Lowe et al. have described a disease characterized by organic aciduria, decreased renal ammonia production, hydrophthalmos, and mental retardation, this disease is now called as Lowe's syndrome or oculo-cerebro-renal syndrome which manifests growth plates, metabolic acidosis, and characteristic eye changes, including bilateral cataract, glaucoma and nystagmus. It is a congenital hereditary affectation. Most of the patients with this syndrome are male sex. We report a new case of Lowe's syndrome in a 5-year old girl.

4.Effects of Substance P on the Hair Growth in Human Hair Follicle Organ Culture.

BACKGROUND: In vitro, some neuropeptides, including substance P(SP), act as a growth factor. The cyclic growth of the richly innervated hair follicle offers a model for probing such functions in a complex, developmentally regulated tissue interaction system under the physiologic condition. Dissecting the role of neuropeptides in this system may also reveal as yet obscure neural mechanisms of hair growth control. OBJECTIVE: The purpose of this study was to investigate the effect of SP on human hair growth using a recently described model in which isolated hair follicles are grown in vitro. METHODS: After the healthy human hair follicles without any visible damage were collected, they were cultured in DMEM with several combination of supplements including insulin, hydrocortisone, sodium selenite, human transferrin, fetal bovine serum at 37 degrees C in an atmosphere of 5% CO2 /95% air incubator, and SP was added to the media. The culture media were supplemented with final concentration of 10(-6),10(-7),10(-8) M SP dissolved in DMEM. The results were evaluated by measuring linear hair fiber growth and hair follicle morphology on light microscopy and electron microscopy and by measuring radioisotope uptake of [methyl-3H] thymidine and [U-14C] leucine of hair follicle. RESULTS: The following results were obtained from this study. 1. SP did not have an statistically significant effect on the rate of linear hair growth in cultured hair follicles. However, it prolonged the anagen stage of hair cycle. 2. We could not find morphological differences of hair follicles cultured in SP groups compared with those cultured in control group. 3. DNA and protein synthesis in hair follicles increased steadily for 5 days of culture. CONCLUSION: From the results, we can conclude that SP has growth-stimulatory effect and especially prolongs the duration of anagen phase without affecting the rate of linear hair growth.

5.A study on the initial tissue changes to intermittennt force in dogs.

The purpose of this study was to help examining force system, optimal treatment, and prevention of relapse. Orthodontic force was generated by electromagnets, therefore duration could be freely controlled, and applied for 4 days in dogs. Force magnitude was 200gm and 50gm. duration was contious and intermittent. Intermittent duration was divided into 2 kinds of types in on/off, 1 mininute/1 minute, 10 seconds/1 minute. The results were as follows: 1. In the intermittent group to which force was applied for 10 seconds and ceased for 1 minute, osteoclast was not observed. 2. In the intermittent group to which 200gm of force was applied for 1 minute and ceased for 1 minute, there was blood circulation in periodontal space on pressure side, but mild hyalinized zone was observed. 3. More number of total osteoclast was counted in the pressure side of continous group to which 200gm of force was applied than in any other groups. 4. More number of frontal osteoclast was counted in the pressure side of intermittent group to which 200gm was applied for 1 minute and ceased for 1 minute than in any other groups. 5. On tension side, less periodontal space widening, more new bone and secondary cementum formation were observed in the intermittent group to which force was applied for 1 minute and ceased for 1 minute than in the continous group.

7.Lumbar HNP: Analysis of the MRI finding and comparative study between MRI and Operative finding

Among the 413 patients of the lumbar herniated nucleus pulposus(HNP) who were admitted and treated at Dong-A University Hospital from March 1990 to May 1994, we analyzed 387 patients who checked magnetic resonance imaging(MRI). MRI study followed by open disectomy was carried out on 127 patients of the lumbar HNP, and we analyzed the clinical and operative findings of the lumbar HNP and compared these with MRI findings. The following results were obtained. 1. MRI findings of 387 patients consisted of bulging discs in 107 cases(27.6%), protruded discs in 165 cases(42.6%), extruded subligamentous discs in 73 cases(18.9%), extruded transligamentous discs in 34 cases(8.8%) and sequestered discs in 8 cases(2.1%). 2. MRI findings of 127 patients who had been treated with open discectomy consisted of protruded discs in 53 cases(41.7%), extruded subligamentous discs in 47 cases(37.0%), extruded transligamentous discs in 19 cases(15.0%) and sequestered discs in 8 cases(6.3%). 3. Operative findings revealed the protruded discs: 49 cases(38.6%), extruded subligamentous discs: 52 cases(40.9%), extruded transligamentous discs: 18 cases(14.2%) and sequestered discs: 8 cases(6.3%). 4. In sequestered disc, we could get the highest sensitivity, specificity and accuracy with MRI in distinguishing from another type of lumbar HNP. MRI can be available for accurate assessment of type and location of the lumbar HNP and it can show the high specificity, sensitivity and accuracy in preoperative diagnostic method.

