.39 ±0.06),PaO2 (96.31 ±5.44)mmHg,SaO2 (98.42 ±6.87)% in the observation group were significantly increased (t =7.56,7.76,7.45,all P <0.05),PaCO2 (57.36 ±4.98)mmHg was significantly decreased (t =7.54, P <0.05),which had significant differences with the control group (t =6.56,7.02,6.73,7.32,all P <0.05).There was no significant difference in the incidence of adverse reactions between the two groups (χ2 =2.14,P >0.05). Conclusion BiPAP ventilator combined with nikethamide meters has remarkable curative effect and high safety in treatment of COPD patients complicated with type II respiratory failure,which is worthy of clinical reference and popularization.
3.Transcatheter arterial chemoembolization therapy combined three-dimensional conformal radiotherapy Clinical observation of primary liver cancer

Objective To evaluate the clinical effect of transcatheter arterial chemoembolization (TACE) combined with three-dimension conformal radiotherapy (3DCRT) in treating unresectable primary Hepatic carcinoma(PHCC).Methods 64 cases of PHCC were randomly divided into 2 groups,TACE combined with 3DCRT group (Treatment group) and TACE alone group (control group).34 cases in treatment group were treated with TACE 2 ~3 times at first,after resting for 2-4 weeks,3DCRT treatment was used.30 cases in control group were treated with TACE 2 ~ 3 times only.Results The percentage of tumor regression rate (CR + PR) was 73.5% in treatment group and 36.6% in control group.The difference was significant (P <0.01).The percentage of AFP declined was 100% in treatment group and 92.9% in control group,which difference was not significant (P >0.05).The cumulative survival rate of 1-,2-and 3-years were 76.5%,58.8% and 41.2% in treatment group,and 56.7%,26.7%,13.3% in control group respectively.There was significant difference between the two groups (P < 0.05).Conclusion The treatment of TACE combined with 3DCRT is an effective and security treatment method for primary Hepatic carcinoma.
4.Analysis of the clinical efficacy and safety of intravenous sequential oral voriconazole treatment of blood dis-eases combined invasive fungal infections

Objective To investigate the clinical efficacy and security of intravenous sequential oral voricon -azole treatment of blood diseases combined invasive fungal infections (IFI).Methods Combined blood of patients were reated with invasive fungal infections ,88 patients were randomly divided into the control group and treatment group using sequential treatment , the control group were treated with conventional anti-infective therapy , treatment group based on the use of conventional anti-infective drugs ,plus implementation of voriconazole therapy .The clinical efficacy were compared and adverse drug reactions were observed .Results The total effective rate of the treatment group was 90.91%,which was significantly higher than 70.45%of the control group (χ2 =5.906,P<0.05);the in-fection control in patients treated with time ,continuous medication and incidence of adverse events of the treatment group were significantly lower than those of the control group (t=2.019,P<0.05;t=1.987,P<0.05;χ2 =4.062, P<0.05).Conclusion Intravenous sequential oral voriconazole therapy combined blood invasive fungal infections in patients is safe ,effective,fewer adverse reactions ,which is widely used clinically .
5.Echinococcosis Control in China:Challenges and Research Needs

Based on a review of the practices of echinococcosis control in China and of the historical experience from the international community in the field,this paper discussed the research achievements and challenges.The author indicated that it is a realistic time to launch a comprehensive control program in western China and there are urgent needs for scientific research integrating into the program.
6.Comparative study the single drug treatment with Oxcarbazepine and Carbamazepine on children with partial epilepse

The 2 cases in Carbamazepine group appeared severe complications.Conclusions There are significant curative effects of single drug treatment with Oxcarbazepine and Carbamazepine on new diagnostic children with partial epilepse,and the adverse reaction and tolerance are similar. The severe complication is not appeared in Oxcarbazepine group.

Thyroid associated ophthalmopathy (TAO) is an organ-specific autoimmune-inflammatory disorder,ranking top among the orbital diseases with highest incidences.TAO is characterized by a complex pathogenesis,including glycosaminoglycan and hyaluronic acid deposition,fibrosis of extraocular muscle and adipose degeneration of ocular tissue.The physical manifestations of the disease include periorbital edema,double vision,ophthalmoptosis,ocular dryness,eyelid retraction and on occasion vision loss secondary to optic nerve compression.Selenium evolves in the diversified pathogenesis of TAO.Although the therapeutic effects for TAO of selenium have already been proven,some problems appear to be no clear among the clinicians,such as its optimum dose,duration of the treatment,and safety to date,etc.Nowadays,the mechanism of selenium supplementation in TAO treatment includes neutralization of reactive oxygen,inhibition of HLA-DR expression,inhibition of secretion of TSHR-Ab and TPO-Ab.The aim of the review was to summarize the studies of selenium supplementation theray for TAO.

two types of osseous defects, i.e. dehiscence and
fenestration that arise during the placement of dental
implant in the edentulous area (delayed implant
placement). A systematic online search of main
database from 1975 to 2009 was made. Five
randomised controlled trials have been identified based
on the inclusion criteria. Different management
procedures were identified, in which guided bone
regeneration procedure was most commonly advocated.
Resorbable and non-resorbable m'embranes were
compared, in which resorbable membrane was
preferred as it caused less complicatiQn of membrane
exposure or risk of infection. The benefit of using bone
substitute along with membrane in rypairing bony
defects cannot be concluded.

10.Perioperative Nursing in Dissection Aortic Aneurysm Patients Receiving Blood Vessel Prosthesis

Objective To assess perioperative nursing potential effects on dissection aortic aneurysm cases receiving blood vessel prosthesis. Methods In order to prevent the aortic aneurysm rupture, pain degree and mental condition were evaluated in 28 patients with dissection aortic aneurysm before therapy. Blood pressure was also controlled in proper level. All were received blood vessel prosthesis. Postopertive care were performed for every organism system, controlling blood pressure and preventing complication. Results After perioperative nursing for (3.3?2.2) days, 27 cases were recoveried from illness while 1 case died from MODF.Conclusions Perioperative nursing play an important role in blood vessel prosthesis.