8.Clinical Aspects of Infectious Endogenous Endophthalmitis.

Infectious endogenous endophthalmitis is a relatively rare, but seriously devasting disease. To evaluate clinical aspects of this disease, we performed a retreospective study on 7 eyes of 7 patients who had been treated with this diagnosis from July 1994 to January 1998 at Asan Medical Center. The preceding systemic diseases consisted of liver abscess, non-small cell lung cancer, spontaneous bacterial peritonitis, acute cholangitis, cystitis, systemic candidiasis. Microorganisms were recovered in blood culture in 4 of them(3 Klebsiella pneumoniae and one Candida albicans). One among these patients also revealed culture positive for Klebsiella pneumoniae in the vitreous as well as in his blood. All patients received intravitreal vancomycin and amikacin injection following vitreous and aqueous tapping. Four patients underwent pars plana vitrectomy as the second procedure and 2 of them expired. At the final follow up, retina remained attached succesjully in six of 7 eyes, yet the visual outcome was so poor that only two recovered over 5/200 or better. Infectious endogenous endophthalmitis has guarded prognosis since causative microorganisms are often extremely virulent, detection of this disease is likely to be delayed and appropriate management can not be started immediately. High index of suspicion for the diagnosis and the intensive treatment are strongly suggested to achieve the best result.

9.Prevalence and Risk Factors of Peripheral Artery Disease in Elderly Chronic Ischemic Stroke Patients: A Study of a Single Geriatric Hospital.

BACKGROUND: Peripheral artery disease (PAD), which is caused by atherosclerosis of the peripheral vessels, is a progressive disease, especially in patients with many of the vascular risk factors. The aim of this study was to investigate the prevalence and risk factors of PAD in elderly chronic stage ischemic stroke patients in Korea. METHODS: Ninety-six elderly patients (> or =60 years) admitted to a local geriatric hospital with ischemic stroke in the chronic stage (mean duration of stroke, 590 days) were included in this study. Vascular risk profiles, routine blood tests, severity of stroke and ankle-brachial index (ABI) were measured. RESULTS: Of the 96 patients, 55 (57.3%) had PAD (ABI<0.9) of which 6 patients (6.3%) had severe PAD (ABI<0.5) and 22 patients (22.9%) had bilateral PAD. PAD was more frequently observed in patients with diabetes (65% vs. 15%, p=0.001), hypercholesterolemia (65% vs. 15%, p<0.001), previous history of coronary artery disease (CAD) (25% vs. 5%, p=0.01) and recurrent ischemic stroke (36% vs. 12%, p=0.01). The means of the total cholesterol and blood urea nitrogen were higher in patients with PAD. From the results of the binary logistic regression analysis, presence of diabetes (odds ratio [OR], 4.740; p=0.026), hypercholesterolemia (OR, 18.991; p<0.001), previous history of CAD (OR, 7.579; p=0.038) and lower Korean version of Modified Barthel Index (OR, 0.971; p=0.009) were independent risk factors for PAD. CONCLUSION: PAD is frequently observed in elderly patients with chronic ischemic stroke. Measuring ABI to diagnose PAD and controlling risk factors may be beneficial in these patients seen at geriatric hospitals and centers.

10.A study of Cytokine in Peritoneal Fluid of Infertile Patients with Endometriosis.

OBJECTIVE: The presence of the various cytokines in human peritoneal fluid has been evaluated incompletely. Changes in cytokine lefels may be related to activation of peritoneal macrophage and T-lymphocyte, development of endometriosis, and infertility. This study assesses peritoneal fluid levels of interleukin-6 (IL-6), interleukin-8 (IL-8), interleukin-10 (IL-10) and tumor necrosis factor-alpha (TNF-alpha) in infertile women with endometriosis and normal women without endometriosis. Design : Prospective and case-control study in university hospital. MATERIALS AND METHODS: Cytokine levels in peritoneal fluid obtained during laparotomy or laparoscopy from 21 patients in infertile patients in infertile patients with endometriosis and 24 controls undergoing laparotomy or laparoscopy with no evidence of pelvic endometriosis were determined by enzymelinked immunosorbent assay. RESULTS: The mean levels of interleukin-6 in infertile patients with endometriosis and controls were 72.7+/-23.7 pg/ml and 18.5+/-9.7 pg/ml respectively (p=0.02). Similarly, the mean levels of interleukin-8 in infertile patients with endometriosis was significantly higher than that of controls (445.0+/-89.6, vs 45.1+/-48.4, p=0.04). The mean concentration of interleukin-10 in infertile patients with endometriosis was significantly lower than that of controls (1.09+/-0.04 vs 2.19+/-0.03, p=0.03). The level of tumor necrosis factor-alpha was not significantly different between the two study groups. CONCLUSIONS: Increased IL-6 and IL-8 and decreased IL-10 levels in the peritoneal fluid may be related to pathogenesis in the endometriosis and infertility, suggesting that partially contribute to the disturbed immune regulation observed in infertili women with endometriosis.
